The OTOF gene encodes otoferlin, essential for neurotransmitter release and vesicle recycling at cochlear inner hair cell (IHC) synapses. Variants in OTOF leads to prelingual auditory neuropathy spectrum disorder (ANSD). Gene therapy has improved hearing thresholds in mouse models, yet otoferlin remained suboptimal and synapse counts not significantly improved, suggesting persistent hidden hearing loss (HHL). This study explores whether single heterozygous mice (Otof+/p.R1934Q) carrying the DFNB9 founder allele in East Asians (p.R1939Q of OTOF) exhibit hidden audiological deficits. Using CRISPR/Cas9, we generated a mouse model with the p.R1934Q at the Otof locus. While auditory brainstem response thresholds were normal, Otof+/p.R1934Q mice showed reduced wave I and II amplitudes, indicating potential synaptic transmission defects. Histological analysis revealed fewer paired IHC ribbon synapses in Otof+/p.R1934Q mice compared to the wild-type controls, particularly in the apical and middle cochlear turns, without loss of IHCs or outer hair cells. This suggests that synaptic disruptions rather than hair cells loss contribute to the observed auditory deficits. Furthermore, it was followed by spiral ganglion neuron (SGN) density reduction in Otof+/p.R1934Q mice with earlier SGN loss in the basal turn of Otof p.R1934Q/p.R1934Q mice. Otof mRNA levels were similar across genotypes, but otoferlin protein levels were reduced in Otof+/p.R1934Q mice and notably lower Otofp.R1934Q/p.R1934Q mice, suggesting that reduced otoferlin level affects the observed audiological phenotype. Human pilot studies of normal hearing OTOF variant carriers revealed difficulties in frequency discrimination and gap detection, consistent with the mouse findings. This study demonstrates that single heterozygous carriers of the Otof mutant allele exhibit HHL due to haploinsufficiency. This implies that parents or siblings of DFNB9 patients may also have undiagnosed HHL. Additionally, successful OTOF gene therapy may not fully address these subclinical audiological deficits, highlighting the need for further research on the auditory phenotypes of single heterozygous carriers of OTOF in the general population and DFNB9 families.
