    암 유전체 연구는 종양의 발생과 진행, 전이에 관련하는 분자적 기전을 밝혀내는데 핵심적인 역할을 해왔다. 그러나 지금까지의 연구에도 불구하고, 여전히 희귀한 암종의 유전체 및 전사체적 특성이나 전이를 유발하는 유전자 수준의 기전은 아직 충분히 규명되지 않았다. 특히 전이의 분자적 기전은 암종에 따라 매우 다양하며, 고형암 치료에 있어 여전히 가장 중요한 과제로 남아 있다. 본 학위논문에서는 세 가지 독립된 연구를 통해 육종 및 유방암에서의 전이 관련 및 치료 함의를 나타내는 유전체 및 전사체 특징을 다각도로 탐색하였다. 분석 대상은 내막 육종, 뇌전이를 동반한 육종, 그리고 전이 유무에 따른 유방암이다.
    첫 번째 연구에서는 극히 희귀하고 예후가 불량한 내막 육종(intimal sarcoma)의 분자 아형을 규명하였다. 서울대병원 및 공개 데이터(MSKCC, AACR GENIE)의 DNA, RNA, 임상 패널 데이터를 통합 분석한 결과, 뚜렷하게 구분되는 두 개의 분자 아형을 확인하였다. 하나는 유전자 복제수 변이(CNV)가 높게 나타나는 아형(CNV-H)으로 면역 결핍형의 특징을 함께 보였고, 다른 하나는 MLH1 유전자의 변이를 수반해 종양 변이 부담(TMB)이 높게 나타나는 아형(MSI-H-like)으로 염증성 신호 전달 경로와 종양 침윤 림프구(TILs)가 풍부한 면역 활성형 특성을 보였다. 특히 후자의 경우, 일부 환자에서 면역관문 억제 치료에 반응을 보인 사례가 관찰되어 향후 치료적 가능성을 시사한다.
    두 번째 연구는 뇌전이를 동반한 육종 환자 5명의 사례를 바탕으로, 원발 병변 및 타 전이 병변과 뇌 전이 병변 간의 유전체 및 전사체 특성을 분석하였다. 뇌전이 병변의 프로파일은 전반적으로 원발 종양 및 다른 전이 병변과 비슷했으나, CNV 및 뇌 특이적 미세환경에서의 염색질 구조 조절 등의 재프로그래밍 가능성을 시사하였다.
    세 번째 연구는 유방암에서의 전이성 특성을 규명하기 위해 환자 종양 및 정상 샘플과 환자유래 이종이식(PDX) 모델을 이용한 DNA 및 RNA 시퀀싱 분석을 수행하였다. 원발암과 PDX간의 주요 유전자 변이가 잘 보존되어 유전적 유사성이 확인되었으며, 전이성 종양에서는 저산소증(hypoxia), MYC 신호전달 경로가 유의미하게 활성화되어 있음을 확인하였다. 면역세포 구성을 분석한 결과, 전이성 종양에서 M2 대식세포의 비율이 증가해, 종양 진행 및 면역 회피에 기여할 가능성을 보였다. PDX 모델의 단일세포 전사체 분석에서도 이러한 경향성은 일관되게 관찰되었으며, 특히 AK1과 MET 유전자의 발현이 모든 전이 경향성이 높은 샘플에서 증가한 것을 확인했다. 또 전이성 PDX 모델에서 M2 대식세포 마커 유전자(Arg1)의 발현이 증가한 대식세포가 다수 존재하였고, 이들 세포에서 저산소증과 해당과정 등 관련 경로의 활성화가 확인되어, M2 대식세포의 특성을 가지는 것을 확인하였다. 반대로, 전이 경향성이 낮은 PDX 모델에서는 면역 활성 관련 특성(TLSs)이 상대적으로 두드러지게 나타났다.

    Over the past few decades, progress in cancer genomics have provided important insights in understanding the molecular basics of tumor development and progression. These efforts have uncovered various genetic and transcriptomic alterations that contribute to cancer biology. However, there are still many questions unanswered, especially regarding the molecular characteristics of rare cancer types and the specific mechanisms that drive metastasis. Since these mechanisms can vary greatly depending on the cancer types, it is important to conduct more detailed and cancer-specific studies that are necessary to better understand how metastasis occurs in different contexts.
    In this dissertation, I present an integrative, multi-platform investigation focusing on molecular features affecting therapeutic strategies and metastasis of cancers through the lens of genomics and transcriptomics—especially intimal sarcoma, brain-metastatic soft tissue sarcoma, and metastatic versus non-metastatic primary breast cancer.
    In the first part, molecular subtypes of intimal sarcoma – very rare and aggressive -are characterized through integrated analysis of whole-exome, transcriptome, and clinical next generation sequencing (NGS) data from Seoul National University Hospital (SNUH) and public cohorts. We identified two distinct molecular subtypes with differing immune profiles: a copy number variation-high (CNV-H) subtype with an immune-desert phenotype and a microsatellite instability-high-like (MSI-H-like) subtype enriched for inflammatory signaling and tumor-infiltrating lymphocytes (TILs), the latter showing potential responsiveness to immune checkpoint blockade.
    In the second part, five cases of soft tissue sarcoma with brain metastases are analyzed to explore their molecular features. Genomic and transcriptomic profiles of brain metastatic lesions were generally similar to those of primary or other metastatic sites, though copy number alterations and pathway-level transcriptomic differences suggest possible reprogramming in the brain microenvironment.
    In the final part, we analyzed integrative genomic and transcriptomic datasets of primary breast cancers, matched normal tissues, and corresponding patient-derived xenograft (PDX) models. By integrating mutation, CNV, and molecular subtypes, we confirmed overall genomic and transcriptomic features in breast cancer patients. We observed a generally high level of concordance between primary and xenograft tumors using the landscape, supporting the reliability of the PDX model. Subsequently, we next compared metastatic and non-metastatic primary tumors of patients using bulk RNA sequencing data and identified features associated with hypoxia, epithelial-mesenchymal transition (EMT) signaling, increased pro-tumoral M2-like macrophage signatures, and downregulated immune features in the metastatic group. To explore these observations in a cell-type specific manner, we performed single-cell RNA sequencing (scRNA-seq) of xenograft tumors. We revealed Arg1-positive macrophages that show M2-like characteristics and MET-expressing cancer cells in metastatic tumors, whereas non-metastatic tumors showed immune-related features resembling tertiary lymphoid structures (TLS) in the stromal context. These results highlight that both tumor-intrinsic and microenvironmental factors are jointly associated with metastatic behavior.
    Taken together, this dissertation helps to shed light on the complex molecular heterogeneity in different types of cancer. This work suggests potential biomarkers and molecular pathways that could be useful in developing more personalized approaches to cancer treatment in the future.
