    본 연구는 처음 암을 진단받은 환자의 불안, 사회적 지지 및 극복력 수준을 파악하고, 이들 요인이 극복력에 미치는 영향을 규명하고자 하였다.
    본 연구는 서울시 소재 A상급종합병원 종양내과 외래에 내원한 처음 암을 진단받은 환자 162명을 대상으로 2025년 4월 1일부터 6월 30일까지 수행된 서술적 조사연구이다. 불안은 Hospital Anxiety and Depression Scale- Anxiety(HADS-A), 사회적 지지는 Multidimensional Scale of Perceived Social Support(MSPSS), 극복력은 Connor-Davidson Resilience Scale(CD- RISC)을 사용하여 측정하였다. 수집된 자료는 기술통계, independent t-test, one-way ANOVA, Pearson 상관분석, 다중회귀분석으로 분석하였다.

    본 연구의 결과는 다음과 같다.

    1. 대상자의 불안은 평균 6.84±5.22점(21점 만점)으로 정상 범위가 103명(63.6%), 경증 불안이 29명(17.9%), 중증 불안이 30명(18.5%)이었다. 사회적 지지는 평균 43.31±7.78점(60점 만점)이었으며, 하위 영역별로 살펴보면, 가족 지지가 18.54±2.27점으로 가장 높았고 의료진 지지가 8.83±5.19점으로 가장 낮았다. 극복력은 평균 75.96±16.65점(100점 만점)이었다.

    2. 일반적 특성과 질병 관련 특성에 따른 각 변수의 차이는 다음과 같다. 불안은 성별(t=-3.148, p=.002)과 종교 유무(t=3.496, p<.001)에서 통계적으 로 유의한 차이가 나타났다. 사회적 지지는 교육 수준(t=-2.571, p=.011)에 서만 유의한 차이를 보였다. 극복력은 결혼 상태(F=6.073, p=.003)와 종교 유무(t=-2.615, p=.010)에서 유의한 차이가 있었다.

    3. 상관분석 결과, 극복력은 불안과 유의한 음의 상관관계(r=-.408, p<.001), 사회적 지지와는 유의한 양의 상관관계(r=.303, p<.001)를 보였다.

    4. 처음 암을 진단받은 환자의 극복력에 유의한 영향을 미치는 요인은 불안(β=-.343, p<.001), 결혼 상태에서 기혼인 경우(β=.305, p<.001), 사회적 지지(β=.242, p<.001)순으로 확인되었다. 회귀모형은 통계적으로 유의하였고(F=12.897, p<.001), 설명력은 27.0%로 나타났다.

    본 연구는 처음 암을 진단받은 환자의 극복력에 불안과 사회적 지지가 중요한 영향 요인임을 확인하였으며, 특히 불안 고위험군의 조기 선별과 가족, 친구 및 의료진과의 상호작용을 통한 사회적 지지 강화의 필요성을 제시한다. 본 연구 결과는 진단 초기 외래에서 심리적 취약군을 조기에 선별하고 극복력 증진을 위한 정서적, 지지적 간호중재 개발의 근거 자료로 활용될 수 있을 것으로 기대된다.

    This descriptive study aimed to identify the levels of anxiety, social support, and resilience in patients newly diagnosed with cancer and to examine the factors influencing their resilience. The findings provide foundational data for developing early nursing interventions to support psychological adaptation during the initial diagnostic phase.

    A total of 162 patients who first visited the outpatient oncology clinic at a tertiary hospital in Seoul were recruited. All participants were within two months of diagnosis and had not yet initiated treatment. Anxiety was measured using the anxiety subscale of the Hospital Anxiety and Depression Scale (HADS-A), social support using the Multidimensional Scale of Perceived Social Support (MSPSS), and resilience using the Connor–Davidson Resilience Scale (CD-RISC). Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficients, and multiple regression analysis.

    The major findings of this study were as follows.

    1. The mean anxiety score of the participants was 6.84±5.22, with 36.4% exhibiting clinically significant anxiety (mild to severe). The mean social support score was 43.31±7.78, with family support rated highest (18.54±2.27) and medical staff support lowest (8.83±5.19). The mean resilience score was 75.96±16.65.

    2. Anxiety showed significant differences according to gender (t=-3.148, p=.002) and religion (t=3.496, p<.001). Social support differed significantly by educational level (t=-2.571, p=.011). Resilience showed significant differences by marital status (F=6.073, p=.003) and religion (t=-2.615, p=.010).

    3. Resilience was negatively correlated with anxiety (r=-.408, p<.001) and positively correlated with social support (r=.303, p<.001).

    4. Multiple regression analysis revealed that anxiety (β=-.343, p<.001), being married (β=.305, p<.001), and social support (β=.242, p<.001) were significant predictors of resilience, explaining 27.0% of the variance (F=12.897, p<.001).

    This study confirms that anxiety and social support are critical predictors of resilience in patients newly diagnosed with cancer. High levels of anxiety may deplete the emotional energy required for coping, while social support serves as a vital psychological foundation for adaptation. These findings underscore the need for early anxiety screening and the development of nurse-led emotional support and integrated psychosocial interventions. Furthermore, longitudinal studies are recommended to track the trajectory of resilience and to establish stage-specific intervention strategies from the point of diagnosis.
