    생분해성 이식용 의료기기는 일정 기간 동안 이식부위에서 치료적 기능을 수행한 후 체내에서 분해되는 특성을 가지고 있다. 따라서 생분해성 이식용 의료기기는 분해 특성과 이식 후 물리화학적 특성 변화에 대한 정확한 평가가 필수적이다. 그러나 기존의 생리식염수나 인산완충용액 기반의 생체외 실험은 실제 이식 부위의 생리학적 조건을 충분히 반영하지 못하고 있어, 이식 후 의료기기의 생체내 성능을 예측하기에는 한계가 있다. 이에 본 학위논문에서는 이러한 한계를 극복하기위해 생분해성 이식용 의료기기의 이식 후 물리화학적 특성 변화와 성능 저하를 보다 정밀하게 평가할 수 있는 생체모사 생리 환경 기반 생체외 평가 방법을 개발하였다. 추가로 2종의 의료기기를 제작하여 개발한 평가 방법을 활용하여 물리화학적 특성 및 성능의 변화를 정확하게 평가하였다. 이를 위해 본 논문은 다음과 같은 세부 연구를 수행하였다. (1) 인공타액을 이용한 구강 생리모사 조건 개발, (2) 돼지 혈액 및 인공체액을 활용한 두개골 결손 생리모사 조건 개발 (3) 치주조직 재생 유도재와 두개골 성형용 플레이트 제작 및 생리모사 조건 6개월 보관 후 물리화학적 특성과 성능 변화 평가 (4) 생리모사 조건 실험 결과를 바탕으로 이식 후 분해기간 예측 (5) 생분해성 마이크로니들의 제조 방식에 따른 물리기계적 특성 및 성능 비교분석. 이와 같은 연구의 주요 결과는 다음과 같다. 3장에서는 생분해성 치주조직 재생 유도재를 대상으로, 인공 타액을 이용한 구강 생리모사 조건을 설정하고 장기간 노출에 따른 분자량 감소, 인장강도 저하 등의 물리화학적 변화를 정량적으로 평가하였다. 그 결과, 구강 생리모사 조건에서의 생체외 데이터는 토끼 피하 이식 실험에서 관찰된 생체내 분해 양상과 유사한 경향을 보여 해당 평가 방법의 유효성이 입증되었다. 4장에서는 생분해성 두개골 성형용 플레이트의 성능 평가를 위해 돼지 혈액 및 인공체액을 활용한 두개골 결손 생리모사 조건을 개발하였다. 24주에 걸친 평가 결과, 질량과 기계적 강도는 유의하게 감소하였으며, 생체내 결과와 방향성 측면에서 일관된 분해 경향을 나타냈다. 또한, 보정계수를 적용한 선형 회귀모델을 통해 실사용 수명을 예측할 수 있는 기초자료를 확보하였다. 5장에서는 두 가지 방식(마이크로 몰딩, 액적 기반 공기분사)으로 제작된 생분해성 마이크로니들의 기계적 강도, 치수 정밀도, 피부 침투 성능 등을 비교하였다. 마이크로 몰딩 방식은 구조적 안정성과 강도에서 우수하였고, 액적 기반 공기분사 방식은 제조 속도와 피부 침투 효율에서 뛰어났다. 이 연구는 제조 방법에 따른 기능적 차이를 명확히 하여 임상 적용에 있어 기기 설계 선택에 유용한 기준을 제시하였다. 이와 같은 결과를 종합하면, 생리 환경을 반영한 생체외 평가 방법이 실제 생체 내 분해 양상과 밀접한 상관관계를 가지며, 동물실험을 대체하거나 보완할 수 있는 실용적인 평가 도구로 활용될 수 있음을 확인하였다. 본 연구에서 제시한 평가 방법은 생분해성 의료기기의 초기 설계, 규제 심사, 수명 예측 등 다양한 단계에서 활용 가능하며, 향후에는 다양한 생체 적용 부위 및 재료로의 확장이 기대된다.

    Biodegradable implantable medical devices are designed to function therapeutically for a limited time before degrading within the body. Accurate evaluation of their degradation behavior and post-implantation physicochemical changes is essential. However, conventional in vitro tests using saline or phosphate-buffered solutions often fail to reflect the physiological conditions of implantation sites, limiting their ability to predict in vivo performance. To address this limitation, this dissertation developed physiologically simulated in vitro evaluation methods for more precise assessment of the degradation and performance of biodegradable devices after clinical application. The thesis conducted the following detailed studies. (1) development of an oral physiological simulation using artificial saliva, (2) development of a cranial defect simulation using pig blood and simulated body fluid, (3) evaluation of physicochemical and functional changes in dental barrier membranes and cranioplasty plates after 6-month exposure, (4) prediction of in vivo degradation duration using linear regression models based on in vitro data, and (5) comparative analysis of microneedles fabricated via micro molding and droplet-born air blowing (DAB). The major findings of the study are as follows. In Chapter 3, a simulated oral physiological condition was established using artificial saliva to evaluate biodegradable dental barrier membranes. Quantitative analysis of molecular weight reduction and tensile strength degradation after long-term exposure confirmed the validity of the evaluation method, as the in vitro degradation trends closely mirrored those observed in rabbit subcutaneous implantation experiments. In Chapter 4, a cranial defect physiological simulation condition was developed using pig blood and simulated body fluid to evaluate the performance of biodegradable cranioplasty plates. Over the course of 24 weeks, both mass and mechanical strength significantly decreased, and the degradation pattern was consistent in direction with the in vivo results. Furthermore, by applying correction coefficients to the in vitro data, a linear regression model was proposed to provide foundational data for estimating practical device lifespan. In Chapter 5, microneedles fabricated via micro molding and droplet-born air blowing methods were compared in terms of mechanical strength, dimensional accuracy, and skin penetration performance. Micro molded microneedles demonstrated superior structural integrity and mechanical properties, while DAB-fabricated microneedles excelled in manufacturing speed and skin penetration efficiency. This study clearly elucidated the functional differences arising from manufacturing techniques, offering useful guidance for device design in clinical applications. Overall, the simulated evaluation methods showed strong correlation with in vivo degradation behavior and serve as practical alternatives to animal testing. These methods support early design, regulatory evaluation, and predictive modeling of biodegradable devices, with potential expansion to various materials and implantation sites.
