    Climate change has led to more frequent and intense extreme weather events, including abnormally high temperatures that pose growing health and economic challenges. While previous studies have shown a clear relationship between rising temperatures and increases in mortality, emergency department visits, and hospitalizations, little is known about the effects of heat on primary care utilization, which plays a critical role in managing mild and chronic conditions.
    This dissertation investigates the health impacts of high temperatures using nationally representative outpatient visit data to primary hospitals in South Korea. The customized database includes anonymized individual-level data for approximately 10 million people, covering medical service use, insurance contributions, and residential information. Focusing on primary care utilization during the summer, the study identifies non-linear and cumulative effects of heat exposure, with short-term reductions and longer-term increases in outpatient visits. These effects vary by region, age group, and working environment, reflecting differences in climate adaptation capacity across populations.
    Chapter 3 examines regional disparities by endogenously estimating temperature thresholds effects for outpatient care in metropolitan and non-metropolitan areas. The results show that daily maximum temperatures above 27.2℃ significantly reduce outpatient visits, potentially disrupting the continuity of care and increasing avoidable hospitalizations or deaths. In contrast, cumulative heat exposure over the prior two weeks increases outpatient visits in non-metropolitan regions, where vulnerable populations and limited access to primary hospitals are concentrated. These findings underscore the need to incorporate climate vulnerability into regional policy design to promote health equity.
    Chapter 4 focuses on outpatient mental health care, using Poisson hurdle models to examine age-specific responses to heat exposure among individuals diagnosed with depressive or anxiety disorders. The results reveal that adolescents are more likely to initiate and increase clinic visits in response to high temperatures, whereas middle-aged and older adults tend to reduce their visit frequency. Given existing evidence that high temperatures increase suicide risk and psychiatric emergencies, such reductions may reflect avoidance behavior rather than symptom improvement, indicating the need for policies to support continuous mental health care during extreme weather.
    Chapter 5 examines the relationship between occupational vulnerability, environmental infrastructure, and labor productivity. Regions with higher proportions of outdoor workers experience larger increases in outpatient visits under high-temperature conditions, while those with greater park areas per capita show reduced visit rates. Estimates of labor hour losses suggest that temperature-induced outpatient care may translate into productivity loss. These findings contribute to the literature by offering empirical evidence on heat-related vulnerability among workers and the mitigating effects of environmental infrastructure.
