    COVID-19, which first emerged in December 2019, has significantly impacted national healthcare systems due to the rapid spread of infection, increased mortality rate, and higher rate of severe cases. This study aimed to analyze medication use in high risk patients during the COVID-19 pandemic, focusing on two specific substudies. The first substudy evaluated the impact of potentially inappropriate medication (PIM) use on the health outcomes of older COVID-19 patients, while the second substudy assessed inhaler medication adherence among COPD (Chronic Obstructive Pulmonary Disease) patients during the COVID-19 lockdown.
    Substudy 1 was a retrospective cohort study based on the NHIS-KDCPA (National Health Insurance Service-Korea Disease Control and Prevention Agency) linked COVID-19 database. We evaluated the risk of mortality and critical care service utilization among patients aged 60 years or older who were diagnosed with COVID-19 between January 20 and June 4, 2020. The COVID-19 patients were classified into PIM-exposed and non-exposed based on the Beers criteria within the 30-day period prior to the date of diagnosis, and were followed up until July 31, 2020. To ensure comparability between the PIM-exposed and non-exposed groups in terms of the distribution of sociodemographic variables such as age and sex, propensity score matching (PSM) was conducted. The matched cohorts were then analyzed using logistic regression to evaluate the association between PIM exposure and the risks of mortality and critical care service utilization. This analysis was performed using three models: (1) logistic regression before PSM, (2) logistic regression after PSM, and (3) logistic regression after PSM with adjustment for COVID-19 related medications and hospitalization variables. To assess the robustness of the findings, sensitivity analysis was conducted based on different PIM exposure windows and PSM algorithm methods. In addition, subgroup analysis was performed by age, comorbidities, and the number of PIM API (Active Pharmaceutical Ingredient).
    Substudy 2 was a retrospective cohort study based on claims data from NHIS. It included patients who had been prescribed COPD inhalers [ICS_LABA (inhaled corticosteroids with long-acting β₂-agonists) or LABA_LAMA (inhaled long-acting β₂-agonists with long-acting muscarinic antagonists)] for at least 180 days in 2018, as recommended for category D patients according to the GOLD (Global Initiative for Chronic Obstructive Lung Disease) guidelines. We compared medication adherence before (in 2019) and during the COVID-19 lockdown (in 2020) among the target COPD patients using the proportion of days covered (PDC). Furthermore, a logistic regression analysis was conducted to identify factors associated with non-adherence (PDC < 80%) during the COVID-19 lockdown period, considering sociodemographic, health condition, and pharmacotherapy characteristic variables.
    In substudy 1, a total of 2,217 patients were included, of whom 604 were classified as PIM-exposed group. After PSM, 583 pairs of PIM-exposed and non-exposed were analyzed. The PIM-exposed group had significantly higher mortality (19.7% vs. 9.8%, P < 0.001) and critical care service utilization rate (13.4% vs. 8.9%, P = 0.016) compared to the non-exposed. Logistic regression analysis showed that PIM use significantly increased the risk of mortality (RR = 1.68, 95% CI = 1.23 ~ 2.24) and critical care service utilization (RR = 1.75, 95% CI = 1.26 ~ 2.39). Furthermore, the results of the sensitivity and subgroup analysis showed that the risk of mortality and critical care service utilization increased with the number of PIM API and a shorter interval between PIM exposure and COVID-19 diagnosis.
    In substudy 2, a total of 15,971 COPD patients were included, 79.2% of whom were male. During the COVID-19 lockdown, the medication adherence of COPD inhaler significantly decreased compared to the previous year (49.8% in 2020 vs. 56.3% in 2019, P < 0.001). In addition, the proportion of adherent patients (PDC ≥80%) declined from 22.0% to 18.0% during the lockdown. Factors associated with medication adherence during the lockdown included being a Medical Aid beneficiary, residing in a rural area, and receiving care at a clinic.
    In conclusion, this study showed that PIM use was associated with an increased risk of mortality and critical care service utilization among older COVID-19 patients, indicating that PIM use had a negative impact on health outcomes in these patients. Additionally, medication adherence to COPD inhalers significantly declined during the COVID-19 lockdown, highlighting the need for targeted intervention strategies to maintain adherence during the COVID-19 pandemic.
    Declining medication adherence and inappropriate medication use can negatively impact a patient's health status. Therefore, during infectious disease pandemic such as the COVID-19, healthcare providers must take proactive measures to protect high risk patients, including older adults and patients with underlying diseases such as COPD. Specifically, they should carefully evaluate the potential for adverse drug reactions (ADRs) and drug-drug interactions when prescribing medications, and ensure rational prescribing that balances therapeutic benefits with potential risks in these vulnerable group. Furthermore, they should actively engage with high risk patients to enhance medication adherence, thereby supporting the achievement of intended therapeutic goals.

    COVID-19(코로나-19, coronavirus disease of 2019)는 2019년 12월, 중국 우한에서 최초 발생된 이후, 감염병 확산 및 감염병으로 인한 사망률 및 중증화율의 증가로 인해 전세계적으로 보건의료체계에 큰 영향을 미쳤다.본 연구는 COVID-19 시기의 고위험군 그룹의 의약품 적정 사용 현황을 분석하기 위해 세부연구1로서 고위험군인 COVID-19 감염 노인 환자에 있어 PIM(잠재적 부적절 약물, Potentially Inappropriate Medication) 복용이 사망 및 중증 치료 서비스 이용 등 건강 예후에 미치는 영향을 평가하였고, 세부연구2로서 COVID-19 lockdown 시기의 고위험군인 COPD(만성폐쇄성폐질환, Chronic Obstructive Pulmonary Disease) 환자의 흡입기 치료제의 복약순응도를 평가하였다.
