마취 중 발생할 수 있는 다양한 합병증 중, 호흡기 합병증은 수술 중에는 징후가 미미하게 나타나는 반면, 수술 후에는 중대한 이환율 및 사망률을 초래할 수 있어 특히 관리가 어려운 영역이다. 그러나 개에서의 마취 중 및 마취 후 호흡기 합병증의 발생, 관리, 그리고 예측에 대한 연구는 여전히 수의학 분야에서 부족한 실정이다. 본 연구는 이러한 연구의 공백을 해소하고자, 개에서 마취 전후 호흡기 합병증의 임상적 양상, 치료적 접근법, 그리고 위험 예측에 대해 체계적으로 조사하여 새로운 임상적 통찰을 제공하고 향후 연구의 기반을 마련하고자 하였다.
제1장에서는 전신마취 중 컴퓨터단층촬영으로 확인된 편측성 무기폐 증례 두 건을 기술하였다. 이들 증례는 기관내삽관에 의한 미주신경 자극에 따른 반사성 기관지수축에 의해 발생한 것으로 추정되었다. 두 개체 모두 마취 30분 이내에 완전한 좌측 무기폐가 발생하였으며, 마취 전 흉부 방사선검사에서는 관련한 이상 소견이 없었다. 동맥혈가스분석 결과, 산소분압 (PaO2: 130 및 199 mmHg, 100% 산소 하) 저하 및 이산화탄소분압 (PaO2: 50.7 및 48.3 mmHg) 상승이 관찰되었고, 한 마리에서는 저산소혈증 (SpO2: 82–88%)이 지속되었다. 무기폐는 폐포 모집술 (alveolar recruitment maneuver)에는 반응하지 않았으나, 발관 이후 해소되었다. 이 증례들은 기도 과민반응을 가진 개체에서 반사성 무기폐가 발생할 수 있음을 시사하며, 수술 중 철저한 호흡기 기능 확인과 조기 개입의 중요성을 의미한다.
제2장에서는 마취 중 기관지수축이 의심되는 7마리의 기계환기를 받은 개를 대상으로 프로포폴 지속정주의 기관지확장 효과를 후향적으로 평가하였다. 즉각적인 환기 최적화 및 약물 처치에도 불구하고 이들 개체는 100% 산소 하에서도 PaO2 < 300 mmHg를 보였으며, 낮은 일회호흡량과 폐순응도 감소가 관찰되었다. 프로포폴 지속정주 후 60분동안 동맥내산소분압은 유의하게 증가하였으며, 동맥내이산화탄소분압은 유의하게 변화하지 않았다. 이 결과는 프로포폴 지속정주가 마취 중 기관지수축이 의심되는 개체에서 산소화를 개선할 수 있으며, 마취 중 호흡기 장애에 대한 치료적 대안으로 고려될 수 있음을 시사한다.
제3장에서는 168마리의 기계환기를 받은 개를 대상으로, 마취 유도 직후 측정한 동맥내산소분압:흡입산소농도 비율 (PaO2:FIO2 ratio)과 동적 폐순응도가 마취 후 저산소혈증 (PaO2 < 80 mmHg)을 예측할 수 있는지 평가하였다. 낮은 PaO2:FIO2 ratio와 동적 폐순응도 값은 마취 후 저산소혈증과 유의하게 연관되었으며, 수신자조작특성 (ROC) 곡선 분석에서 양호한 예측력 (곡선하면적 각각 0.696 및 0.716)을 보였다. 또한 민감도 95% 기준으로 최적 절단값을 부트스트랩 분석을 통해 도출하였다. 이 결과는 마취 중 동맥내산소분압 및 동적 폐순응도 확인이 마취 후 호흡기 합병증을 조기에 예측하는 지표로 활용될 수 있음을 시사한다.
제4장에서는 24개의 마취전후 변수를 이용하여 마취 후 저산소혈증을 예측하는 앙상블 머신러닝 모델을 개발하였다. 원 데이터 (137마리)와 합성 소수 오버샘플링 기법으로 생성한 증강 데이터 (105마리)를 통합하여 클래스 불균형 (저산소혈증군과 비저산소혈증군)을 보정하였으며, 이 중 별도의 검증 데이터셋 (32마리)에서 모델 성능을 평가하였다. 해당 모델은 88.4%의 정확도와 수신자조작특성 곡선에서 우수한 예측력 (곡선하면적 0.886)을 보였다. 주요 예측 변수로는 단두종 여부, 폐순응도, 좌측측와위, 일회호흡량, 최대기도압이 도출되었다. 이는 머신러닝 기반 저산소혈증 위험 예측 모델이 마취 후 호흡기 합병증 조기 탐지에 유용할 수 있음을 보여준다.
