    배경; 자기공명췌담관조영술(MRCP)은 원발경화담관염(primary sclerosing cholangitis, PSC)의 질병 중증도 평가와 치료 반응 모니터링에 필수적인 영상 기법이다. ANALI 점수는 주로 전반적인 예후와 질병 부담을 반영하는 지표로 사용되어 왔으며, 최근 International Primary Sclerosing Cholangitis Study Group(IPSCSG)에서 구조화된 MRCP 판독 기준을 제안하였다. 그러나 두 평가 체계가 실제 임상 추적관찰에서 치료 반응을 얼마나 민감하게 포착하며, 판독자 간 재현성이 어떠한지에 대한 직접 비교는 부족하다.
    목적; PSC 환자에서 ANALI 점수와 IPSCSG MRCP 평가 기준의 판독자 간 재현성과 치료 반응 검출 능력을 비교·평가하는 것이다.

    결과; 세 판독자 모두에서 ANALI 점수는 추적 MRCP 에서 유의하게 증가하였으나(p = 0.002~<0.001), 호전으로 분류된 환자는 단 한 명도 없었다. 환자의 대부분은 안정(70.3–75.7%) 또는 악화(24.3–29.7%)로 분류되었다. 연속형 ANALI 점수의 재현성은 매우 우수하여 baseline ICC 는 0.690–0.892, 추적 ICC 는 0.776–0.934 였다. 반면, ANALI 기반 치료 반응(호전/안정/악화)의 판독자 간 일치도는 중등도 수준(κ = 0.584)에 그쳤다. 반대로 IPSCSG 평가에서는 **담관 확장(bile duct dilatation)**과 담관벽 비후(bile duct wall thickening) 항목에서 각각 2.7–18.9%, 10.8–16.2%의 의미 있는 호전 사례가 관찰되어 ANALI 에서 포착되지 않던 치료 반응을 감지할 수 있었다. IPSCSG 항목의 재현성은 항목별로 달랐으며, 담관 확장(κ = 0.759), 담관벽 비후(κ = 0.645), 문맥고혈압(κ = 0.634)은 높은 수준의 일치도를 보였다. 반면, 실질 변화(κ = 0.489)와 국소 간 병변(κ = 0.480)은 중등도, 담관 협착은 가장 낮은 일치도(κ = 0.391)를 보였다.
    결론; ANALI 점수는 연속형 지표로서 매우 높은 재현성을 보였으나, 치료 관련 호전을 전혀 감지하지 못해 단기간 치료 반응 평가보다는 질병의 전반적 부담 및 장기적 예후 평가에 더 적합한 지표임을 확인하였다. 반면, IPSCSG 의 담관 확장 및 담관벽 비후 항목은 실제 환자에서 호전을 포착하고 상당히 높은 판독자 간 일치도를 보임으로써 PSC 치료 효과를 평가하는 민감하고 의미 있는 MRCP 기반 바이오마커가 될 수 있음을 보여주었다. 향후 더 큰 규모의 전향적 연구가 필요하지만, IPSCSG 평가 기준은 PSC 임상시험에서 치료 반응 평가 지표로 유망한 역할을 할 것으로 기대된다.

    Background: Magnetic resonance cholangiopancreatography (MRCP) is essential for evaluating disease severity and monitoring treatment response in primary sclerosing cholangitis (PSC). The ANALI score is widely used as a global prognostic index, whereas the International Primary Sclerosing Cholangitis Study Group (IPSCSG) recently proposed structured MRCP reporting criteria. However, the comparative responsiveness and interobserver reliability of these two MRCP-based assessment systems during clinical follow-up remain unclear.
    Purpose: To compare the interobserver reproducibility and treatment-response detectability of the ANALI score and IPSCSG MRCP criteria in patients with PSC. Methods: This retrospective study included 34 patients with PSC who underwent baseline and follow-up MRCP within a two-year interval. Three abdominal radiologists independently evaluated all MRCP examinations using the ANALI score (continuous) and the IPSCSG reporting standards (categorical: improved, stable, worsening). Interobserver agreement for continuous ANALI values was assessed using single-measure intraclass correlation coefficients (ICC). Agreement for categorical treatment response using ANALI-derived response labels and IPSCSG parameters was assessed using Fleiss’ kappa.
    Results: ANALI scores significantly increased from baseline to follow-up for all readers (p = 0.002– <0.001), and no patient was classified as improved by any reader. Most cases were labeled as stable (70.3–75.7%) or worsening (24.3–29.7%). Interobserver reliability for raw ANALI scores was very good to excellent (baseline ICC = 0.690–0.892; follow-up ICC = 0.776–0.934), whereas agreement for ANALI-derived treatment-response categories was only moderate (κ = 0.584). In contrast, the IPSCSG criteria demonstrated detectable improvement in a subset of patients, especially in duct- specific parameters: improvement was identified in 2.7–18.9% for bile duct (BD) dilatation and 10.8– 16.2% for BD wall thickening. Interobserver agreement varied by parameter, with substantial agreement for BD dilatation (κ = 0.759), BD wall thickening (κ = 0.645), and portal hypertension (κ = 0.634), but only moderate or fair agreement for parenchymal change (κ = 0.489), focal liver lesions (κ = 0.480), and BD stricture (κ = 0.391).
    Conclusion: ANALI demonstrated excellent reproducibility but failed to detect any treatment-related improvement, confirming its role as a global prognostic marker rather than a short-term response metric. In contrast, IPSCSG duct-level parameters, particularly bile duct dilatation and wall thickening, captured measurable improvement and showed substantial interobserver agreement. These findings suggest that IPSCSG duct-specific MRCP features may serve as more sensitive and clinically meaningful imaging biomarkers for evaluating therapeutic efficacy in PSC, while ANALI remains well suited for assessing overall disease severity.
