This study was conducted to evaluate the therapeutic efficacy of the natural compound-based antiviral agent AlimenWOW under field conditions of Porcine Reproductive and Respiratory Syndrome Virus (PRRSV) infection, and to verify the improvement of respiratory health using an artificial intelligence (AI)-based acoustic monitoring system (SoundTalks®). The trial was performed at a commercial grower– finisher pig farm in Korea, involving 2,000 three-way crossbred pigs (YLD, 12 weeks old). The animals were divided into four groups: Group 1 (G1, pigs transferred from a PRRSV-stable farm), Group 2 (G2, pigs transferred from an unstable farm), and two control groups—Control 1 (antibiotic-treated) and Control 2 (antipyretic-treated). AlimenWOW was administered once daily at a dose of 10 mg/kg for three consecutive days, followed by an additional administration upon SoundTalks® alarm detection. Respiratory health status (ReHS), quantified by the AI cough monitoring system, showed a marked improvement in AlimenWOW-treated groups. In particular, G2—which initially exhibited poor respiratory conditions with ReHS ≤ 30—showed a steady increase to 60–80 during weeks 3–4 after administration. The Clinical Cough Index (CCI = 100 – ReHS), representing the inverse relationship between cough intensity and health status, remained significantly lower in AlimenWOW-treated pigs compared to controls after day 14 (p ≤ 0.0001). Although data from two monitoring devices reflected slight differences due to pen structure and body weight distribution, no statistical difference was found, and their mean values were used as representative data, confirming the consistency and reliability of the AI-based monitoring system. These findings indicate that AlimenWOW effectively alleviated coughing and respiratory distress caused by PRRSV infection. In productivity indicators, AlimenWOW administration also produced favorable outcomes. Mortality rates were 0.4 % in G1 and 1.2 % in G2, notably lower than those in Control 1 (2.6 %) and Control 2 (6.6 %), with no wasting cases observed in G2. Quantitative RT-PCR analysis revealed a significant increase in cycle threshold (Ct) values in G2 at day 14 (p < 0.01), indicating viral load reduction, and all pigs tested negative by day 28. Serological analysis further demonstrated that the mean antibody S/P ratio in G2 decreased from 3.7 at day 14 to 3.3 at day 28, suggesting modulation of antibody response associated with infection control and recovery. Collectively, these results demonstrate that AlimenWOW effectively mitigates PRRSV-induced respiratory impairment, mortality, and growth retardation, with its improvement objectively validated through SoundTalks®-derived ReHS indices. Therefore, AlimenWOW presents a promising antiviral strategy that could complement or replace conventional antibiotic and antipyretic management systems, contributing to the reduction of PRRSV-associated productivity losses and the enhancement of biosecurity management at the farm level.
