섬유화 질환은 세포 외 기질에 콜라겐이 비정상적으로 축적되어 다양한 장기의 기능 장애를 초래하는 것을 특징으로 한다. 피부에서는 과도한 콜라겐 침착이 비대성 흉터와 켈로이드로 나타나며 현재까지 효과적인 치료제가 없는 실정이다.
HSP47(heat shock protein 47)은 적절한 콜라겐 형성에 필수적이며 병적인 섬유증의 형성에 기여한다. 따라서 HSP47 억제제를 찾는 것은 섬유화 질환의 치료제 개발의 주요 과제였다. 본 연구에서는 비대성 흉터 치료를 위한 HSP47 억제제로서 벤즈브로마론(Benzbromarone, BBR)이 효과가 있음을 확인하였다. BBR은 HSP47과 결합하여 켈로이드 환자의 섬유아세포에서 콜라겐 생성 및 분비를 억제하고 HSP47과 콜라겐 간의 상호작용을 저해했다. 흥미롭게도 BBR은 HSP47을 억제할 뿐만 아니라 프로테아좀 의존적 분해를 촉진하는 분자 접착 분해제(molecular glue)로도 작용한다. 이러한 분자 메커니즘을 통해 BBR은 화상과 절제 피부 손상을 입은 미니 돼지와 랫드의 비대성 흉터를 효과적으로 감소시켰다.
설포닐우레아(Sulfonylurea)는 제2형 당뇨병 관리에 오랫동안 사용되어 왔다. 그러나 심각한 저혈당과 이차적인 설포닐우레아 실패의 위험으로 인해 임상적 사용에 제한을 받아왔다. 설포닐우레아로 인한 저혈당증의 기저 메커니즘은 잘 알려져 있지만, 설포닐우레아 내성을 유발하는 정확한 요인은 아직 잘 알려지지 않았다. 본 연구는 인슐린종 세포와 동물 모델에서 설포닐우레아 내성의 분자적 기초를 밝히는 것을 목표로 하였다.
설포닐우레아에 내성이 생긴 인슐린종 세포는 정상적인 인슐린종 세포에 비해 휴지 막 전위가 상승하였다. 전사체 분석을 통해 설포닐우레아 내성세포의 유전자의 차별적 발현이 밝혀졌으며, 그 중 막전위와 인슐린 분비에서의 역할을 가지고 있는 kcnn3(Potassium Calcium-Activated Channel Subfamily N Member 3)가 설포닐우레아 내성 세포주에서 높게 발현됨을 확인했다. kcnn3의 단백질 산물인 SK3(Small conductance calcium activated potassium channel 3)의 western blot 분석 결과 설포닐우레아 노출 기간과 비례하여 SK3 단백질의 상향 조절이 확인되었다. 동물모델인 랫드에서 설포닐우레아를 장기간 투여하면 혈당강하 효능이 감소하고 췌장의 랑게르한스 섬(Islets of Langerhans)에서 SK3 발현이 증가함을 확인하였다.
본 논문에서는 임상적으로 오랜 기간 사용되어온 벤즈브로마론과 글리벤클라마이드(Glibenclamide)에 대한 새로운 약리 기전 연구를 통해 벤즈브로마론은 HSP47의 단백질 분해제로 작용하여 섬유화질환의 치료제로 사용될 수 있음을 제시하였고, 글리벤클라마이드의 장기 사용은 SK3 채널의 과발현을 유도함을 밝혀내어 설포닐우레아 내성의 기저에 있는 분자 메커니즘을 규명하였다.

Fibrotic diseases are characterized by the abnormal accumulation of collagen in the extracellular matrix, leading to dysfunction of various organs. In the skin, excessive collagen deposition manifests as hypertrophic scars and keloids, for which there are currently no effective treatments. Heat Shock Protein 47(HSP47) is essential for proper collagen formation and contributes to the formation of pathological fibrosis. Therefore, finding HSP47 inhibitors has been a major challenge in the development of therapies for fibrotic diseases. In the present study, we confirmed the efficacy of benzbromarone(BBR) as an HSP47 inhibitor for the treatment of hypertrophic scarring. BBR bound to HSP47, inhibited collagen production and secretion in fibroblasts from keloid patients and impaired the interaction between HSP47 and collagen. Interestingly, BBR not only inhibits HSP47, but also acts as a molecular glue that promotes proteasome-dependent degradation. Through these molecular mechanisms, BBR effectively reduced hypertrophic scarring in mini pigs and rats with burn and/or excisional skin injuries.
Sulfonylureas have long been used in the management of type 2 diabetes, but their clinical use has been limited by the risk of severe hypoglycemia and secondary sulfonylurea failure. While the mechanisms underlying sulfonylurea-induced hypoglycemia are well understood, the exact factors that lead to sulfonylurea resistance remain poorly understood. The present study aimed to elucidate the molecular basis of sulfonylurea resistance in insulinoma cells and animal models.
Insulinoma cells that became resistant to sulfonylureas had elevated resting membrane potential compared to sensitive cells. Transcriptome analysis revealed differential expression of genes in sulfonylurea-resistant cells, among which kcnn3(Potassium Calcium-Activated Channel Subfamily N Member 3), which has roles in membrane potential and insulin secretion, was highly expressed in sulfonylurea-resistant cell lines. Western analysis of SK3(Small-conductance calcium-activated K+ channels 3), the protein product of kcnn3, confirmed the upregulation of SK3 protein in proportion to the duration of sulfonylurea exposure. In an animal model, rats, long-term administration of sulfonylureas decreased their hypoglycemic efficacy and increased SK3 expression in the islets of Langerhans in the pancreas.
In this dissertation, I newly elucidated the wound healing effects of benzbromarone, a uricosuric agent that has been used clinically for many years, and the mechanisms underlying sulfonylurea failure, an antidiabetic drug. Through addressing these pharmacological challenges, I identified HSP47 and SK3 as novel drug targets.
