    목적: 경추 후종인대 골화증(ossification of the posterior longitudinal ligament, OPLL) 수술에서 수술 중 운동유발전위(motor evoked potential, MEP) 감시는 널리 사용되고 있으나, 수술 중 MEP 감소의 임상적 의미는 여전히 논란이 있다. 본 연구의 목적은 수술 중 MEP 감소가 발생한 환자에서 감소 자체의 크기보다 감소 이후 회복 정도가 수술 후 조기 신경학적 결과와 더 밀접한 관련이 있는지를 평가하고, 이러한 연관성이 수술 접근법에 따라 차이를 보이는지를 탐색하는 데 있다.
    방법: 2018 년부터 2024 년까지 단일 기관에서 경추 OPLL 수술을 받은 164 명을 후향적으로 분석하였다. 이 중 수술 중 MEP 감소가 발생한 27 명(전방 접근 19 명, 후방 접근 8 명)을 분석 대상으로 포함하였다. MEP 변수로 Eventworst, Eventbest, ENDworst, ENDbest 를 산출하였으며, 회복지표 중에서는 EP 총대소회복지표(EPrec = ENDbest - Eventworst)에 중점을 두었다. 조기 수술 후 신경학적 결과는 Frankel 등급의 즉각적인 변화량(ΔNEximm)으로 평가하였다. 상관 분석, 선형 회귀 분석 및 탐색적 수신자조작특성곡선(ROC) 분석을 시행하였으며, 전방 접근과 후방 접근 간 하위군 분석을 수행하였다.
    결과: 수술 중 MEP 변수 중 회복 기반 지표가 감소 기반 지표보다 조기 신경학적 결과와 더 강한 연관성을 보였다. 특히 EPrec 는 ΔNEximm 과 가장 강한 상관관계를 나타냈으며(Spearman ρ = 0.590, R² = 0.341, p < 0.01), 반면 수술 중 최저 MEP 값(Eventworst)은 유의한 연관성을 보이지 않았다. 탐색적 ROC 분석에서 EPrec 는 조기 신경학적 결과를 구분하는 데 있어 비교적 뚜렷한 판별 신호(AUC = 0.85)를 보였으나, 표본 수가 적어 결과 해석에는 주의가 필요하였다. 상대적으로 회복 정도가 큰 경우 조기 신경학적 악화가 관찰되지 않는 경향이 있었으나, 본 연구에서는 이를 임상적으로 적용 가능한 절단값이나 예측 기준으로 제시하기에는 한계가 있었다. 하위군 분석에서 전방 접근군은 수술 중 MEP 감소에도 불구하고 전반적으로 더 양호한 신경학적 경과를 보였으며, 후방 접근군에서는 EPrec 와 신경학적 결과 간의 연관성이 상대적으로 더 강하게 나타났다.

    Objective: Intraoperative motor evoked potential (MEP) monitoring is widely used during cervical ossification of the posterior longitudinal ligament (OPLL) surgery, yet the clinical significance of MEP deterioration remains controversial. This study aimed to investigate whether neurological outcome is more closely associated with the degree of MEP recovery after intraoperative deterioration rather than with the magnitude of the deterioration itself, and to explore whether this relationship differs according to surgical approach. Methods: We retrospectively reviewed 164 patients who underwent cervical OPLL surgery at a single center between 2018 and 2024. Among them, 27 patients (19 anterior and 8 posterior approaches) experienced intraoperative MEP deterioration and were included in the analysis. MEP parameters were extracted from the most affected muscle leads and included Eventworst, Eventbest, ENDworst, and ENDbest. Recovery-based metrics were calculated, with particular focus on total recovery (EPrec = ENDbest - Eventworst). Early postoperative neurological outcome was assessed using the immediate change in Frankel grade (ΔNEximm). Correlation analyses, linear regression, and exploratory receiver operating characteristic (ROC) curve analyses were performed. Subgroup analyses were conducted to compare anterior and posterior approaches. Results: Among the evaluated intraoperative MEP parameters, recovery-based metrics demonstrated stronger associations with early neurological outcome than deterioration-based metrics. EPrec showed the strongest correlation with ΔNEximm (Spearman ρ = 0.590, R² = 0.341, p < 0.01), whereas the magnitude of intraoperative deterioration alone (Eventworst) was not significantly associated with outcome. Exploratory ROC analysis suggested that EPrec showed a potential discriminative signal for immediate postoperative neurological deterioration (AUC = 0.85); however, given the small sample size, this finding should be interpreted as hypothesis- generating rather than confirmatory. Although patients with relatively greater MEP recovery tended not to experience early neurological worsening, no definitive cutoff value could be established, and the sensitivity remained limited in this cohort. Subgroup analysis suggested approach-specific trends: anterior decompression was associated with more favorable neurological trajectories despite intraoperative MEP deterioration, whereas the association between EPrec and outcome appeared stronger in posterior procedures. Conclusions: In patients who experience intraoperative MEP deterioration during cervical OPLL surgery, the degree of MEP recovery appears to be more closely associated with early postoperative neurological outcome than the magnitude of the initial signal loss. Although definitive predictive thresholds cannot be established due to methodological limitations, these findings support a recovery-centered framework for interpreting intraoperative MEP changes. Larger, multicenter studies are required to validate the clinical utility of recovery-based MEP metrics and to clarify approach-specific differences.
