    1. 목 적
    치과 시술은 통증뿐만 아니라 기구 소리나 환경에 대한 불안으로 인해 환자에게 정신적 스트레스를 유발할 수 있으며, 이는 적절한 치료를 방해하는 요소로 작용할 수 있다. 이를 해소하기 위한 진정요법 중 자가진정조절법(Patient-Controlled Sedation, PCS)은 환자가 스스로 진정제의 투여 시점을 조절함으로써 과도한 진정을 방지하고 환자 만족도를 높일 수 있는 방법으로 주목받고 있다. Midazolam (MDZ), Remimazolam (RMZ), Dexmedetomidine (DMDT)은 치과 진정에 사용되는 대표적인 진정제로, 각각 상이한 약리학적 특성과 임상적 장단점을 가진다. 본 연구의 목적은 이 세 가지 진정제를 PCS 환경에서 비교함으로써 각 약물의 진정 효율, 안전성, 회복 특성 및 환자와 시술자의 만족도에 미치는 영향을 종합적으로 분석하고, 치과 시술에 적합한 진정 전략을 제시하는 데 있다.

    2. 방 법

    Background: Patient-controlled sedation (PCS) is an emerging approach designed to improve patient comfort and give patients greater control during dental procedures. Although midazolam (MDZ) and dexmedetomidine (DMDT) have been studied for this purpose, remimazolam (RMZ) remains relatively under-investigated. This study aimed to evaluate and compare the efficacy, recovery characteristics, safety, and satisfaction associated with MDZ, RMZ, and DMDT when used for PCS in dental surgery.

    Methods: Eighty-nine adult patients scheduled for dental procedures were randomly assigned to receive PCS with MDZ (N = 30), RMZ (N = 30), or DMDT (N = 29). Each sedative was administered through bolus-only PCS without a loading dose or continuous infusion (0.01 mg/kg for MDZ and RMZ; 0.1 mcg/kg for DMDT) with a 1-minute lockout interval. The primary outcomes were patient and surgeon satisfaction; secondary measures included sedation depth (assessed by the Patient State Index [PSI] and the Modified Observer’s Assessment of Alertness/Sedation [OAAS]), recovery profile, frequency of PCS button presses, need for supplemental sedatives, and intraoperative and postoperative vital signs. Supplemental MDZ (0.02 mg/kg) was administered as necessary. Oxygen was supplied at 2 L/min through nasal prongs throughout the procedure.

    Results: All three groups attained adequate sedation, with no significant difference in mean intraoperative PSI scores (MDZ, 77.5 ± 7.4; RMZ, 81.6 ± 5.6; DMDT, 77.8 ± 8.2; P = 0.061). Although total PCS button presses did not differ significantly among the groups (P = 0.115), RMZ was associated with significantly more effective doses delivered (P = 0.008), especially during certain intraoperative periods. DMDT produced the most stable sedation with minimal need for intervention but was associated with cardiovascular suppression and slower recovery. MDZ offered robust amnestic effects but showed reduced intraoperative responsiveness and increased postoperative pain management needs. RMZ allowed the quickest recovery and greatest patient cooperation but required more frequent bolus attempts and supplemental dosing, likely due to its pharmacokinetic characteristics causing variable plasma concentrations and unstable sedation depth. This variability can hinder stable, patient-regulated PCS.

    Conclusions: Surgeon and patient satisfaction remained high across all groups, with no significant differences between them. Although all three sedatives achieved effective sedation, they displayed different pharmacologic and clinical profiles. These results highlight the importance of agent-specific PCS protocols and support PCS as a viable sedation method when adjusted for each drug’s unique properties.
