    당뇨병 콩팥병은 말기신부전과 투석의 주요 원인으로, 기존 치료만으로는 질환의 진행을 완전히 억제하기 어렵다. 본 연구에서는 콩팥 거주 대식세포가 당뇨병 콩팥병의 면역 병태생리에 중요한 역할을 한다고 가정하고, 공공데이터와 연구진이 보유한 단일세포 전사체 데이터를 통합 분석하였다. 이를 통해 당뇨병 콩팥병 환자에서 콩팥 거주 대식세포의 산화적 인산화(OXPHOS) 관련 유전자 발현이 유의하게 증가함을 확인하였다. 기존에 골수 유래 대식세포에서 산화적 인산화는 주로 M2-like 특징과 연관된다고 알려졌으나, 조직 거주 대식세포는 대사적 가소성이 커 실제 콩팥 환경에서는 다른 기능적 의미를 가질 가능성이 있음을 시사한다. 또한 리간드-수용체 상호작용 분석 결과, 콩팥 거주 대식세포가 인접 콩팥 세포의 기능과 면역 반응 조절에 관여함을 확인하였다.
    이러한 결과를 바탕으로, i) 인체 콩팥 조직 코호트에서 산화적 인산화 유전자 발현과 예후 간의 연관성을 입증하였고, ii) 마우스 제2형 당뇨병 콩팥병 모델에서 조직학적·생화학적 검사와 단일세포 전사체 분석, 산화적 인산화 억제제 투여 등의 in vivo 실험을 수행하였다. 그 결과, 콩팥 거주 대식세포의 산화적 인산화를 억제하면 당뇨병 콩팥병의 질병 활성도가 유의하게 감소함을 확인하였다. 즉, 본 연구는 당뇨병 콩팥병의 복잡한 병태생리를 면역학적 관점에서 새롭게 이해하고, 산화적 인산화를 표적으로 삼는 치료 전략이 당뇨병 콩팥병의 효과적인 치료 방법이 될 수 있음을 보여주었다.

    Diabetic kidney disease (DKD) remains a leading cause of end-stage kidney disease worldwide, posing a significant burden on patients and healthcare systems. Despite progress in controlling hyperglycemia and hypertension, many individuals with diabetes still experience progressive deterioration of kidney function. Accumulating evidence suggests that DKD pathogenesis involves not only metabolic and hemodynamic factors, but also immunological and inflammatory processes. In particular, kidney resident macrophages (KRMs) have recently been highlighted as key mediators of early inflammation and tissue remodeling in the diabetic milieu.
    In this study, we examined the role of KRMs in DKD using human single-cell transcriptomic analyses combined with in vivo mouse experiments. First, integrated single-cell RNA-sequencing datasets of DKD kidneys showed a marked increase in oxidative phosphorylation gene expression within KRM populations. Subsequent immunohistochemical analyses in human diabetic kidney specimens confirmed that KRMs overexpressed mitochondrial enzymes such as COX5A, correlating with worse kidney outcomes. In a murine model of type 2 diabetes, KRM-specific oxidative phosphorylation was similarly elevated, driving inflammatory signals and fibrosis. Pharmacological inhibition of oxidative phosphorylation using metformin or a direct OXPHOS inhibitor significantly reduced albuminuria, histologic damage, and fibrotic pathways, suggesting that modifying KRM metabolism can slow disease progression.
    Taken together, these findings provide a novel immunometabolic perspective on DKD pathogenesis, wherein mitochondrial metabolism in KRMs perpetuates renal inflammation and fibrosis. Targeting macrophage bioenergetics may represent a promising therapeutic strategy to complement existing DKD management. Further research on selective KRM metabolic modulation could offer new avenues for improving outcomes in diabetic patients.
