    배경:
    급성 콩팥손상(AKI)은 종종 만성콩팥병(CKD)으로 진행되어, 임상적·사회경제적으로 심각한 부담을 초래합니다. AKI 환자 수가 지속적으로 증가하고 있음에도 불구하고, 이러한 전이를 예방하거나 역전시킬 수 있는 효과적인 치료 전략은 여전히 부족한 실정입니다. 본 연구의 궁극적인 목표는 AKI에서 CKD로의 이행 과정에서 발생하는 세포 수준의 변화를 체계적으로 규명하고, 주요 세포 집단 간의 상호작용을 파악하는 한편, 질병의 진행을 억제하거나 역전시킬 수 있는 혁신적인 치료법을 탐색함으로써 미래 임상 적용을 위한 견고한 전임상 근거를 확립하는 데 있습니다.

    방법:
    본 연구에서는 고해상도 공간 전사체 분석을 통해 일측성 허혈-재관류 손상(uIRI) 이후 중요한 치료 시점을 규명하는 시공간적 프레임워크를 구축하였습니다. 또한, 단일 세포 염기서열분석(Single Cell Sequencing) 및 공간 전사체 분석을 통합적으로 수행하여 AKI 진행의 분자 기전을 규명하였고, 3D 바이오프린팅 자가 그물막 패치의 치료 효과를 생체 내에서 검증하였습니다.
    아울러, 그래핀 양자점(Graphene Quantum Dots, GQDs)을 나노의학 플랫폼으로 적용하여 AKI-to-CKD 및 후기 CKD 모델에서 그 효능을 체계적으로 평가하였습니다.


    결과:
    공간 전사체 분석 결과, AKI-to-CKD 이행을 주도하는 주요 다세포 상호작용(근위세뇨관 세포 감소, 대식세포 침윤, 근섬유아세포 활성화 등)이 확인되었으며, 손상 후 48시간과 7일 두 개의 핵심 중재 시점이 도출되었습니다.
    분자 기전적으로는 SPP1/CD44 경로를 통한 대식세포의 손상 근위세뇨관 세포로의 유인이 확인되었습니다. 3D 바이오프린팅 자가 대망 패치는 Gpx3 및 Ly6a 발현 증가, TGF-β 및 TNF-α 신호 억제를 통해 산화 스트레스와 염증을 줄였으며, Sox17⁺ 혈관 내피 전구세포 유도를 통한 혈관 재생 및 항염증성 대식세포 표현형 전환을 촉진하여 세뇨관 복구와 기능 회복을 도왔습니다. GQDs는 강력한 ROS 제거 효과와 TGF-β/Smad 신호 억제 효과를 나타냈고, 세뇨관 구조 보존, 섬유화 완화, 상피 및 혈관 손상 회복을 유도하여 신손상 진행 단계 전반에 걸쳐 광범위한 치료 가능성을 입증하였습니다.

    Background
    Acute kidney injury (AKI) frequently progresses to chronic kidney disease (CKD), posing a significant clinical and socioeconomic burden. Although the number of patients with AKI continues to rise, effective therapeutic strategies to prevent or reverse this transition remain limited. This study aims to systematically characterize cellular alterations during the AKI-to-CKD transition, identify interactions among key cell populations, and explore innovative therapeutic interventions that can halt or reverse disease progression. The ultimate goal is to establish robust preclinical evidence to facilitate future clinical translation.

    Methods
    A spatiotemporal framework was established to identify critical therapeutic windows following unilateral ischemia-reperfusion injury (uIRI) through high-resolution spatial transcriptomic analysis. Building upon this, integrated single-cell RNA sequencing and spatial transcriptomics were performed to elucidate the molecular mechanisms of AKI progression. The therapeutic effects of a 3D-bioprinted autologous omentum patch were validated in vivo, and the efficacy of graphene quantum dots (GQDs) as a nanomedicine platform was systematically investigated in AKI-to-CKD and late-stage CKD models.

    Results
    Spatial transcriptomics uncovered key multicellular interactions driving the AKI-to-CKD transition, including proximal tubular cell depletion, macrophage infiltration, and myofibroblast activation. Two pivotal intervention windows were identified at 48 hours and 7 days post-injury. Mechanistically, analysis revealed macrophage recruitment to injured proximal tubular cells via the SPP1/CD44 axis. The 3D-bioprinted omentum patch effectively restored the renal microenvironment by reducing oxidative stress and inflammation (upregulating Gpx3 and Ly6a; suppressing TGF-β and TNF-α), promoting vascular regeneration (inducing Sox17⁺ endothelial progenitor-like cells), and facilitating an anti-inflammatory macrophage phenotypic shift, ultimately supporting tubular repair and functional recovery. GQDs exhibited potent ROS-scavenging capabilities, inhibited TGF-β/Smad signaling, preserved tubular architecture, reduced fibrosis, and promoted the reversal of epithelial and vascular injury, highlighting broad therapeutic potential across progressive stages of kidney injury.

    Conclusion
    These three studies collectively establish a multidimensional strategy for addressing the AKI-to-CKD transition. High-resolution spatial transcriptomics was leveraged to visualize renal cellular dynamics and identify key therapeutic targets. Additionally, novel interventions, including 3D bioprinting-based autologous stem cell regenerative engineering and nanomedicine, were explored to mitigate disease progression.
    This work lays the foundation for next-generation therapies targeting progressive kidney disease. 3D-bioprinted autologous stem cell grafts and GQD-based nanotherapeutics not only paves the way for personalized nephrology but also advances the field toward clinically translatable, mechanism-driven therapeutic paradigms.
