    정신응급상황이란 환자의 사고, 행동, 감정 및 사회적 관계가 급격히 악화되어 즉각적인 개입이 필요한 상태를 의미한다. 만성질환 유병률이 높은 정신질환자의 비정신과 병동 입원이 증가함에 따라 비정신과 간호사가 정신응급상황에 효과적으로 대처할 수 있는 교육의 필요성이 강조된다. 이에 본 연구의 목적은 비정신과 간호사의 정신응급상황 대처 능력을 향상시키기 위해 가상현실 기반 정신응급상황 시뮬레이션 교육을 개발하고, 이를 적용하여 교육 효과를 평가하는 것이다.
    가상현실 기반 정신응급상황 시뮬레이션 교육은 NLN/Jeffries Simulation Theory를 이론적 토대로 하고, ADDIE 모델의 5단계를 적용하여 개발되었다. 시나리오는 전문가 검토를 통해 내용 타당도를 확보하였으며, 파일럿 테스트를 거쳐 수정·보완한 뒤 최종 시뮬레이션 교육이 완성되었다.
    본 연구에서는 개발된 시뮬레이션 교육의 효과를 평가하기 위해 무작위 대조군 실험설계를 적용하였다. 2025년 1월 22일부터 3월 2일까지 종합병원급 이상의 의료기관에서 근무하는 총 임상 경력 6개월 이상의 비정신과 간호사 38명이 참여하였으며, 중재군과 대조군에 각각 19명씩 무작위 배정되었다. 중재군은 가상현실 기반 정신응급상황 시뮬레이션 교육을 1회 실시하였으며, 대조군은 연구 종료 후 동일한 교육을 제공받았다. 교육 효과는 정신응급상황 지식, 정신간호 역량, 위기 자기효능감에 대한 자가보고형 설문을 통해 평가되었다. 교육 참여 전에는 사전조사를, 교육 종료 직후에는 사후조사를, 종료 4주 후에는 추후조사를 진행하였으며, 대조군도 동일한 시기에 동일한 간격으로 사전·사후·추후조사를 시행하였다. 시뮬레이션 교육의 효과는 반복측정 분산분석과 일반화 추정 방정식을 활용하여 분석되었다. 그 결과, 시간에 따른 정신응급상황 지식(χ2=68.11, p<.001), 정신간호 역량(F=33.83, p<.001), 위기 자기효능감 (χ2=62.13, p<.001)의 변화량에서 중재군과 대조군 간 유의한 차이가 확인되었다.
    본 연구에서 개발한 가상현실 기반 정신응급상황 시뮬레이션 교육은 비정신과 간호사의 정신응급상황 대처 능력 강화를 위한 교육 자료로서 유용한 가능성을 보여주었다. 또한, 본 연구는 해당 교육의 임상 및 교육 현장 적용 가능성을 탐색한 초기 연구로서 의의를 지닌다. 다만, 일부 지역의 의료기관에 근무하는 소규모 표본을 대상으로 수행된 연구이므로 교육 효과의 일반화를 위해서는 다양한 기관과 충분한 표본을 포함한 후속 연구를 통해 반복 검증이 필요하다.

    A psychiatric emergency refers to a condition in which a patient’s thoughts, behaviors, emotions, and social relationships rapidly deteriorate, necessitating immediate intervention. With the increasing admission of psychiatric patients to non-psychiatric wards due to a high prevalence of chronic physical illnesses, there is a growing need to educate non-psychiatric nurses to respond effectively to psychiatric emergencies. Therefore, this study aimed to develop and evaluate the efficacy of a virtual reality (VR)-based simulation psychiatric emergency education for non-psychiatric nurses.
    The VR-based psychiatric emergency simulation education was developed based on the NLN/Jeffries Simulation Theory and implemented following the five phases of the ADDIE model. The scenario’s content validity was reviewed by experts, and the simulation education was finalized after pilot testing and subsequent revisions.
    To evaluate the efficacy of the developed simulation education, a randomized controlled trial was conducted. A total of 38 non-psychiatric nurses with at least six months of clinical experience at general hospitals or higher-level institutions participated in the study between January 22 and March 2, 2025. Participants were randomly assigned to either the intervention group (n = 19) or the control group (n = 19). The intervention group received the VR-based simulation education once, and the control group was offered the same education after the study was completed.
    Educational outcomes were assessed using self-reported questionnaires measuring psychiatric emergency knowledge, psychiatric nursing competency, and crisis self-efficacy. Pre-tests were conducted before the intervention, post-tests immediately after, and follow-up tests four weeks later. The control group followed the same assessment schedule. The efficacy of the simulation education was analyzed using repeated measures ANOVA and generalized estimating equations. Significant group-by-time interaction effects were found for psychiatric emergency knowledge (χ2=68.11, p<.001), psychiatric nursing competency (F=33.83, p<.001), and crisis self-efficacy (χ2=62.13, p<.001).
    The VR-based simulation education developed in this study demonstrated promising potential as an educational resource for enhancing non-psychiatric nurses’ competencies in responding to psychiatric emergencies. Moreover, this study holds significance as an initial investigation exploring the feasibility of implementing such education in clinical and educational settings. However, as the study was conducted with a small sample from medical institutions in limited regions, further research involving more diverse institutions and larger samples is needed to validate and generalize its efficacy.
