세균 유래 세포 외 소포체(Extracellular Vesicles, EVs)는 세균의 병원성 인자와 내성 유전자를 포함하고 있어 질병 진단 및 내성 기전 연구의 핵심적인 바이오 마커로 주목 받고 있다. 특히 메티실린 내성 황색포도상 구균(MRSA) 및 다제 내성균의 확산을 방지하기 위해서는 다중 균주 유래 소포체에 대한 신속한 동시 분석과 항생제 감수성 판별 기술이 필수적이다. 그러나 기존 방식은 복잡한 전처리 과정과 장시간의 배양을 요 구하며, 현장 진단(POCT) 적용 시 시스템의 자동화와 소포체의 물리적 상태 보존이 중요한 과제로 남아있다. 본 연구에서는 이러한 한계를 극복하기 위해 단일표적 공압 구동 미세 유체 플랫폼(ST-PDMFP)과 이를 제어하는 자동화 입자 농축 시스템 (FAPCS)을 개발하고, 이를 확장하여 다중표적 검출 플랫폼(MT- PDMFP)을 구축하였다. 개발된 플랫폼은 3차원 유체 네트워크와 미세 공압 밸브의 정밀 제어를 통해 시료 주입부터 입자 농축, 세척 및 용출 에 이르는 전 과정을 자동화하였다. 시스템 성능 검증 결과, 미세 입자 농축 및 표적 결합을 포함한 공정을 35분 이내에 완료하였으며, 약 992 배의 높은 농축 비율과 100%의 입자 회수 효율을 달성하였다. 특히, 시스템의 범용성과 정밀도를 입증하기 위해 SARS-CoV-2의 RBD가 발현된 외막 소포체(OMV-RBD)를 바이러스 모방 나노 모델로 활용하여 검증을 수행하였다. 기존의 초고속 원심분리 방식은 강한 물리적 힘으로 인해 소포체의 구조적 변성을 유발할 수 있으나, 본 플랫폼은 농축 과정에서 소포체의 물리적 변성이나 손상이 없도록 유체역학적 스트레스를 최소화한 공압 제어 방식을 채택하였다. 나아가 본 논문에서는 본 플랫폼을 활용하여 메티실린 감수성 황색포도 상구균(MSSA) 및 메티실린 내성 황색포도상구균(MRSA) 유래 소포체 의 특성 분석과 검출 성능을 실증하였다. 추출된 소포체(100~140 nm) 를 대상으로 특이적 포집을 수행한 결과, SEM 및 RT-PCR 분석을 통 해 각 균주 유래 소포체가 성공적으로 검출됨을 확인하였다. 특히, 다중 채널 기반의 MT-PDMFP(4개 층 구조)를 제작하여 5개 의 개별 챔버 내에서 서로 다른 크기의 마이크로 비드(4 μm, 10 μm)를 동시에 농축하는 성능을 입증함으로써, 한 번의 샘플 로딩으로 다종의 표적을 동시 검출할 수 있는 다중화 성능의 기술적 토대를 마련하였다. 결론적으로, 본 연구에서 구현한 전자동 미세 유체 플랫폼은 기존의 세균 배양 기반 검사 대비 분석 시간을 획기적으로 단축시키고(전처리 및 검출 공정 약 8시간), 나노 스케일의 소포체를 효과적으로 농축 및 변별할 수 있는 효율적인 도구임을 입증하였다. 이는 향후 감염병의 정 밀 진단과 환자 맞춤형 항생제 처방을 위한 차세대 현장 진단 플랫폼으 로서 높은 임상적 가치를 제공할 것으로 기대된다.

Bacterial extracellular vesicles (EVs) have emerged as pivotal biomarkers for disease diagnosis and antimicrobial resistance (AMR) studies, as they carry pathogenic factors and resistance genes. In particular to prevent the spread of multi-drug resistant bacteria and Methicillin-resistant Staphylococcus aureus (MRSA), rapid simultaneous analysis of multi-strain EVs and antibiotic susceptibility determination are essential. However, conventional methods are hindered by complex pretreatment processes and prolonged incubation times, posing significant challenges for automation and the preservation of vesicle structural integrity in point-of-care testing (POCT) applications. In this study, to overcome these limitations, a Single-Target Pneumatically Driven Microfluidic Platform (ST-PDMFP) and a Fully Automated Particle Concentration System (FAPCS) were developed and further expanded into a Multi-Target Detection Platform (MT- PDMFP). The developed platform automates the entire process— from sample injection to particle concentration, washing, and elution—through the precise control of a 3D fluidic network and micro pneumatic valves. Performance validation demonstrated that the pretreatment process, including bead concentration and target binding, was completed within 35 minutes, achieving a high concentration ratio of approximately 992-fold (99,200%) and 100% particle recovery efficiency. To verify the system's versatility and precision, outer membrane vesicles expressing the SARS-CoV-2 RBD (OMV-RBD) were utilized as a virus-mimetic nano model. While conventional ultracentrifugation can induce structural deformation due to high physical forces, this platform confirmed structural stability by maintaining the intact state of vesicles without damage during the concentration process, leveraging a pneumatic control method that minimizes hydrodynamic stress. Furthermore, the platform successfully demonstrated the characterization and detection of EVs derived from Methicillin-susceptible S. aureus (MSSA) and MRSA (100–140 nm), as confirmed by SEM and RT-PCR analyses. Specifically, the 4-layer MT-PDMFP enabled the simultaneous concentration of micro-sized beads (4 μm and 10 μm) within five individual chambers, establishing a technical foundation for the concurrent detection of diverse targets from a single sample loading. In conclusion, the fully automated microfluidic platform implemented in this study drastically reduced the analysis time to approximately 8 hours (including pretreatment and detection) compared to traditional culture-based tests. It proved to be an efficient tool for the effective concentration and differentiation of nano-scale vesicles, offering high clinical value as a next-generation POCT platform for precision diagnosis and tailored antibiotic prescription.
