    Purpose
    This study aimed to evaluate both finite element analysis (FEA) and in vitro resonance frequency analysis (ISQ) to evaluate the effects of thread depth and pitch on the biomechanical performance and primary stability of dental implants in bones of varying quality.

    Materials and Method
    Twelve implant models with varying thread depths (0.4–1.0 mm) and pitches (0.8–1.2 mm) were designed using computer-aided design software and analyzed via finite element analysis under vertical (200 N) and oblique (100 N, 30°) loading in two bone models: D3 bone (representing ordinary bone quality) and simulated osteoporotic bone. Implants and bone structures were assigned isotropic, linearly elastic properties. Fully bonded contact conditions were assumed at all interfaces. Implant stability was additionally evaluated in vitro using polyurethane foam blocks (pounds per cubic foot (PCF) 10 and PCF 20) to simulate osteoporotic and D3 bone, respectively. Four implant types were tested: BLX® (Straumann Co., Basel, Switzerland), BLT® (Straumann Co., Basel, Switzerland), and two BlueDiamond® (MegaGen Co., Daegu, Korea) implants featuring regular or deep thread designs. A total of 80 implants were placed following standardized drilling protocols. Resonance frequency analysis (RFA) was performed using the MEGA ISQ® device (MegaGen Co., Daegu, Korea). Mean implant stability quotient (ISQ) values were compared between groups using t-test with a significance level of α = 0.05.

    Results
    Finite element analysis revealed that stress and implant displacement were higher in osteoporotic bone than in D3 bone under all loading conditions. Oblique loading consistently increased crestal cortical stress and implant micromotion, with the maximum stress concentration occurring around the first thread. Increasing thread depth reduced trabecular stress under vertical loading but increased cortical stress and implant displacement under oblique loading, particularly in deeper-thread models. Thread pitch had a less consistent and minimal effects. In the artificial bone model test, BLX® implants exhibited significantly higher ISQ values than BLT® implants in osteoporotic and D3 bone models (p < 0.01). BlueDiamond® implants with deep threads showed significantly greater stability than regular-thread implants in both bone conditions (p < 0.001).

    Conclusion
    Thread depth was identified as a significant factor influencing implant biomechanics and primary stability in low-density bone. Deeper threads enhanced stress distribution and reduced displacement in FEA simulations, while in vitro testing yielded higher ISQ values, particularly in osteoporotic bone. These findings support optimizing thread depth as a key strategy for improving implant stability in compromised bone conditions.
