    흡연은 전신 염증을 유발하고 불리한 지질 프로파일을 촉진하는 것으로 알려져 있으나, 한국 성인을 대상으로 한 대표성 있는 인구 기반 근거는 여전히 제한적이다. 기존 연구들은 주로 흡연, 염증, 이상지질혈증 간 단순 상관성을 분석하였으며, 잠재적 교란 요인을 체계적으로 조정하거나 인과적 방향성을 고려한 분석은 미흡하였다. 이에 본 연구는 생물학적 기전과 역학적 근거를 종합하여 방향성 비순환그래프(DAG)를 구축하고, 이를 기반으로 흡연-염증-지질 축 경로를 시각적으로 제시하였다. 또한, DAG를 토대로 흡연행태, hs-CRP, 지질지표 간의 다변량 관계를 동시에 평가할 수 있도록 구조방정식모형(SEM)을 적용하였다.
    본 연구는 제9기 국민건강영양조사(KNHANES, 2022-2023)를 사용하여 결측치 제외 후 19세에서 69세 사이의 성인 6,611명을 최종 분석 대상으로 하였다. 흡연행태는 비흡연, 과거흡연, 현재흡연으로 구분하였으며, hs-CRP는 AHA/CDC 가이드라인에 따라 <1.0, 1.0-<3.0, ≥3.0 mg/L로 구분하였다. 이상지질혈증은 고콜레스테롤혈증, 고중성지방혈증, 저HDL-콜레스테롤혈증, 고LDL-콜레스테롤혈증으로 정의하였다. 인구사회학적 요인, 체질량지수, 허리둘레, 사회경제적 요인, 음주, 혈당, 간효소 등 주요 공변량을 단계적으로 조정한 복합표본 로지스틱 회귀분석을 수행하여 교차비(OR)를 산출하였다.
    그 결과, 현재흡연은 고중성지방혈증(OR=2.26) 및 저HDL-콜레스테롤혈증과 유의한 연관성을 보였다(OR=2.01). 반면, 고콜레스테롤혈증 및 고LDL-콜레스테롤혈증과의 연관성은 유의하지 않았다(p>0.05). 층화 분석에서 현재흡연군 중 hs-CRP 1.0-<3.0 mg/L 범주에서는 고콜레스테롤혈증, 고중성지방혈증, 저HDL-콜레스테롤혈증의 위험도가 증가하였으며(p<0.05), hs-CRP ≥3.0 mg/L 범주에서도 저HDL-콜레스테롤혈증과의 유의한 연관성이 지속되었다(p<0.05). 비흡연자에서도 hs-CRP는 모든 범주에서 저HDL-콜레스테롤혈증과 유의하게 관련되었다(p<0.001).
    구조방정식 분석에서 흡연의 직접 효과와 hs-CRP 매개 효과를 가정한 기본 모형을 검증한 결과, 현재흡연은 hs-CRP에 대한 유의한 직접 효과를 보이지 않았으나(β=0.000), 중성지방 증가(β=0.137), HDL-C 감소(β=-0.065), LDL-C 감소(β=-0.086)와는 유의한 직접적 관련성을 나타냈다. 허리둘레 위험군은 정상군에 비해 hs-CRP(β=0.454)와 지질지표 모두에서 강력한 직접 효과를 보였으며, hs-CRP를 매개로 한 간접 효과도 확인되어 복부비만-염증-지질 이상으로 이어지는 주요 경로를 형성하였다. 추가적으로 흡연이 지질지표 변화를 통해 hs-CRP에 영향을 미치는 경로를 가정한 대안 모형을 분석한 결과, 대안 모형에서도 현재흡연은 중성지방 증가(β=0.142) 및 HDL-C 감소(β=-0.071)와 강하게 연관되었다. 특히 지질지표가 hs-CRP에 미치는 영향을 확인했을 때, 중성지방(β=0.056)과 LDL-C(β=0.056)의 증가는 hs-CRP 상승과 유의하게 관련되었고, HDL-C(β=-0.125)는 hs-CRP 감소와 연관되었다. 이는 염증 수준이 지질 대사 이상과 복부비만이 복합된 대사 상태를 반영하는 하위 지표일 가능성을 시사한다.
