본 연구는 고령자 신체 기능과 인지 기능 향상을 목적으로 게이미피케이션 기반 신체·인지 통합 프로그램을 적용하여 온라인과 오프라인 환경에서 그 효과를 다각적으로 검증하는 데 목적이 있다. 1차 온라인 실험에서는 신체 및 인지 게임을 적용하여 동적균형능력, 보행속도, 균형능력 변화와 인지기능 향상을 확인하고자 하였다. 2차 오프라인 실험에서는 컬러스텝 기반 단일 게임을 중심으로 프로그램을 보완 및 재구성하여 신체 기능 측정의 정밀성을 확보하기 위해 마커리스 모션 캡처 시스템(OpenCap)을 도입하였다.
연구 결과, 1차 실험에서는 FTSST와 TUG 수행시간이 유의하게 개선되었으나 BBS와 인지 기능에서는 통계적으로 유의한 차이가 나타나지 않았다. 반면 2차 실험에서는 발판 거리 증가에 따라 FTSST, FSST, F8WT가 모두 유의하게 항상되어 하지 근력, 동적균형능력, 곡선보행능력의 개선 효과가 확인되었다. 또한 인지기능에서도 난이도 증가에 따른 수행 향상이 나타나 프로그램의 인지 자극 효과를 확인하였다.
OpenCap 기반 운동역학 분석에서는 FTSST 수행 과정 중 전체 관절 지표 가운데 vel_time_50% 구간에서의 각속도에서 유의한 결과를 도출하였으며, RoM과 기타 타 지표에서는 통계적으로 유의한 차이가 확인되지 않았다. 이는 프로그램 참여가 FTSST 중간 구간에서의 속도 조절 능력 향상이 일부 부분에서만 제한적으로 기여하였음을 의미하며, 고령자의 운동 변화를 정량적으로 파악할 수 있는 근거를 제시한다.
두 실험은 모두 출석률이 77~78.7%로 비교적 높게 나타났으며, 1차 온라인 실험에서는 사용성 만족도가, 2차 오프라인 실험에서는 프로그램의 안전성이 가장 높은 평가를 보였다. 특히 2차 실험에서는 단체 활동에서 개인 활동 보다 더 높은 몰입도과 참여 의지가 보임으로 인해, 사회적 촉진 효과가 고령자의 운동 지속성에 중요한 역할임을 확인하였다.
본 연구는 게이미피케이션 기반 신체·인지 통합 프로그램이 고령자의 신체 기능과 인지 기능에 긍정적인 영향을 미칠 수 있음을 온라인·오프라인 조건에서 단계적으로 검증하였다. 또한 OpenCap을 활용한 운동역학 분석을 통해 고령자의 동작 전략 과 속도 특성을 파악하여 제시함으로써, 향후 디지털 헬스케어 기반 고령자 맞춤형 중재 설계에 실질적 활용 가능한 학술적 근거를 제공한다.

This study examined the effects of a gamification-based integrated physical and cognitive program for older adults in online and offline settings. The first online experiment assessed changes in Dynamic balance ability, Gait speed, balance, and cognitive function using game-based activities, while the second offline experiment refined a color-step–based program and incorporated OpenCap to enhance assessment precision.
In the first experiment, significant improvements were observed in FTSST and TUG, whereas no significant changes were found in BBS or cognitive outcomes. In contrast, the second experiment showed significant improvements in FTSST, FSST, and F8WT with increasing step-pad distances, along with improved cognitive performance under higher task difficulty.
OpenCap-based analysis revealed a significant change only in angular velocity at the vel_time_50% phase during FTSST, suggesting selective improvement in mid-phase movement speed control and demonstrating OpenCap’s potential for quantitatively capturing movement characteristics in older adults.
Program attendance was high in both studies (77~78.7%). Participants reported strong usability in the online experiment and strong perceived safety in the offline experiment. Group-based activities elicited greater engagement than individual tasks, highlighting the importance of social facilitation for exercise adherence in older adults.
Overall, the findings demonstrate that a gamified integrated program can positively impact both physical and cognitive function in older adults. The incorporation of OpenCap further provides useful biomechanical insights for designing personalized digital healthcare interventions.
