Kidney replacement strategies, including transplantation and emerging stem cell-based approaches, face challenges related to metabolic and redox imbalances, two intertwined biological processes, that impact long-term success. I investigate these factors through two distinct studies. First, the clinical significance of metabolic acidosis (MA) in kidney transplant recipients (KTRs) was examined. A large, multicenter retrospective cohort study (n=2,318) demonstrated that MA (serum TCO2 < 22 mmol/L) is prevalent in KTRs, even with relatively preserved graft function. Utilizing conventional, time-varying, and marginal structural Cox models, MA was identified as a significant and independent risk factor for long-term graft loss, death-censored graft failure, and patient mortality, highlighting MA as a potential therapeutic target in KTRs. Second, the study explored the role of ferroptosis, an iron-dependent form of regulated cell death, in pluripotent stem cells, relevant for regenerative medicine. Using isogenic naive and primed mouse embryonic stem cell (ESC) pairs, it was found that naive ESCs, unlike primed ESCs, are highly susceptible to ferroptosis upon antioxidant withdrawal. This susceptibility stems from their unique metabolic profile, including high mitochondrial oxidative phosphorylation, reduced endogenous coenzyme Q10 synthesis via the mevalonate pathway, and elevated iron uptake via Transferrin Receptor 1. Glutathione Peroxidase 4 (GPX4) was identified as crucial for protecting naïve ESCs from ferroptosis, maintaining their viability and pluripotency. Together, these studies underscore the critical roles of metabolic homeostasis (acid-base balance) and redox regulation (ferroptosis defense via GPX4) in determining outcomes in both clinical kidney transplantation and fundamental stem cell biology relevant to future kidney replacement strategies.
