    Objective
    Traumatic brain injury (TBI) can cause disturbances in hypothalamic–pituitary–adrenal (HPA) axis function, yet the prognostic significance of hypocortisolism in severe TBI remains unclear. This study aimed to evaluate the impact of hypocortisolism on clinical outcomes by analyzing serial cortisol and adrenocorticotropic hormone (ACTH) levels in patients with severe TBI.
    Methods
    This retrospective observational study included patients with severe TBI (initial Glasgow Coma Scale < 9) who were admitted to a single regional trauma center between January 2019 and December 2021. Serum cortisol and ACTH levels were measured at admission and on hospital days 3 and 7. Patients were categorized into a normal cortisol group and a hypocortisolism group based on the cortisol level on hospital day 3. Clinical characteristics and outcomes were compared between the groups. Univariate and multivariate logistic regression analyses were conducted to identify factors independently associated with in-hospital mortality.
    Results
    Of 162 screened patients, 87 met the inclusion criteria, including 9 (10.3%) who were classified as having hypocortisolism. Initial cortisol levels did not differ significantly between groups; however, a marked divergence was observed by hospital day 3 (normal: 12.85 ± 6.50 µg/dL vs. hypocortisolism: 3.45 ± 1.32 µg/dL; p = 0.001), with partial recovery by day 7. ACTH levels showed similar temporal patterns, with consistently lower values in the hypocortisolism group. During hospitalization, 24 patients (27.6%) died. Multivariate analysis demonstrated that unilateral pupil dilation, bilateral pupil dilation, shock, hypocortisolism (OR 29.608, 95% CI 3.872–226.401), and barbiturate coma therapy were independent predictors of mortality.
    Conclusion
    Hypocortisolism is a significant and independent predictor of mortality in patients with severe TBI. Serial hormonal measurements indicate that hospital day 3 represents the physiological nadir of adrenal function, highlighting the importance of early identification and continued monitoring of adrenal dysfunction in the management of severe TBI.

    연구방법
