    This descriptive survey study was conducted to identify educational needs by analyzing the current level of performance and perceived importance of educational items related to oncologic emergencies among emergency department nurses in the Busan, Ulsan, and Gyeongnam regions, and to provide foundational data for the future development of education programs related to oncologic emergencies based on the findings.
    The study participants were 131 emergency department nurses working at regional emergency medical centers, local emergency medical centers, and local emergency medical institutions located in Busan, Ulsan, and Gyeongnam, all of whom had at least 12 months of experience working in an emergency department.
    Data were collected using a self-administered questionnaire from September 8 to September 26, 2025, and the questionnaire used in this study was developed by the researcher based on a literature review and expert consultation.
    The collected data were analyzed using IBM SPSS Statistics version 24.0, employing descriptive statistics, independent t-tests, paired t-tests, and one-way analysis of variance, and Borich’s needs assessment and the Locus for Focus model were applied to determine priorities for educational needs.

    The main findings of this study are as follows.
    1. The mean age of the participants was 30.24 ± 4.37 years, and their mean length of emergency department work experience was 5.28 ± 3.54 years. Among the participants, 82 nurses (62.6%) were working at regional emergency medical centers, 39 nurses (29.8%) at local emergency medical centers, and 10 nurses (7.6%) at local emergency medical institutions, and 81 nurses (61.8%) were employed at institutions
    with an in-hospital cancer center. Only 11 participants (8.3%) reported having previously received education related to oncology, while 106 participants (80.9%) reported that additional education was necessary.
    2. The mean performance level for educational items related to oncologic emergencies was 3.15 ± 0.48 points out of 5. By domain, the performance level was highest in the nursing skills domain (3.33 ± 0.58), followed by the module domain (3.09 ± 0.57) and the disease domain (3.05 ± 0.57). The mean importance level was 3.66 ± 0.60, with the disease domain scoring highest (3.69 ± 0.63), followed by the nursing skills domain (3.66 ± 0.70) and the module domain (3.63 ± 0.68). Analysis of differences between performance and importance levels showed statistically significant differences in 11 out of 12 items in the module domain, 19 out of 20 items in the disease domain, and 9 out of 15 items in the nursing skills domain.
    The items with the largest differences were “management of thromboembolism in cancer patients” in the module domain, “toxicities associated with CAR-T cell therapy” in the disease domain, and “assistance and management of cricothyrotomy” in the nursing skills domain.
    3. According to the results of Borich’s needs assessment, the highest-priority items were “management of thromboembolism in cancer patients” in the module domain, “toxicities associated with CAR-T cell therapy” in the disease domain, and “assistance and management of cricothyrotomy” in the nursing skills domain.

    Based on these results, application of the Locus for Focus model identified the highest priority (High–High) items as “management of thromboembolism in cancer patients” in the module domain and “assistance and management of pericardiocentesis,” “assistance and management of tracheostomy,” and “assistance and management of cricothyrotomy” in the nursing skills domain.
    The secondary-priority (High–Low) items were “management of bleeding in cancer patients” in the module domain, “cardiac tamponade” and “increased intracranial pressure” in the disease domain, and “transcutaneous cardiac pacing” in the nursing skills domain.
    This study identified educational needs related to oncologic emergencies among emergency department nurses using a multidimensional approach by categorizing educational content into module, disease, and nursing skills domains and by presenting priorities for each item.
    This approach aims to enhance the practicality of developing and implementing systematic education programs for emergency department nurses in diverse emergency care settings by reflecting educational modules for oncologic emergency situations, essential clinical diseases and theoretical knowledge, and required nursing skills.
    In addition, this study suggests the need for future research that reflects the type of emergency medical institution, regional characteristics, the presence of cancer centers, and the emergency department work environment. The results of this study may be used as foundational data for improving the clinical practice competence of emergency department nurses in oncologic emergencies and for designing future education programs.

