    Wearable technology has evolved from Holter’s 24 hour electrocardiogram (ECG) in 1949 into lightweight, multi sensor devices that continuously track cardiac, respiratory, movement, and other biomarkers outside clinical settings. In the early 21st century, IoT and mobile connectivity expanded their reach into under resourced regions, and the COVID 19 pandemic spurred rapid adoption of skin and biofluidic based sensors for continuous monitoring, early warning, and risk prediction. Physical activity (PA) remains a critical focus for those at risk of metabolic syndrome (MetS), where patterns and habits matter more than total volume. While wrist worn wearables reliably capture steps and calories, earable devices offer greater signal stability and comfort for long term, real world use. To address workforce shortages and equity gaps in chronic disease prevention, participatory service design frameworks are needed to turn continuous sensor data into sustainable behavior change.

    This dissertation synthesizes three inter linked studies to examine how earable device can broaden prevention strategy by detecting cardiometabolic risk, timing pattern specific PA nudges, and scaffolding habit formation among adults with MetS risk factors. This is to explore how earable devices can promote PA among adults at MetS risk, integrate seamlessly into daily life, and scale chronic disease management within Digital Public Health initiatives. Study 1 mapped the health-related indicators of earable device; Study 2 validated measurement accuracy and extracted time series PA rhythms; Study 3 co designed an audio–haptic coaching service and user journey map that embeds those insights into everyday self care.

    Study 1 involved a scoping review of English language publications from 2015 to 2020 in PubMed, Web of Science, and Embase, applying PRISMA ScR criteria to screen 995 records and retain 51 original human participant studies on non auditory earable sensing, with data extracted on outcomes, biomarkers, sensor modalities and placement, and prevention stages. Study 2 enrolled adults aged 30 years or older with at least one MetS risk factor who habitually used both Apple AirPods (3/Pro/Max) and Apple Watch Series, collected 7 days of 5 minute energy expenditure (EE), step count, distance, and GPS data during PA (217 person days), and applied TADPole time series clustering optimized by dynamic time warping (DTW) parameters and silhouette index alongside hierarchical and partitional methods, followed by evaluation of device agreement and assessment of the influence of habitual use patterns on cluster stability through Pearson correlations, generalized additive and regression models, and Firth penalized logistic regression with mediation analysis. Study 3 engaged aged 40 years or older adults at least one MetS risk in a 3-stage Double Diamond service design process, beginning with focus group interviews (N = 11) and a 3 day diary probe (N = 10) analyzed using an AEIOU coding framework to identify user needs, proceeding to a 4 hour co design workshop (N = 7) employing Bingo, IDEO Method Cards, and Six Thinking Hats to generate and refine self care scenarios, and culminating in the development of a coaching persona and user journey map that align audio haptic earable prompts with daily routines and habit formation opportunities.

    In Study 1, EEG (22 %), body movement (12 %) and core temperature (10 %) dominated the biomarker landscape, electrical and photoelectric sensors prevailed, and 69% of investigations focused on secondary prevention; nevertheless, the breadth of bio-signals suggested wider applicability to primary prevention and tertiary prevention. In Study 2, ear derived EE moderately correlated with wrist EE (r = 0.63, p < .001), particularly at light to moderate intensities; TADPole time series clustering revealed 3 archetypal chronotypes (Sedentary, Explorer, Super Activist) and captured weekday to weekend drift. In a Firth penalized model each 1 point rise in a habitual use composite corresponded to a 77 % reduction in pattern instability (OR 0.23, 95 % CI 0.03 0.67), while the simple routine of checking the device immediately before movement yielded an even stronger association (OR 0.15, 95 % CI 0.02 0.76), hinting that technology engagement points of behavioral change is needed. In Study 3, the process converged on a voice first, low burden coaching persona and seven scenario based user journey maps. Interventions included a short audio plus vibration prompt triggered by simultaneous declines in HRV and activity, an earbud initiated neck and shoulder stretch after 1 hour of sitting, and an “Our local doctor” loop that channels in ear vitals to primary care clinics in community or Seoul’s Wrist Doctor 9988 + service via FHIR compliant personal health records. Participants prioritized context aware micro feedback, emotionally secure data sharing within trusted circles, and a calibrated balance between autonomous guidance and user agency, underscoring the importance of user centered design.