    결론적으로 본 연구는 한국 성인에서 흡연-염증-지질 축이 분명하게 드러나며, 특히 현재흡연과 복부비만이 결합될 때 그 부정적 영향이 더욱 강화됨을 확인하였다. 아울러 대안 모형 분석을 통해 hs-CRP는 흡연자의 심혈관 질환 위험을 단순히 반영하는 보조지표가 아니라, 지질대사와의 연관성을 포함한 보다 복합적인 위험성 평가의 구성요소로 활용될 수 있음을 제시한다.

    Smoking is known to induce systemic inflammation and promote unfavorable lipid profiles; however, population-based evidence representative of Korean adults remains limited. Previous studies primarily examined simple associations among smoking, inflammation, and dyslipidemia, without systematically adjusting for potential confounders or inferring causal directions. In this study, we synthesized biological mechanisms and epidemiological evidence from the literature to construct a directed acyclic graph (DAG) and identify major pathways within the smoking-inflammation-lipid metabolism axis. Based on this causal framework, we applied structural equation modeling (SEM) to simultaneously evaluate the multivariate relationships among smoking behavior, high-sensitivity C-reactive protein (hs-CRP), and lipid parameters.
    Using data from the 2022-2023 Korea National Health and Nutrition Examination Survey (KNHANES), 6,611 adults aged 19-69 years were included after applying exclusion criteria. Smoking behavior was categorized as never, former, or current smoking. hs-CRP levels were classified as <1.0, 1.0-<3.0, and ≥3.0 mg/L. Dyslipidemia outcomes included hypercholesterolemia, hypertriglyceridemia, low HDL cholesterol, and high LDL cholesterol. Complex-sample logistic regression analyses with stepwise adjustment for demographic, anthropometric, socioeconomic, and clinical covariates were conducted to estimate odds ratios (ORs).
    Current smoking was strongly associated with hypertriglyceridemia and low HDL cholesterol (OR=2.23 and 2.01, respectively; p<0.001), whereas associations with hypercholesterolemia and high LDL cholesterol were not statistically significant (p>0.05). Stratified analyses showed that among current smokers, hs-CRP levels of 1.0-<3.0 mg/L were associated with increased risks of hypercholesterolemia, hypertriglyceridemia, and low HDL cholesterol (p<0.01), and hs-CRP ≥3.0 mg/L remained associated with low HDL cholesterol (p<0.01). Among non-smokers, hs-CRP was consistently associated with low HDL cholesterol across all categories (p<0.001).
    In the SEM framework, current smoking did not exhibit a significant direct effect on hs-CRP (β=0.000), but was directly associated with increased triglycerides (β=0.137), decreased HDL-C (β=-0.065), and decreased LDL-C (β=-0.086). Former smoking was associated with lower hs-CRP (β = -0.040) and lower LDL-C (β=-0.042) compared with never smoking, while showing a slight tendency toward higher triglycerides (β=0.025). The direct effect of former smoking on HDL-C was not statistically significant (β=0.007).
    hs-CRP showed consistent associations with lipid parameters, including higher triglycerides (β=0.119), higher LDL-C (β=0.066), and lower HDL-C (β=-0.156). Individuals with high-risk waist circumference exhibited higher hs-CRP (β=0.454), triglycerides (β=0.330), and LDL-C (β=0.148), and lower HDL-C (β=-0.301) than those in the normal group. Waist circumference also demonstrated indirect effects mediated by hs-CRP, amplifying its total effects on triglyceride and LDL-C increases and HDL-C decreases beyond the magnitude of direct effects.
    After adjustment for covariates, males had higher triglycerides (β=0.083) and lower HDL-C (β=-0.241) than females, and increasing age was associated with higher triglycerides (β=0.098) and higher LDL-C (β=0.078).
    These findings indicate that the smoking-inflammation-lipid axis is clearly observable in Korean adults. Current smoking exerted its primary effects through direct increases in triglycerides and decreases in HDL-C, with limited mediation by hs-CRP. In contrast, abdominal obesity demonstrated strong direct and indirect pathways linking inflammation and lipid abnormalities. Overall, this study suggests that hs-CRP may serve not merely as an auxiliary marker but as an important component for evaluating cardiometabolic risk among smokers.
