    Digital health literacy, defined as the ability to search for, understand, evaluate, and apply health information in digital environments, plays a critical role in the health management and decision-making of children and adolescents. In particular, multicultural children and adolescents are more likely to experience difficulties in effectively using digital health information due to socio-structural constraints. This study aimed to develop a digital health literacy improvement program tailored for multicultural children and adolescents and to evaluate its effectiveness.
    The program was developed based on the Transactional Model of eHealth Literacy (TMeHL) and applied the five stages of the ADDIE (Analysis, Design, Development, Implementation, Evaluation) instructional design model. The intervention was designed to strengthen the functional, communicative, critical, and translational components of digital health literacy through structured educational and practical activities. A non-equivalent control group pretest-posttest design was employed to evaluate the program’s effectiveness. The primary outcome variables were digital health literacy, information overload, and perceived usefulness of internet health information.
    A total of 31 multicultural children and adolescents aged 10 to 15 years participated in the study (14 in the experimental group and 17 in the control group). The experimental group engaged in a five-session program, with one 60-minute session per week. Each session was delivered in a small-group format using digital platforms such as Zoom and Google Classroom. Data were collected at two time points—pre-intervention and post-intervention—between January and March 2025. The data were analyzed using descriptive statistics, independent t-test, Mann-Whitney U test, chi-square test, and Fisher’s exact test.
    The results showed significant differences in the pre-post change scores between the experimental and control groups for digital health literacy (t = 2.73, p = .011, d = 0.99) and perceived usefulness of internet health information (t = 3.11, p = .004, d = 1.12). However, there was no significant difference between the groups regarding changes in information overload (t = -0.17, p = .854, d = -0.06).
    This study is significant in that it developed and validated a tailored intervention reflecting the developmental characteristics and needs of multicultural children and adolescents. The program can be applied in various practical settings, including schools, public health centers, and community childcare centers, and may serve as foundational data for the systematization and institutionalization of digital health literacy education. Furthermore, this study may contribute to improving health equity among vulnerable populations by presenting a concrete strategy to reduce the digital health information gap.
