The rise in aging populations, chronic diseases, and healthcare expenditures has intensified the critical need to balance efficient allocation of healthcare resources with equitable access to innovative treatments. These burdens are particularly pronounced in chronic conditions requiring long-term management, where the expanded use of biologics places a significant strain on healthcare budgets. Biosimilars, which offer comparable quality and efficacy at lower costs, have emerged as a promising solution. However, in South Korea, the market penetration of biosimilars and their anticipated cost-saving benefits remain limited due to structural impediments such as supplier-centric price-linking policies and insufficient prescribing incentives. Furthermore, the absence of a genuinely patient-centered approach results in an environment where patients cannot choose treatments based on their personal circumstances.
To address this gap, the first quantitative analysis in South Korea exploring factors influencing treatment decisions between biosimilars and original biologics was conducted. This study employed a discrete choice experiment methodology with a sample of 162 patients with inflammatory bowel disease. The findings reveal that while patients predominantly prioritize treatment efficacy and the risk of adverse effects, their specific preferences diverge based on the treatment type. When choosing biosimilars, patients place a higher value on practical considerations such as cost and convenience of administration. Conversely, selections of original biologics are primarily guided by healthcare professionals' recommendations and a established clinical trust. Notable, the influence of a doctor's recommendation was a significant factor for original biologics but not for biosimilars, indicating a potential shift towards more autonomous patient decision-making in this area.
These findings indicate that simple pricing strategies or expansion of supply alone are insufficient to stimulate the biosimilar market. This research comprehensively addresses the structural, psychological, and socioeconomic factors impeding biosimilar adoption, highlighting the need for multifaceted and integrated policies that address both supply- and demand-side considerations. Specifically, strategies such as tailored patient information provision, education and awareness-building for both healthcare professionals and patients, targeted support for socioeconomically vulnerable populations, and facilitation of shared decision-making are required to cultivate an environment where end-users can make informed and confident choices about cost-effective alternatives.