본 연구는 개에서 마취 전후 호흡기 합병증의 임상적 특징 규명부터 조기 예측 모델에 이르기까지 체계적인 접근을 통해, 호흡기 합병증에 대한 임상적 경각심을 높이고, 정밀 예측 도구를 통한 수술 전후 관리 향상의 가능성을 제시하였다. 본 연구 결과는 향후 수의마취학 분야에서 호흡기 합병증 예방 및 치료 전략 수립에 기여할 것으로 기대된다.

Among various complications that may arise during anesthesia in dogs, respiratory complications remain particularly challenging due to their subtle intraoperative presentation and their potential to cause significant postoperative morbidity and mortality, even with prompt intervention. However, the occurrence, management, and prediction of intra- and post-anesthetic respiratory complications in dogs remain underexplored in the veterinary literature. This study aimed to address this gap by investigating the clinical presentation, therapeutic approaches, and risk prediction of peri-anesthetic respiratory complications in dogs, thereby offering new clinical insights and providing a foundation for future research in veterinary anesthesiology.
Chapter 1 described two cases of unilateral lung collapse, identified on computed tomography (CT), during general anesthesia in dogs, presumed to result from intubation-related vagal stimulation leading to reflex bronchoconstriction. Both dogs developed complete left lung atelectasis within 30 minutes of intubation without evidence in pre-anesthetic thoracic radiographs. Arterial blood gas (ABG) analysis revealed impaired oxygenation (PaO₂: 130 and 199 mmHg in 100% oxygen) and ventilation (PaCO₂: 50.7 and 48.3 mmHg), with one dog exhibiting sustained hypoxemia (SpO₂: 82–88%). Atelectasis was unresponsive to recruitment maneuvers but resolved following extubation. These cases highlighted the potential for reflex-mediated peri-anesthetic atelectasis in dogs with airway hyperreactivity and emphasized the importance of vigilant respiratory monitoring and early supportive interventions.
Chapter 2 retrospectively evaluated the potential bronchodilatory effect of propofol constant rate infusion (CRI) in seven mechanically ventilated dogs with suspected intra-anesthetic bronchoconstriction. Despite immediate interventions, including ventilatory optimization and pharmacologic management, the patients continued to exhibit impaired oxygenation (PaO₂ < 300 mmHg in 100% oxygen) and abnormal ventilatory parameters, including low tidal volume and decreased respiratory compliance. Following administration of propofol CRI, PaO₂ significantly increased over 60 minutes, while PaCO₂ remained unchanged. These findings suggested that propofol CRI may improve oxygenation in dogs with suspected bronchoconstriction and could be considered as a therapeutic option in peri-anesthetic respiratory compromise.
Chapter 3 investigated whether post-induction PaO₂:FIO₂ and dynamic compliance (Cdyn) could predict post-anesthetic hypoxemia (PaO₂ < 80 mmHg) in 168 mechanically ventilated dogs. Lower PaO₂:FIO₂ and Cdyn values measured after induction were significantly associated with post-anesthetic hypoxemia. Both parameters showed good to satisfactory diagnostic test accuracy in receiver operating characteristics (ROC) curve analysis [area under the curve (AUC) 0.696 and 0.716, respectively], with optimal cut-off values determined via bootstrap analysis with a fixed sensitivity of 95%. These findings suggested that intra-anesthetic monitoring of oxygenation (PaO₂) and respiratory mechanics via spirometry (Cdyn) may provide supportive information for identifying dogs at increased risk of postoperative respiratory compromise.
Chapter 4 developed an ensemble machine learning model to predict post-anesthetic hypoxemia in dogs using 23 peri-anesthetic features from a retrospective dataset. The model was trained on a combined dataset of original (n = 137) and SMOTE-generated (n = 105) cases to address class imbalance (hypoxemic versus non-hypoxemic), and evaluated on a separate validation set (n = 32). It achieved an accuracy of 88.4% and an AUC of 0.890. Key predictive features included brachycephalic breeds, dynamic pulmonary compliance, peak airway pressure, left lateral recumbency, and American Society of Anesthesiologists (ASA) physical status classification. These results highlighted the potential of a novel machine learning model for early risk identification and emphasize the clinical relevance of specific factors in predicting postoperative respiratory complications.
Collectively, this study offered a comprehensive approach to understanding and managing peri-anesthetic respiratory complications in dogs. Through the clinical identification of reflex-mediated atelectasis, the therapeutic use of propofol for suspected bronchoconstriction, and the comparative evaluation of intra-anesthetic oxygenation and Cdyn, the research built a foundation for early risk detection. Furthermore, the development of a robust machine learning model demonstrated the feasibility of data-driven prediction, identifying specific risk factors. These findings raised clinical awareness of respiratory complications in veterinary anesthesia and demonstrate the potential utility of precision prediction tools for improving perioperative respiratory outcomes.
