This study aims to comprehensively evaluate the effects of Biobloc appliances on skeletodental structures and airway space in patients with Class II malocclusion. The study provides insight into the effectiveness of Biobloc appliances in addressing both functional and structural aspects of Class II malocclusion during the critical growth period.

In this retrospective study, 27 patients with class II malocclusion in the mixed dentition stage treated with Biobloc appliances were selected. A total of 25 landmarks, covering both soft tissue and airway structures, were digitized using lateral cephalometric radiographs taken before treatment (T0) and after treatment (T1). The analysis evaluated horizontal mandibular growth, vertical growth patterns, improvements in overbite, and changes in airway width.

The mandible showed substantial forward growth, as indicated by significant decreases in ANB and Wits measurements, along with increases in SNB, APDI, and Pog-N perpendicular values. Vertical growth patterns changed from clockwise to counter-clockwise, as shown by significant decreases in SUM, FMA, Sn-GoGn, and the Y-axis to FH. The width of the airway at the soft palate tip and the retroglossal distance both significantly increased, reflecting an overall improvement in airway width.

Orthotropic treatment with Biobloc appliances is an effective method for addressing skeletal discrepancies in Class II malocclusion. The appliances promote forward and horizontal mandibular growth while limiting vertical growth, contributing to a more balanced facial profile and improved occlusal relationships. Additionally, the significant increase in airway width observed in this study highlights the functional benefits of the Biobloc, particularly in growing patients. These findings emphasize the value of early intervention and the potential of Biobloc appliances to achieve both structural corrections and functional improvements in orthodontic care.
