피지샘암(sebaceous carcinoma, SC)은 드물지만 공격적인 악성 종양으로, 특히 절제가 불가능하거나 진행된 단계의 경우 표준 전신 치료의 효과가 제한적이다. 임상적으로 중요한 질환임에도 불구하고, SC의 발병 기전은 아직 명확히 규명되지 않았으며, 신뢰할 수 있는 예후 인자나 치료 표적 역시 확립되어 있지 않다. 본 연구는 핵수용체(nuclear receptors, NRs), 포도당 대사 변화, 면역관문 단백질이 SC의 병태생리에 미칠 수 있는 영향을 분석하고, 예후 인자 또는 치료 표적으로서의 임상적 가능성을 평가하고자 하였다.
방법
2002년부터 2019년까지 서울대학교병원, 분당서울대학교병원, 서울특별시 보라매병원에서 조직학적으로 진단된 피지샘암 39예를 후향적으로 분석하였다. 종양의 해부학적 위치에 따라 눈부속기관 피지샘암 (ocular adnexal sebaceous carcinoma, OA-SC)과 눈부속기관 이외의 피지샘암 (non-ocular adnexal sebaceous carcinoma, NOA-SC)으로 분류하고, 두 군 간 분자생물학적 특성과 임상 경과의 차이를 비교하였다. 포르말린 고정-파라핀 포매 조직에 대해 면역조직화학염색을 시행하였으며, 염색 대상은 GR(glucocorticoid receptor), AR(androgen receptor), ER(estrogen receptor), PR(progesterone receptor), GLUT1, MCT1, MCT4, CD147, pAMPK(phosphorylated AMP-activated protein kinase), PD-L1(programmed cell death-ligand 1)이었다. 단백질 발현은 반정량적으로 평가되었으며, 다양한 임상병리학적 변수들과의 연관성을 분석하였다.
결과
전체 39예 중 OA-SC는 19예, NOA-SC는 20예였다. 핵 수용체는 높은 빈도로 발현되었으며, GR이 89.7%로 가장 흔하게 발현되었고, 그 뒤를 이어 AR과 ER이 각각 51.3%, PR이 41.0% 발현되었다. 대사 관련 단백질 중에서는 CD147, GLUT1, MCT1이 각각 100%, 89.7%, 87.2%로 높게 발현되었으며, MCT4와 pAMPK는 각각 38.5%, 35.9%로 낮은 발현을 보였다. PD-L1 발현은 5예(12.8%)에서 관찰되었으며, 그 중 4예는 NOA-SC였다. OA-SC와 NOA-SC 간에는 NRs 및 당 대사 관련 단백질의 발현 양상에서 차이를 보였다. 다변량 분석 결과, 진행 병기, 낮은 AR 발현, 높은 MCT1 발현이 무전이 생존율 감소와 독립적으로 관련된 예후 불량 인자로 확인되었다 (P < 0.05).
결론
SC의 병태생리에는 호르몬 수용체 상태 및 대사 변화가 밀접하게 연관되어 있으며, 특히 AR과 MCT1은 유용한 예후 인자이자 잠재적인 치료 표적이 될 수 있다. 또한, PD-L1이 일정 수준 이상으로 발현된 일부 SC 환자군 에서는 면역관문 억제제 치료를 적용해 볼 수 있을 것으로 기대된다.

Sebaceous carcinoma (SC) is a rare but aggressive malignancy for which standard systemic therapies have shown limited efficacy, particularly in unresectable or advanced-stage cases. Despite its clinical significance, the pathogenesis of SC remains poorly understood, and reliable prognostic markers or therapeutic targets have yet to be established. This study aimed to investigate the potential pathogenic roles of nuclear receptors (NRs), altered glucose metabolism, and immune checkpoint proteins in SC, focusing on their potential clinical relevance as prognostic indicators or therapeutic targets.
Methods
A total of 39 pathologically confirmed SC cases diagnosed between 2002 and 2019 at Seoul National University Hospital, Seoul National University Bundang Hospital, and Seoul Metropolitan Government Boramae Medical Center were retrospectively analyzed. Patients were stratified into ocular adnexal sebaceous carcinoma (OA-SC) and non-ocular adnexal sebaceous carcinoma (NOA-SC) groups according to the primary tumor location, in order to explore potential differences in molecular characteristics and clinical outcomes between these anatomically distinct entities. Immunohistochemical staining was performed on formalin-fixed, paraffin-embedded tumor specimens for the following markers: glucocorticoid receptor (GR), androgen receptor (AR), estrogen receptor (ER), progesterone receptor (PR), glucose transporter 1 (GLUT1), monocarboxylate transporters (MCT1 and MCT4), CD147, phosphorylated AMP-activated protein kinase (pAMPK), and programmed cell death-ligand 1 (PD-L1). Protein expression levels were evaluated semiquantitatively and correlated with various clinicopathological parameters.
Results
Among the 39 cases, 19 involved OA-SC and 20 involved NOA-SC. Expression of NRs was frequently observed, with GR being the most commonly expressed (89.7%), followed by AR and ER (both 51.3%), and PR (41.0%). For altered glucose metabolism-related proteins, CD147, GLUT1, and MCT1 were highly expressed (100%, 89.7%, and 87.2%, respectively), while MCT4 and pAMPK exhibited relatively low expression (38.5% and 35.9%, respectively). Membranous PD-L1 expression was detected in five cases (12.8%), four of which were NOA-SC. OA-SC and NOA-SC exhibited distinct expression patterns of NRs and altered glucose metabolism-related proteins. Multivariate analysis identified advanced tumor stage, low AR expression, and high MCT1 expression as independent poor prognostic factors for metastasis-free survival (all P < 0.05).
Conclusion
These findings suggest that NR status and glucose metabolism alterations are closely associated with the pathogenesis of SC. In particular, AR and MCT1 may serve as valuable prognostic biomarkers and potential therapeutic targets. Furthermore, a subset of SC patients with PD-L1 expression within the druggable range may benefit from immune checkpoint inhibitor therapy.