    본 연구는 부산, 울산, 경남 지역의 응급실 간호사를 대상으로 종양학적 응급 관련 교육 항목에 대한 현재 수행도와 중요도를 분석하여 교육 요구도를 규명하고 이를 토대로 향후 종양학적 응급 관련 교육 프로그램 개발을 위한 기초 자료를 제공하고자 수행된 서술적 조사 연구이다. 연구대상은 부산, 울산, 경남 소재의 권역응급의료센터, 지역응급의료센터, 지역응급의료기관 에서 근무하고 있는 응급실 간호사 131 명으로, 12 개월 이상 근무경력이 있는 응급실 간호사들이었다.
    자료수집은 2025 년 9 월 8 일부터 9 월 26 일까지 자가 보고식 설문지를 이용하여 시행되었으며, 설문지는 문헌고찰과 전문가 검토를 통해 연구자가 직접 개발한 설문지를 사용하였다. 수집된 자료는 IBM SPSS statics 24.0 프로그램을 이용하여, 기술통계, 독립표본 t-검정, 대응표본 t-검정, 일원배치 분산분석으로 분석하였으며, 교육 요구도 우선순위를 도출하기 위해 Borich’s needs assessment 와 the Locus for Focus model 을 활용하였다.

    본 연구의 주요 결과는 다음과 같다.
    1. 연구대상자들의 평균 연령은 30.24±4.37 세였고, 이들의 응급실 근무경력은 평균 5.28±3.54 년이였다. 이들은 권역응급의료센터(82 명, 62.6%), 지역응급의료센터 (39 명, 29.8%), 지역응급의료기관(10 명, 7.6%)에 근무하고 있었으며, 병원내 암 센터가 있는 기관의 근무자는 81 명(61.8%)이었다. 종양학적 교육을 받은 적이 있다고 응답한 대상자는 11 명(8.3%)이었으며, 106 명(80.9%)이 추가 교육이 필요하다고 응답하였다. 2. 종양학적 응급 관련 교육 항목에 대한 이들의 수행도 수준은 3.15±0.48 점(5 점만점)으로 나타났으며, 영역별로는 간호 술기 영역(3.33±0.58 점)이 가장 높았고, 다음으로 모듈 영역 (3.09±0.57 점), 질병 영역 (3.05±0.57 점) 순이었다. 중요도 수준은 3.66±0.60 점으로 나타났으며, 질병영역 (3.69± 0.63 점), 간호 술기 영역(3.66± 0.70 점), 모듈영역 (3.63± 0.68 점)순 이였다. 수행도와 중요도 수준 차이를 분석한 결과, 모듈영역 항목 12 개 중 11 개, 질병영역 항목 20 개 중 19 개, 간호 술기 영역 항목 15 개 중 9 개에서 통계적으로 유의한 차이를 보였다. 가장 차이가 컸던 항목은 모듈영역에서 ‘암 환자의 혈전 색전증 상황 관리’, 질병영역에서는 ‘CAR-T 세포치료 독성’, 간호 술기 영역에서는 ‘윤상갑상 절개술의 보조 및 관리’였다. 3. Borich 요구도 분석 결과, 모듈영역에서는 ‘암 환자의 혈전 색전증 상황 관리’, 질병영역에서는 ‘CAR-T 세포 치료 독성’, 간호 술기 영역에서는 ‘윤상 갑상 절개술의 보조 및 관리’가 우선 순위가 가장 높게 나타났다.

    이러한 분석 결과를 토대로 the Locus for Focus model 을 적용한 결과, 최우선 순위 항목(High-High)은 모듈영역에서 ‘암 환자의 혈전 색전증 상황 관리’, 간호 술기 영역에서 ‘심낭 천자의 보조 및 관리’, ‘기관절개술의 보조 및 관리’, ‘윤상 갑상 절개술의 보조 및 관리’로 확인되었다. 차우선 순위 항목(High-Low)은 모듈영역에서 ‘암 환자의 출혈상황 관리’, 질병영역에서 ‘심낭압전’과 ‘두개내압 상승’, 간호 술기 영역에서 ‘경피적 심장박동조율’ 이였다.
    본 연구는 응급실 간호사들을 대상으로 종양학적 응급 관련 교육 요구도를 다차원적으로 파악하기위해 교육내용을 모듈, 질병, 간호 술기영역으로 분류하고, 각 항목의 우선순위를 제시하였다. 이는 종양학적 응급 관련 상황의 교육 모듈과 필요한 임상 질환 및 이론적 지식, 그리고 간호 술기를 반영하여 다양한 응급실 환경에서 응급실 간호사를 위한 체계적인 교육 프로그램 개발 및 현장 적용의 실용성을 높이기 위함이다. 또한 본 연구는 향후 응급의료기관 유형과 지역적 특성, 암 센터 유무 및 응급실 근무 환경을 반영한 후속 연구의 필요성을 제언하였다. 본 연구의 결과는 종양학적 응급 관련 응급실 간호사의 임상 실무 역량 향상과 향후 교육 프로그램 설계를 위한 기초 자료로 활용될 수 있을 것이다.
