거대세포바이러스(Human cytomegalovirus, HCMV)는 헤르페스바이러스과(Herpesviridae)에 속하는 보편적인 바이러스로, 숙주 내에서 평생 잠복상태로 존재한다. 대부분의 선행 연구는 면역억제 환자에서의 재활성화 기전과 치료에 집중되어 왔으나, 면역정상 성인에서 잠복 CMV 감염의 정도와 이에 대응하는 CMV 특이적 면역반응을 정량적으로 평가한 연구는 제한적이다. 본 연구는 건강한 성인을 대상으로 연령에 따른 CMV 특이적 세포매개면역(cell-mediated immunity, CMI) 반응과 말초혈 단핵세포(peripheral blood mononuclear cells, PBMC) 내 잠복 CMV DNA 부하의 변화를 정량적으로 분석하고자 하였다. 40 세에서 79 세까지의 건강한 성인 80 명(각 연령대별 20 명)을 대상으로 하였다. CMV 특이 T 세포 반응은 pp65 및 IE-1 항원을 이용한 Interferon-γ ELISpot 분석으로 정량하였으며, PBMC 내 잠복 CMV DNA 는 UL55(gB) 유전자를 표적하는 Droplet Digital PCR(ddPCR)을 통해 절대정량하였다. 내부대조군으로 RPP30 유전자를 사용하였고, qPCR 을 병행하여 결과를 검증하였다. 통계 분석은 Kruskal–Wallis test, Dunn’s post hoc test, Spearman 순위상관분석을 이용하였다. IE-1 특이적 IFN-γ 분비 T 세포 반응은 연령과 유의한 상관관계를 보이지 않았으나, pp65 특이적 반응은 연령 증가에 따라 상승하는 경향을 보였다(p=0.0518). qPCR 에서는 모든 시료에서 CMV DNA 가 검출되지 않았지만, ddPCR 에서는 모든 연령대에서 잠복 CMV DNA 가 검출되었으며 연령 증가에 따라 유의하게 증가하였다(p=0.0002). 연령대별 중앙값은 40 대 7.7, 50 대 3.5, 60 대 22.2, 70 대 24.0 copies/10⁵ PBMCs 로 나타났으며, 60 세 이상에서 잠복 바이러스 검출 비율이 85–90%로 현저히 증가하였다. 본 연구는 면역정상 성인에서 CMV-specific T 세포 반응은 연령대 간 큰 차이를 보이지 않았던 반면, 잠복 CMV DNA 는 연령 증가에 따라 누적됨을 확인하였다. 이는 면역정상 성인에서도 CMV-specific 면역반응의 개인차가 크고, 고령에서의 잠복 바이러스 증가가 연령 관련 면역노화에 따른 면역감시 기능 저하와 연관될 가능성을 시사한다. ddPCR 기반 잠복 CMV DNA 정량은 잠복 바이러스 정량적으로 평가할 수 있어, 고령층 또는 면역저하 환자군에서 CMV 감염 고위험군을 예측할 수 있는 바이오마커로 활용될 수 있는 가능성을 제시한다.

Background: Human Cytomegalovirus (HCMV), a member of the Herpesviridae family, is a ubiquitous virus that establishes a lifelong state of latency within the host. While research has primarily focused on reactivation mechanisms in immunocompromised patients, quantitative evaluation of latent CMV infection characteristics and its immunological regulation in healthy adults has been insufficient. This study aimed to quantitatively assess the age-related changes in CMV-specific cell-mediated immunity (CMI) and the load of latent CMV DNA within peripheral blood mononuclear cells (PBMCs) in healthy adults.
Method: Eighty healthy adults, aged 40 to 79 years (20 per decade), were recruited. CMV-specific T-cell responses were quantified using IFN-γ ELISpot assays stimulated with pp65 and IE-1 overlapping peptides. The absolute quantification of latent CMV DNA in PBMCs was performed via Droplet Digital PCR (ddPCR) targeting the UL55 (gB) gene, with RPP30 as an internal control. Real-time PCR (qPCR) was conducted for comparative validation. Statistical analysis included the Kruskal–Wallis test, Dunn’s post hoc test, and Spearman rank correlation analysis.
Result: The IE-1 specific IFN-γ secreting T-cell response showed no significant correlation with age, whereas pp65 specific response exhibited a trend toward increase with aging (p=0.0518). While CMV DNA was undetectable by qPCR in all samples, latent CMV DNA was successfully detected across all age groups by ddPCR and showed a significant accumulation with increasing age (p=0.0002). Median DNA loads (copies/10⁵ PBMCs) were 7.7 (40s), 3.5 (50s), 22.2 (60s), and 24.0 (70s). Notably, the detection rate of latent virus markedly increased to 85–90% in subjects aged 60 and over.
Conclusion: In this study, CMV-specific T-cell responses in immunocompetent adults did not show substantial differences across age groups, whereas latent CMV DNA levels increased with aging. These findings indicate that CMV-specific immune responses vary widely among healthy adults and suggest that the age-associated rise in latent viral burden may be linked to diminished immune surveillance due to immunosenescence. Quantification of latent CMV DNA using ddPCR enables sensitive assessment of viral persistence and may serve as a useful biomarker for evaluating latent infection in older adults or in individuals who may require future immunosuppressive therapy.
