    족세포의 생물학적 기능과 그에 따른 신장 섬유화 기전은 단백뇨와 미토콘드리아 기능장애를 해결하기 위한 유망한 표적 중 하나로 주목받고 있다. 이러한 병리적 변화는 세포 내 칼슘 농도와 산화 스트레스에 의해 크게 영향을 받으며, 이는 급성 신손상이 만성 신질환으로 진행되는 주요 지표인 족세포 손상과 단백뇨로 나타난다. 현재까지 미토콘드리아 기능 개선을 목표로 한 다양한 전략이 제시되어 왔으나, 임상적으로 효과적인 치료제는 여전히 부족한 실정이다. 본 저자는 탄소 기반 나노 물질인 그래핀 양자점 (Graphene Quantum Dots, GQDs)을 활용하여 신장 섬유화를 완화하고, 그 기전을 규명하고자 하였다.
    족세포 손상 및 사구체 경화증 (glomerulosclerosis)을 반영하는 두 가지 in vivo 모델, 즉 Adriamycin 유도 신병증 (Adriamycin-induced nephropathy, ADN)과 5/6 부분 신절제 (5/6 subtotal nephrectomy, 5/6Nx) 모델을 이용하여, GQDs의 항염증, 항섬유화, 항세포 사멸 효과를 평가하였으며, 특히 족세포의 액틴 사이토스켈레톤 (actin cytoskeleton) 복원 및 미토콘드리아 막전위 회복에 초점을 맞추었다. ADN 모델에서는 BALB/c 마우스에 Adriamycin (11.5 mg/kg, 정맥주사)와 GQDs (20 mg/kg, 복강 주사)를 투여하여 신독성을 유도하였고, 투여 7일 후 신장을 적출하여 분석을 진행하였다. GQDs의 치료 효과를 평가하기 위해 면역염색, western blotting, fluorescence-activated cell sorting (FACS), quantitative PCR을 실시하였다. 특히, FACS 분석을 통해 ADN 동물 모델에서 T 세포 (CD3+, CD25+, CD44+)와 myeloid 계통 세포 (CD11b+, GR1high, inter, high-inter, CD206+)의 변화를 비교한 결과, GQDs 처치군에서 myeloid (high; p-value 0.0008; inter; p-value 0.0023; inter-high; p-value 0.0046) 계통 세포가 T 세포 (p-value 0.0135)보다 통계적으로 유의하게 더 많이 감소하는 것이 관찰되었다. 다음으로 mRNA sequencing을 통해 차등 발현 유전자 (differentially expressed genes, DEGs)를 확인하였다. 또한, GQDs가 칼슘 신호전달 및 관련 이온 채널에 미치는 영향을 분석하기 위해 유전자 온톨로지 (Gene Ontology, GO) 주석 및 경로 분석 (pathway enrichment analysis)을 수행하였다.

    5/6Nx 모델에서는 인간 만성 신장질환 (CKD)를 모사한 쥐 모델을 구축하고, 수술 전후 다양한 시점에서 GQDs (4 mg/kg)를 투여하여 8주간 치료를 진행하였다. GQDs 처리 결과 혈압 감소 (p-value 0.0046), 크레아티닌 청소율 (creatinine clearance) 개선 (p-value 0.009), 그리고 신장 조직 내 섬유화 감소가 관찰되었다. in vitro 실험에서는 산화 스트레스 및 기계적 압력이 가해지는 조건에서 GQDs가 세포 내 칼슘 농도를 낮춤으로써 항섬유화 및 항산화, 항노화 효과를 나타냄을 확인하였다. 특히, 기존 임상 약물인 Amlodipine과 Vitamin C와 비교했을 때, GQDs는 유사한 치료 효과를 보였으며 TRPC5 (p-value < 0.0001)를 선택적으로 억제하는 것으로 나타났다. 또한, 전자현미경 분석을 통해 GQDs가 미토콘드리아 크리스테 구조를 개선하는 효과도 확인되었다.

    Podocyte biology and its role in kidney fibrosis represent a promising target for addressing proteinuria and mitochondrial dysfunction, both of which are influenced by intracellular calcium levels and oxidative stress. In this study, the author employed carbon-based nanomaterials, specifically graphene quantum dots (GQDs), to attenuate renal fibrosis and investigate the underlying mechanisms. The study included two in vivo models of podocyte injury and glomerulosclerosis, adriamycin-induced nephropathy (ADN) and 5/6 subtotal nephrectomy (5/6Nx), to evaluate anti-inflammatory, anti-fibrotic, and anti-apoptotic effect of GQDs, with a particular focus on the restoration of podocyte actin cytoskeletal integrity.
    In the Adriamycin-induced nephropathy (ADN) model, the authors administered Adriamycin (11.5 mg/kg, intravenous) and graphene quantum dots (GQDs, 20mg/kg, intraperitoneal) to BALB/c mice to induce nephrotoxicity. Kidneys were harvested 7 days’ post-injection for analysis. To evaluate the therapeutic effect of GQDs, immunohistochemistry, western blotting, FACS analysis, and real-time PCR (qPCR) were performed. To investigate GQDs’potential role in calcium signaling and related calcium channels, gene ontology (GO) annotation and pathway enrichment analyses were performed. A total of 677 genes were upregulated and 1,164 genes were downregulated (p-value < 0.05) following GQDs treatment. Enriched GO terms included suppression of calcium ion transport (GO:0006816) and regulation of the apoptotic signaling pathway (GO:2001233), while processes such as mitochondrial translation (GO:0032543) and ATP metabolic process (GO:006979) were significantly overrepresented. In particular, the calcium ion receptor gene Trpv5, identified through GO term analysis, has been reported to influence the activation of TRPC5 (transient receptor potential channel 5), which is associated with podocyte effacement. Immunostaining of kidney sections for TRPC5 (p-value < 0.0001) confirmed that GQDs downregulate TRPC5 expression, a change that is closely associated with reduced kidney fibrosis and improved mitochondrial function.
    In the 5/6 nephrectomy (5/6Nx) rat model, the author established a model mimicking human CKD and administered GQDs (4 mg/kg, intraperitoneal) at various time points before and after surgery for up to 8 weeks.
