본 연구는 임종 과정에서 사전의료지시서(Advance Directives, AD)를 작 성한 노인 환자와 그 가족의 경험을 탐색하고, 이들이 삶 속에서 형성해 온 연명의료와 죽음의 의미, 그리고 작성 과정에서 겪은 심리적 변화와 갈등을 심층적으로 이해하고자 하였다. 인천시 소재 상급종합병원 생명윤리위원회 의 승인을 받아(IRB No. GAIRB2025-121) 임종 과정에 있는 판단서가 작성 된 65세 이상 노인 3명과 주 3회 이상 직접 돌봄을 제공하는 가족 3명을 대 상으로 내러티브탐구를 수행하였다. 연구자는 연명의료팀 간호사로 참여자 들과 라포를 형성한 상태에서 1인당 2~3회의 심층 면담을 진행하였으며, 초 기 면담은 90~120분, 이후 면담은 30분 이내로 이루어졌다. 면담 자료는 전 사 및 현장 기록지를 병행하여 분석하였고, 시간·공간·관계의 맥락 속에서 경험의 의미가 드러나는 내러티브 분석 틀을 적용하였다. 연구 결과 노인들 은 과거 가족의 임종 경험, 종교적 신념, 질병 경험 등을 통해 오랫동안 연 명의료와 죽음에 대한 기준을 형성해 온 것으로 나타났으며, 신체적 변화 속에서 임종이 가까워졌음을 자연스럽게 받아들이고 있었고, 가족에게 결정 부담을 남기지 않기 위한 배려와 자기 결정의 실천으로 AD 작성을 경험하 였다. 작성 이후에는 결정에 대한 확고함과 정서적 안정감을 느끼고 삶의 마무리에 대한 수용이 강화되었다. 노인의 가족 역시 과거 상실 경험과 노 인과의 대화를 통해 이미 연명의료와 죽음에 대한 의미를 형성해 왔으며 노 인의 상태 악화와 담당 의료진의 설명을 통해 노인의 임종이 다가왔음을 인 식하고, AD 작성 과정을 노인이 스스로 존엄을 지키는 선택으로 이해하였 으며, 이를 계기로 자신의 노년과 죽음에 대한 성찰과 연명의료 자기결정에 대한 필요성을 인식하였다. 본 연구는 AD 작성이 단순히 임종 직전 급하게 선택하는 행위가 아니라, 생애 경험, 관계, 가치가 누적되어 형성된 점진적 결정임을 확인하였다. 이에 따라 임상에서 간호사는 노인과 가족이 자신의 가치와 기준에 따라 연명의료를 결정할 수 있도록 정서적 지지, 충분한 정 보 제공, 의사결정 준비를 돕는 상담과 실무적 지원을 체계적으로 제공해야 하며, 본 연구 결과는 건강한 시기부터 죽음 준비교육과 AD 등록 프로그램 을 활성화하는 제도적 접근이 필요함을 시사한다.

Patients at the end of life experience the completion of Advance Directives(AD) differently depending on their life history and relational context, giving the moment meaning beyond a simple choice. This study explored the experiences of older adults and their families who completed AD during the dying process to understand the meanings they attributed to AD and death, as well as the psychological changes and conflicts they experienced during decision-making.
This study employed a narrative inquiry approach with three older adults aged 65 and above who had completed a death-process judgment form, along with three family members providing care at least three times per week. Data from two to three in-depth interviews per participant were analyzed using verbatim transcripts and field notes within a narrative framework to interpret experiences across temporal, spatial, and relational contexts.
Patients formed their views on death and AD over time through family experiences, religion, illness, and pain, and they accepted approaching death as their bodies declined. By completing AD to avoid burdening loved ones, both patients and families gained emotional stability and greater acceptance of end-of-life. Families had also developed their attitudes through conversations and medical explanations, recognizing AD as a way to protect patient dignity.
AD decisions develop gradually through life experiences, relationships, and values. Nurses should provide systematic emotional support, information, counseling, and practical assistance to help patients and families make value-aligned decisions.
