본 메타분석은 노년층을 대상으로 한 필라테스 중재가 기능적 이동성 지표인 Timed Up and Go(TUG)에 미치는 영향을 통합적으로 평가하고자 수행되었다. 본 연구에서는 필라테스(Pilates)가 중재로 포함되는 무작위 대조시험(Randomized Controlled Trial)과 준실험 연구를 통합하여 필라테스가 노인의 TUG (Timed Up and Go) 개선에 대한 효과와 연구간 이질성을 정량화하여 시험대상, 중재 유형, 기 간, 총 횟수, 세션당 시간, 기저 TUG(초) 등의 조건에 따른 차이를 확인하고자 한 다. PRISMA 가이드라인에 따라 문헌 검색하고 선정하였고(Page et al., 2021), 중재 이전과 이후의 TUG(초)의 차이를 실험 대상에 대한 개선의 정도로 보고 Hedges’ g(표준화 평균차)로 효과크기를 산출하였다. 최초에는 고정효과와 DerSimonian– Laird 랜덤효과 모델을 병행하고, Cochran’s Q와 I²로 이질성을 평가하였다. 또한, 민감도 분석을 통해 순차적 단일 연구 제거 후의 통합 효과의 안정성을 확인하였 고, 중재 기간·주당 빈도·필라테스 유형·기저 TUG 수준에 따른 하위군 분석을 실 시하였다. 최종 분석에는 24의 연구와 25개의 중재를 비교하였다. 고정효과 모형에 서 통합 효과크기는 g=0.90(95% CI 0.78~1.02)로 나타났고, 랜덤효과 모형에서도 g=1.37(95% CI 1.02~1.73)로 통계적으로 의미 있는 단축 효과가 확인되었다. 연구 간 변이는 Q=194.29(df=24), I²≈88%로 높게 관찰되었다. 단일 연구를 순차적으로 제외하는 민감도분석에서 통합 효과는 약 g=1.25 이상의 수준으로 유지되었으나 (1.25~1.30: 3, 1.31~1.39: 11, 1.40 이상: 11), I²는 대체로 85% 이상을(84.68~88.16%) 보였다. 하위군 분석에서는 총 8~11주 (Hedge' g=1.55), 주 2회 (Hedges' g=1.48) 실시한 프로그램에서 효과가 가장 컸다. 기저 TUG가 13.5초를 넘는 집단에서 개선 폭이 상대적으로 컸고(Hedges' g=1.48), 매트 기반 필라테스에서(Hedges' g=1.56) 유의한 단축이 관찰되었다. 이러한 결과는 필라테스가 노년층의 TUG 수행 시간을 줄이는 데 효과적이며, 낙상 위험 감소에 기여할 가능성을 뒷받침한다. 실무적으로 는 8주, 주 2회, 매트 기반 프로그램을 우선 고려할 것을 제안한다. 다만 연구 간 이질성이 높아 중재 구성과 대상자 특성의 표준화, 그리고 메타회귀를 통한 변이의 원인 규명이 필요하다. 분석 결과는 노년층 낙상 예방을 위한 근거 기반 운동처방 수립에 활용될 수 있다.

This study aims to synthesize data from Randomized Controlled Trials(RCTs) and quasi-experimental studies involving Pilates intervention to quantify its effect on improving TUG(Timed Up and Go) performance in older adults. Furthermore, it seeks to determine the heterogeneity among studies and identify differences in effects based on characteristics such as participant type, intervention type, duration, total sessions, time per session, and baseline TUG score (seconds).
Literature search and selection were conducted following the PRISMA guidelines (Page et al., 2021). The difference in TUG (seconds) before and after the intervention was reported as the degree of improvement in the experimental group, and the effect size was calculated using Hedges’ g (Standardized Mean
Difference). Initially, both fixed-effects and DerSimonian–Laird random-effects models were employed, and heterogeneity was assessed using Cochran’s Q and
I² statistics. Sensitivity analysis confirmed the stability of the pooled effect after - 43 - sequential exclusion of single studies. Subgroup analyses were performed based on intervention duration, weekly frequency, Pilates type, and baseline TUG level. A total of 24 studies and 25 interventions were included in the final analysis.
The pooled effect size in the fixed-effects meta-analysis was g=0.90(95% CI 0.78~1.02), and the random-effects meta-analysis also showed a significant
improvement with g=1.37(95% CI 1.02~1.73). This evidence is interpreted as Pilates significantly shortening the TUG time. Heterogeneity was very high, with Q=194.29(df=24), I²≈88%. The pooled effect remained stable at over g=1.25(1.25~1.30: 3, 1.31~1.39: 11, 1.40 이상: 11) even after sequentially removing single studies, though I² generally remained above 85%(84.68~88.16%). Subgroup analysis indicated that the effect was largest under conditions involving a total duration of 8~11 weeks (Hedge' g=1.55) and a frequency of 2 times per week(Hedges' g=1.48). The improvement was generally greater in groups with a baseline TUG exceeding 13.5 seconds (Hedges' g=1.48). A substantial effect was also observed in Mat Pilates programs targeting older adults(Hedges' g=1.56).
Pilates intervention for older adults shows a significant positive effect on improving TUG, an indicator of functional mobility, suggesting its potential contribution to fall risk reduction. The optimal intervention conditions are summarized as 8 weeks, 2 times per week, and a mat-based program. However, the high degree of inter-study heterogeneity necessitates standardization of intervention design and participant characteristics, as well as further investigation into the sources of variation through methods like meta-regression. This meta-analysis provides empirical evidence for the development of evidence-based exercise prescriptions aimed at fall prevention in the older adult population.