    Earable devices positioned close to the brain and heart enable continuous biomarker surveillance for risk detection, pinpoint intervention opportunities through time series clustering of high resolution accelerometry and bio-signals, and foster habit formation with unobtrusive audio haptic nudges integrated into daily routines. Academically, this work proposes an earable enabled prevention continuum that fuses sensor intelligence with behavioral science. Methodologically, it demonstrates a mixed methods pathway spanning scoping review, real world data validation, and participatory service design. Substantively, earables can complement or replace wrist wearables while reducing digital divides among aging populations. At the policy level, integration of earable device data streams into Korea’s MyHealthWay platform offers a blueprint for pattern specific nudging in regional programs and informs governance discussions on interoperability, privacy, and cost effectiveness.

    Key limitations include a small sample size, prototype level hardware without PPG in mass market earbuds, potential publication bias, and lack of long term, multi site trials for efficacy and cost effectiveness. Future work should involve larger, longer cohorts, additional biomarkers (blood pressure, oxygen saturation), adaptive algorithms that merge ear centric signals with environmental context, and ethical data governance frameworks.

    An integrated prevention model for holistic prevention of chronic disease supported by earable based sensing and user centered service design includes secondary prevention through continuous risk monitoring and pattern specific interventions, such as identifying sedentary or explorer profiles and delivering personalized nudges; primary prevention via context aware micro feedback that fosters both conscious and unconscious interactions to promote habit formation; and tertiary prevention focused on functional improvement through emotional reassurance, social sharing, and gamification. This model provides a forward looking strategy for chronic disease management.

    Wearable technology has evolved from Holter’s 24 hour electrocardiogram (ECG) in 1949 into lightweight, multi sensor devices that continuously track cardiac, respiratory, movement, and other biomarkers outside clinical settings. In the early 21st century, IoT and mobile connectivity expanded their reach into under resourced regions, and the COVID 19 pandemic spurred rapid adoption of skin and biofluidic based sensors for continuous monitoring, early warning, and risk prediction. Physical activity (PA) remains a critical focus for those at risk of metabolic syndrome (MetS), where patterns and habits matter more than total volume. While wrist worn wearables reliably capture steps and calories, earable devices offer greater signal stability and comfort for long term, real world use. To address workforce shortages and equity gaps in chronic disease prevention, participatory service design frameworks are needed to turn continuous sensor data into sustainable behavior change.

    This dissertation synthesizes three inter linked studies to examine how earable device can broaden prevention strategy by detecting cardiometabolic risk, timing pattern specific PA nudges, and scaffolding habit formation among adults with MetS risk factors. This is to explore how earable devices can promote PA among adults at MetS risk, integrate seamlessly into daily life, and scale chronic disease management within Digital Public Health initiatives. Study 1 mapped the health-related indicators of earable device; Study 2 validated measurement accuracy and extracted time series PA rhythms; Study 3 co designed an audio–haptic coaching service and user journey map that embeds those insights into everyday self care.

