미용 중재는 미용 수술과 미용 의학을 통합한 광범위한 의료적 접근으로, 외모 및 삶의 질 향상을 위해 점점 더 정교해지고 다양한 치료 옵션을 환자에게 제공하고 있습니다. 최근 재생의학의 비약적인 발전은 구조적 재건 및 피부 재생을 지원하는 융합적 혁신을 가능하게 하여, 미용 수술과 미용 의학 간의 경계를 점차 허물고 있습니다. 미용 수술은 침습적 절차를 통해 장기적인 구조적 변화를 유도하는 반면, 미용 의학은 최소 침습적 기법을 통해 표재성 개선을 중점으로 하며 회복 시간이 짧은 것이 특징입니다. 이러한 두 분야는 해부학적 깊이와 피부 표면의 질적 개선을 동시에 고려하는 통합적이고 개인 맞춤형 미용 전략으로 발전하고 있습니다.
한편, 미용 의학 분야에서는 기존의 단순한 볼륨 복원에서 벗어나, 피부 부스팅 및 진피 재생을 목적으로 한 주사형 필러의 활용이 주목받고 있습니다. 생분해성과 생물학적 자극 특성을 동시에 갖춘 폴리디오산(polydioxanone, PDO) 및 폴리젖산(polylactic acid, PLA) 기반 필러가 최근 각광받고 있으며, 본 논문에서는 PDO 및 PDLLA + HA과 같은 PDO 기반 복합 필러의 전임상 및 임상 효능을 평가하였습니다. 해당 필러들은 기계적 지지 기능뿐만 아니라, 섬유아세포 증식, 콜라겐 I 및 III 생성, TGF-β 발현을 유도함으로써 진피 내 구조적 및 생물학적 재생에 기여하는 것으로 나타났습니다. 동물모델 및 임상적 적용 결과, 이러한 생체자극형 스킨 부스터는 8~12주 동안 주름 깊이 감소, 피부 질감 개선, 콜라겐 형성 및 염증 반응 조절에 있어 유의미한 개선 효과를 보였습니다. 주사제를 통한 최소 침습적 시술은 높은 환자 만족도를 바탕으로, 외과적 치료의 효과적인 대안 혹은 보완재로 자리매김할 수 있습니다.
미용 수술 영역에서는, 탈세포화된 인간 진피로부터 유래한 세포 없는 진피 매트릭스(acellular dermal matrix, ADM)가 특히 유방 재건 및 확대술과 같은 임플란트 기반 시술에서 획기적인 역할을 수행하고 있습니다. 본 연구에서 활용된 초임계 이산화탄소(supercritical carbon dioxide, scCO₂)를 이용한 혁신적인 탈세포화 기술은 면역원성 세포 성분을 효과적으로 제거하면서도, 본래의 세포외기질(extracellular matrix, ECM) 구조를 우수하게 보존하였습니다. 본 논문에 포함된 일련의 실험 결과는 초임계 이산화탄소 처리 세포 없는 진피 매트릭스(scADM)가 뛰어난 생체적합성, 낮은 세포독성, 강력한 항염 특성뿐만 아니라, 섬유아세포 침윤, 혈관신생, 콜라겐 침착 능력을 지님을 입증하였습니다. 생체 내 랫드 모델에서의 이식 실험은 scADM이 염증성 사이토카인의 발현을 억제하고, M2 대식세포의 활성화를 통해 조직 재형성을 촉진함을 보여주었으며, 이는 해당 소재가 외과용 지지체를 넘어 재생 생체재료로서의 가능성을 시사합니다.
본 논문은 미용 재건에 사용되는 외과용 생체재료인 세포 없는 진피 매트릭스와 비수술적 피부 재생에 적용되는 생체자극성 주사 필러의 재생 효과 및 생체적합성에 대해 고찰하였습니다. 콜라겐 합성, 조직 재생, 면역 반응 조절에 대한 각각의 역할을 종합적으로 분석함으로써, 향후 미용 치료 분야에서의 시너지적 활용 가능성과 적용 확장성을 제시하고자 하였습니다.

Aesthetic interventions encompass a broad spectrum of medical treatments that integrate both aesthetic surgery and aesthetic medicine, offering patients diverse and increasingly sophisticated options to improve their appearance and well-being. The boundary between aesthetic surgery and aesthetic medicine continues to narrow as advancement in regenerative science enables cross-disciplinary innovations supporting structural reconstruction and skin rejuvenation. While aesthetic surgery involves invasive procedures with long-lasting structural outcomes, aesthetic medicine emphasizes minimally invasive techniques aimed at surface-level enhancement with minimal downtime. The two fields reflect a growing trend toward distinct approaches that address the deep anatomical framework and the superficial quality of the skin, enabling more holistic and personalized aesthetic outcomes. Aesthetic medicine has shifted from traditional volume restoration to skin boosting and dermal regeneration using injectable fillers. Polydioxanone (PDO) and polylactic acid (PLA) fillers have gained popularity due to their biodegradability and biostimulatory properties. Clinical and preclinical studies assessed in this thesis reveal that PDO-based fillers such as PDO and hybrid fillers like PDLLA-HA not only provided mechanical support but also initiate fibroblast proliferation, collagen I and III production, and TGF-β expression. In both mouse models and clinical settings, bio-stimulator skin boosters significantly reduced wrinkle depth and improved skin texture, with measurable improvements in collagen formation, skin volume, and inflammatory modulation over 8–12 weeks. The injectable feature of these products allows for minimally invasive treatments with high patient satisfaction, making them viable alternatives or complements to surgical options. In aesthetic surgery, acellular dermal matrices (ADMs) derived from decellularized human dermis have revolutionized implant-based procedures, particularly in breast reconstruction and augmentation. These matrices preserve the native scaffold of the extracellular matrix (ECM) while eliminating immunogenic cellular components, especially through innovative decellularization methods using supercritical carbon dioxide (scCO₂). The studies included in this thesis demonstrated that supercritical carbon dioxide acellular dermal matrix (scADM) exhibits excellent biocompatibility, low cytotoxicity, and potent anti-inflammatory properties, along with robust fibroblast infiltration, angiogenesis, and collagen deposition. In vivo implantation in rat models confirmed the ability of scADM to suppress pro-inflammatory cytokines, enhance M2 macrophage polarization, and promote tissue remodeling over time. These findings solidified the value of scADM not only as a surgical scaffold but also as a regenerative biomaterial capable of improving outcomes and aesthetics in reconstructive procedures. This thesis explores and elaborates on the regenerative effects and biocompatibility of bio-stimulatory injectable fillers applied in non-surgical skin rejuvenation and acellular dermal matrices (ADMs) - a surgical biomaterial used in aesthetic reconstruction. By evaluating their respective roles in collagen synthesis, tissue regeneration, and immune modulation, the thesis aims to identify pathways and complementary applications that can enhance the future of aesthetic interventions.
