면역관문 억제제(immune checkpoint inhibitor, ICI)는 비소세포성 폐암(non-small cell lung cancer, NSCLC) 환자에게 표준 치료로 사용되고 있으나, 환자간 치료 반응 및 예후의 이질성을 설명할 수 있는 효과적인 Biomarker를 필요로 한다. 특히, ICI는 환자 순환계 내 면역세포와 직접 상호작용하며, 이러한 면역세포의 전사체 변화는 면역반응 및 예후에 중요한 영향을 미칠 수 있다. 그러나, NSCLC 환자에서 면역세포 전사체가 환자의 유전적 변이나 클론성 조혈증(Clonal hematopoiesis of indeterminate potential, CHIP)과 같은 체세포 변이에 의해 어떻게 조절되는지는 명확히 규명되지 않았다.
본 연구에서는 ICI로 치료받은 NSCLC 환자 100명의 혈액에서 CHIP 검출을 위한 표적 시퀀싱(targeted sequencing)을 수행하였고, 이 중 73명의 환자에 대해 단일세포 RNA 시퀀싱(single-cell RNA sequencing, scRNA-seq)을 진행하였다. 이 대응 샘플(matched samples)을 기반으로 생식세포 및 체세포 변이와 연관된 면역세포 전사체 변화를 분석하였으며, 이를 위해 단일세포 수준의 유전자 발현 eQTL 분석(single-cell-expression quantitative loci mapping)과 공발현 네트워크 분석(co-expression network analysis)을 통합적으로 수행하였다. 또한 CD8⁺ T 세포를 기반으로 반응성 연관 전사체 네트워크를 탐색하였으며, 이와 환자의 생존율과의 관련성을 평가했다.
분석 결과, CHIP 변이를 보유한 NSCLC 환자에서 myeloid lineage의 염증성 네트워크가 보존됨을 확인하였다. 또한 sc-eQTL 분석을 통해 총 3,616개의 eGene을 도출하였으며, 이 유전자들은 기존의 대규모 참조(reference) 혈액 eQTL 데이터와 구분되는 면역특이적 조절 양상을 나타냈다. 이러한 전사체적 특징을 이해하기 위한 네트워크 분석과 그래프 네트워크 학습 기반 프레임워크를 통해 CD8⁺ T 세포의 분화와 관련된 15개의 핵심 유전자를 산출하였다. 이를 기반으로 계산한 15개 주요 유전자 기반 기능적 스코어는 환자의 임상적 변수를 고려하더라도 무진행 생존율(progression-free survival) 및 전체 생존율(overall survival)과 유의미한 연관성을 보였다. 주목할 만한 점은, 생식세포 및 CHIP 변이가 환자의 면역 상태를 폭넓게 형성하는 반면, 예후 예측에 가장 핵심적인 CD8⁺ T 세포 네트워크는 개별 변이보다는 공발현 네트워크의 강력한 조절력에 의해 결정되는 것으로 나타났다.
본 연구는 ICI를 투여받은 NSCLC 환자에서 생식세포 및 CHIP을 포함한 체세포 변이가 면역 전사체 특성 형성에 광범위하게 관여함을 제시한다. 그러나 CD8⁺ T 세포 분화와 같은 예후 예측 관련 핵심 조절 모듈은 개별 유전적 변이보다는 공발현 네트워크에 의한 동적 조절(co-expression dynamics)에 의해 주로 결정되는 것으로 보인다. 이 연구는 폐암 환자의 면역 조절을 이해하는 데 있어 단일세포 수준의 유용한 자원을 제공하며, 도출된 바이오마커의 추가적인 독립적 검증이 필요함을 시사한다.

Background: Immune checkpoint inhibitors (ICIs) have become a standard treatment for non-small cell lung cancer (NSCLC), yet there remains a lack of effective biomarkers to explain the heterogeneity in therapeutic response and prognosis. As ICIs directly interact with circulating immune cells, transcriptomic changes in these cells may critically influence antitumor immunity and patient outcomes. However, the extent to which these transcriptomic features are shaped by germline and somatic variants, such as clonal hematopoiesis of indeterminate potential (CHIP), remains unclear in NSCLC.
Methods: In this study, I performed targeted sequencing on blood samples from 100 NSCLC patients for CHIP detection and conducted single-cell RNA sequencing (scRNA-seq) on 73 of these patients. Using these matched samples, I analyzed transcriptomic differences driven by germline or somatic variants, integrating single-cell expression quantitative trait loci (sc-eQTL) mapping and co-expression network analysis. Additionally, I examined transcriptional networks in myeloid cells to understand the effect of CHIP and across immune clusters for germline variants. Finally, I conducted survival analyses to assess clinical relevance.
Results: I identified distinct, CHIP-driven myeloid-specific inflammatory signatures, notably involving NF-kB activation, in NSCLC patients with CHIP mutations. Concurrently, sc-eQTL analysis identified 3,616 eGenes, many of which showed immune-specific regulatory unique to this cohort. I also derived a functional core gene score based on co-expression network involving CD8⁺ T cell differentiation, which was significantly associated with progression-free and overall survival. Notably, while germline and CHIP-related variants regulated immune states, the CD8⁺ T cell network explaining ICI response was predominantly shaped by strong co-expression regulation rather than individual genetic variants.
Conclusion: This study highlights the contribution of germline and CHIP-driven somatic mutations to immune transcriptional features in NSCLC patients treated with ICIs. However, response-explaining regulatory modules, such as those governing CD8⁺ T cell differentiation, appear to be driven more by co-expression dynamics than by direct genetic control. Our findings provide valuable resources for understanding immune regulation by genetic variants in lung cancer patients and showed the need for further validation of the identified biomarkers in independent cohorts.