    Study 1 involved a scoping review of English language publications from 2015 to 2020 in PubMed, Web of Science, and Embase, applying PRISMA ScR criteria to screen 995 records and retain 51 original human participant studies on non auditory earable sensing, with data extracted on outcomes, biomarkers, sensor modalities and placement, and prevention stages. Study 2 enrolled adults aged 30 years or older with at least one MetS risk factor who habitually used both Apple AirPods (3/Pro/Max) and Apple Watch Series, collected 7 days of 5 minute energy expenditure (EE), step count, distance, and GPS data during PA (217 person days), and applied TADPole time series clustering optimized by dynamic time warping (DTW) parameters and silhouette index alongside hierarchical and partitional methods, followed by evaluation of device agreement and assessment of the influence of habitual use patterns on cluster stability through Pearson correlations, generalized additive and regression models, and Firth penalized logistic regression with mediation analysis. Study 3 engaged aged 40 years or older adults at least one MetS risk in a 3-stage Double Diamond service design process, beginning with focus group interviews (N = 11) and a 3 day diary probe (N = 10) analyzed using an AEIOU coding framework to identify user needs, proceeding to a 4 hour co design workshop (N = 7) employing Bingo, IDEO Method Cards, and Six Thinking Hats to generate and refine self care scenarios, and culminating in the development of a coaching persona and user journey map that align audio haptic earable prompts with daily routines and habit formation opportunities.

    In Study 1, EEG (22 %), body movement (12 %) and core temperature (10 %) dominated the biomarker landscape, electrical and photoelectric sensors prevailed, and 69% of investigations focused on secondary prevention; nevertheless, the breadth of bio-signals suggested wider applicability to primary prevention and tertiary prevention. In Study 2, ear derived EE moderately correlated with wrist EE (r = 0.63, p < .001), particularly at light to moderate intensities; TADPole time series clustering revealed 3 archetypal chronotypes (Sedentary, Explorer, Super Activist) and captured weekday to weekend drift. In a Firth penalized model each 1 point rise in a habitual use composite corresponded to a 77 % reduction in pattern instability (OR 0.23, 95 % CI 0.03 0.67), while the simple routine of checking the device immediately before movement yielded an even stronger association (OR 0.15, 95 % CI 0.02 0.76), hinting that technology engagement points of behavioral change is needed. In Study 3, the process converged on a voice first, low burden coaching persona and seven scenario based user journey maps. Interventions included a short audio plus vibration prompt triggered by simultaneous declines in HRV and activity, an earbud initiated neck and shoulder stretch after 1 hour of sitting, and an “Our local doctor” loop that channels in ear vitals to primary care clinics in community or Seoul’s Wrist Doctor 9988 + service via FHIR compliant personal health records. Participants prioritized context aware micro feedback, emotionally secure data sharing within trusted circles, and a calibrated balance between autonomous guidance and user agency, underscoring the importance of user centered design.

    Earable devices positioned close to the brain and heart enable continuous biomarker surveillance for risk detection, pinpoint intervention opportunities through time series clustering of high resolution accelerometry and bio-signals, and foster habit formation with unobtrusive audio haptic nudges integrated into daily routines. Academically, this work proposes an earable enabled prevention continuum that fuses sensor intelligence with behavioral science. Methodologically, it demonstrates a mixed methods pathway spanning scoping review, real world data validation, and participatory service design. Substantively, earables can complement or replace wrist wearables while reducing digital divides among aging populations. At the policy level, integration of earable device data streams into Korea’s MyHealthWay platform offers a blueprint for pattern specific nudging in regional programs and informs governance discussions on interoperability, privacy, and cost effectiveness.

    Key limitations include a small sample size, prototype level hardware without PPG in mass market earbuds, potential publication bias, and lack of long term, multi site trials for efficacy and cost effectiveness. Future work should involve larger, longer cohorts, additional biomarkers (blood pressure, oxygen saturation), adaptive algorithms that merge ear centric signals with environmental context, and ethical data governance frameworks.

    An integrated prevention model for holistic prevention of chronic disease supported by earable based sensing and user centered service design includes secondary prevention through continuous risk monitoring and pattern specific interventions, such as identifying sedentary or explorer profiles and delivering personalized nudges; primary prevention via context aware micro feedback that fosters both conscious and unconscious interactions to promote habit formation; and tertiary prevention focused on functional improvement through emotional reassurance, social sharing, and gamification. This model provides a forward looking strategy for chronic disease management.
