푸마르산 수화효소 결핍 신세포암(Fumarate hydratase-deficient renal cell carcinoma, FH-deficient RCC)은 푸마르산 수화효소(fumarate hydratase, FH) 효소 기능의 상실로 정의되는 신장 악성 종양으로, 임상적으로 매우 공격적인 경과와 낮은 생존율을 보이는 것으로 알려져 있다. 본 연구는 FH-deficient RCC의 조직병리학적 및 면역조직화학적 특징을 체계적으로 정리하고, 임상 진단에 유용한 지표를 도출하는 것을 목표로 하였다.
1995년 7월부터 2022년 8월까지 서울대학교병원에서 수술 절제 또는 생검을 통해 확보한 조직 표본 중, FH 단백 발현 소실 및 S-(2-succino)cysteine(2SC) 과발현이 면역조직화학염색(immunohistochemistry, IHC)으로 확인된 총 29예의 FH-deficient RCC 사례를 연구 대상으로 포함하였다. 이들에 대해 헤마톡실린-에오신 염색(H&E), 면역조직화학염색, 전장유전체서열분석(whole-exome sequencing, WES)을 실시하였다.
조직형태학적 분석 결과, 모든 종양에서 두드러진 핵인체와 그를 둘러싼 광륜(perinucleolar halo), 호산성 세포질, 관유두상 성장 양상이 공통적으로 관찰되었고, 기질 내 거품세포(foam cells)의 부재가 특징적이었다. 일부 증례에서는 결합조직 섬유화 반응(desmoplastic stromal reaction) 및 종양침윤 림프구(tumor-infiltrating lymphocytes, TILs)가 동반되었으며, 1예에서는 저등급 핵형과 호산과립세포(oncocyte)를 중심으로 한 비전형적 양상이 관찰되었다.
면역조직화학 염색에서는 CD10, keratin 7, keratin 20, anaplastic lymphoma kinase (ALK), GATA3 등은 대부분 음성이었으며, PAX8과 alpha-methylacyl-CoA racemase(AMACR)는 증례별로 다양한 강도의 양성 반응을 보였다. 전장유전체서열분석 결과, 17건 중 15건(88.2%)에서 FH 유전자의 병적 변이(미스센스, 스플라이스 사이트 변이, 틀이동 변이 등)가 확인되었으며, 나머지 2건에서는 비암호(non-coding) 부위 또는 동의적(synonymous) 변이만이 관찰되었다.
결론적으로 본 연구는 FH-deficient RCC의 조직학적, 면역표현형적, 분자유전학적 특징을 통합적으로 기술하였으며, 이 종양의 정확한 진단과 향후 적절한 치료 전략 수립에 기여할 수 있는 실질적인 정보를 제공한다.

Fumarate hydratase (FH)-deficient renal cell carcinoma (RCC) is an uncommon subtype of renal malignancy defined by the loss of FH enzyme activity. It demonstrates marked clinical aggressiveness with unfavorable survival rates. This work presents an extensive analysis of FH-deficient RCC, integrating histopathological, immunohistochemical, and genomic methodologies to clarify features that may inform clinical management.
Twenty-nine cases of FH-deficient RCC, confirmed by loss of FH expression and increased S-(2-succino)cysteine (2SC) staining on immunohistochemistry, underwent retrospective analysis of surgical and core biopsy samples from a single medical center between July 1995 and August 2022. Histomorphological examination identified prominent eosinophilic nucleoli surrounded by perinucleolar halos, eosinophilic cytoplasm, and mainly tubulopapillary architecture. Notably, stromal foam cells were absent, while a subset showed desmoplastic stroma and tumor-infiltrating lymphocytes (TILs). A single tumor case exhibited low-grade oncocytic features.
Immunohistochemically, these tumors were consistently negative for CD10, keratin 7, keratin 20, anaplastic lymphoma kinase (ALK), and GATA3, yet demonstrated uniform PAX8 positivity. Alpha methylacyl-CoA racemase (AMACR) expression was variable among cases.
Whole-exome sequencing conducted on 17 cases revealed a range of pathogenic FH alterations—including missense mutations, splice site variants, deletions, and frameshift mutations—in 15 tumors. Two tumors exhibiting confirmed FH loss by IHC lacked detectable non-synonymous FH mutations, indicating the potential involvement of non-coding or epigenetic mechanisms.
In conclusion, this study comprehensively characterizes the clinical, histopathological, immunophenotypic, and genetic features of FH-deficient RCC, thereby advancing accurate recognition, diagnosis, and clinical management of this aggressive form of renal neoplasm.
