목적: 본 연구는 유방 및 구역 림프절의 세기 조절 방사선 치료 중, 호흡 운동의 영향을 4차원 컴퓨터 단층 촬영 (4D-CT)에 기반한 용량 분포 계산을 통해 분석하고자 한다.
방법: 40명의 유방암 환자가 연구에 포함되었다. 각 환자에 대해 3차원(3D)-CT 및 호흡 구간을 10분할한 4D-CT를 촬영하였고, 치료 용적은 3D-CT 및 4D-CT의 모든 호흡 구획에 표기되었다. 세기 조절 방사선 치료 계획은 3D-CT를 기반으로 생성되었으며 (3D 계산 선량, 43.2Gy, 총 16회), 각 구획 별 치료 계획 매개 변수는 10개 호흡 구간으로 나누어져 해당 단계별 선량 분포에 투영 시켜 계산 및 합산하였다 (4D 계산 선량). 원본 3D 계산 선량 및 4D 계산 선량 간의 치료 계획 선량 비교를 시행하였다.
결과: 호흡 운동으로 인해 생성된 4D 계산 선량은 기존의 3D 계산 선량에 비해 일치도 및 균일성 지수에서 유의한 차이를 보였다. 심장 및 동측 폐의 평균 선량은 4D 계산 선량에서 기존의 3D 계산 선량에 비해 유의하게 높았다 (각, 0.27 Gy, p < 0.001 및 0.67 Gy, p < 0.001). 좌측 유방암 환자에게 조사된 심장 선량이 우측 유방암 환자에게 조사된 심장 선량에 비해 더 큰 3D 및 4D 평균 선량 차이가 관찰되었다 (각, 0.35 Gy, p = 0.013 및 0.20 Gy, p = 0.011). 평균 내유림프절 선량은 4D 계산 선량이 기존 3D 계산 선량에 비해 유의하게 컸다 (1.54 Gy, p < 0.001). 부스트 방사선치료에서는 하부 사분면 부스트의 평균 선량 차이가 상부 사분면 부스트에 비해 유의하게 컸다 (각, 0.14 Gy, p = 0.007 및 0.28 Gy, p = 0.093).
결론: 호흡 운동과 다엽 콜리메이터의 변조의 상호작용이 정상 장기 및 하부 사분면 부스트뿐 아니라, 특히 심장과 내유림프절의 방사선 조사량을 크게 증가시켰다. 심장 질환이 있는 환자, 유방 전절제술 후 방사선치료를 받는 환자, 또는 골다공증을 가진 고위험군에서는 심장과 내유림프절에 대한 과선량으로 인해 심장 독성, 피부 독성, 또는 늑골 골절의 위험이 증가할 수 있다. 이러한 위험을 줄이기 위해 deep inspiration breath-hold (DIBH)나 지속적 양압 호흡 (CPAP) 장치와 같은 호흡 조절 전략이 심장과 내유림프절의 비의도적 선량 증가를 최소화하는데 도움이 될 수 있다.

Objectives: To analyze the effect of breathing motion during intensity-modulated radiation therapy (IMRT) of the breast and regional lymphatics by calculating dose distribution based on four-dimensional computed tomography (4D-CT).
Methods: Forty breast cancer patients were enrolled in this study. Three-dimensional (3D)-CT along with ten phases of 4D-CT were obtained for each patient, along with target volumes separately delineated on both 3D-CT and all phases of 4D-CT. The IMRT plans were derived from 3D-CT (43.2 Gy in 16 fractionations). The parameters for each segment were divided into ten respiratory phases and projected onto the corresponding phase-specific dose distributions to be calculated and summated (4D-calculated dose). The plan parameters between the original and 4D-calculated doses were compared.
Results: There was a significant distortion in the 4D-calculated plan induced by respiratory motion in terms of conformity and homogeneity index compared to the original 3D plan. Mean doses of the heart and the ipsilateral lung were significantly higher in the 4D-calculated plan compared to those of the original 3D plan (0.27 Gy, p < 0.001 and 0.67 Gy, p < 0.001, respectively). A greater mean dose difference in original 3D and 4D-calculated plans was observed in the left-irradiated heart compared to the right-irradiated heart (0.35 Gy, p = 0.013 and 0.20 Gy, p = 0.011, respectively). The mean internal mammary node (IMN) dose was significantly greater in the 4D-calculated plan than in the original 3D plan (1.54 Gy, p < 0.001). In boost radiotherapy, the mean dose difference was notably greater in the lower quadrant boost compared to the upper quadrant boost (0.14 Gy, p = 0.007 and 0.28 Gy, p = 0.093, respectively).
Conclusions: The interplay between respiratory motion and MLC modulation increased the radiation doses, particularly in the heart and IMN. In high-risk patients, such as those with underlying cardiac comorbidities, postmastectomy status, or osteoporosis, dose elevation may increase the risk of cardiac events, skin toxicity, and rib fractures. Motion management strategies, such as deep inspiration breath-holding or continuous positive airway pressure, may help reduce these risks by minimizing unintended dose escalation.
