본 연구는 비정신과 아동간호사의 정신질환자에 대한 태도와 정신간호역량을 파악하고 비정신과 아동간호사의 정신간호에 대한 교육 및 연구의 기초를 마련하기 위해 수행되었다. 어린이 공공 전문진료센터로 지정된 서울 소재 상급종합병원 4곳에서 근무하는 정신과 근무 경험이 없는 비정신과 아동간호사 182명을 대상으로 2024년 10월부터 12월까지 수집된 자료를 SPSS 29.0을 이용하여 빈도분석, 기술통계, Pearson’s correlation으로 분석하였다.
연구대상자의 92.9%는 여성이었으며 평균 연령은 32.04세, 평균 근무 경력은 8년 7개월이었다. 1년 이내에 정신질환을 동반한 환자를 간호한 경험이 있는 간호사는 141명(77.5%)이었으며, 이 중 125명(71.6%)은 정신질환을 동반한 환자를 간호하는 것에 어려움을 호소하였다. 특히 정신질환자와의 의사소통과 폭력적 행동에 대한 대처에 어려움을 보고하였다. 174명(95.6%)이 정신간호와 관련된 교육이 필요하다고 하였으나 실제 대학 졸업 이후 정신간호 관련 교육의 경험률은 12.6%였으며, 정신과적 응급상황 시 대처와 치료적 의사소통의 교육 요구도가 높았다.
정신질환자에 대한 태도의 평균 점수는 2.96(±0.35)점으로 긍정적인 태도를 가지고 있는 것으로 나타났다. 정신간호역량의 평균 점수는 3.33(±0.45)점으로 보통 수준의 역량을 가지고 있는 것으로 나타났으며, 특히 치료적 의사소통 역량이 3.09(±0.63)점 으로 가장 낮은 것으로 나타났다.
대상자의 특성에 따른 정신질환자에 대한 태도를 분석한 결과, 연령(p=.011)과 결혼상태(p=.01)에 따라 유의미한 차이가 있었고, 20대가 30대보다 정신질환자에 대한 태도가 긍정적인 것으로 나타 났다. 정신간호역량은 연령(p<.001), 학력(p=.048), 경력(p=.032), 임상 및 실습 이외에서의 정신질환자 접촉 경험(p=.003), 정신병동 근무 의향(p<.001)에 따라 유의미한 차이를 보였으며 20대가 다른 연령대에 비해 정신간호역량이 높았다. 정신질환자에 대한 태도와 정신간호역량은 유의한 정적 상관관계(r=.345, p<.001)를 보였다.
이러한 결과는 정신질환자에 대한 태도의 개선과 정신간호 역량의 강화를 동시에 고려한 간호교육의 필요성을 시사한다. 특히, 치료적 의사소통과 정신과적 응급상황 시 대처 등 실무에 밀접한 교육 내용이 우선으로 제공되어야 하며, 표준화 환자를 적용한 시뮬레이션, 가상현실 활용 등 효과적인 교육 방법의 도입이 필요하다.
본 연구는 비정신과 아동간호사를 대상으로 정신질환자에 대한 태도와 정신간호역량을 파악하고 아동간호의 특성을 고려하여 분석한 연구로, 이는 소아청소년의 정신질환 증가 추세에 대응한 정신간호 교육 및 정책 수립의 기초자료로 활용될 수 있다.

This study was conducted to identify both the attitudes to mental illness and psychiatric nursing competencies of pediatric nurses without psychiatric ward experience, and also to lay the foundation for education and research on psychiatric nursing for non-psychiatric pediatric nurses. Data were collected from October to December 2024 from 182 pediatric nurses without psychiatric ward experience, employed at four tertiary hospitals in Seoul designated as specialized public medical centers for children. The collected data were analyzed by frequency analysis, descriptive statistics, and Pearson's correlation using SPSS 29.0.
Among the study participants, 92.9% were female, with an average age of 32.04 years and an average work experience of 8 years and 7 months. Among them, 141 nurses (77.5%) had experience caring for patients with mental illness within the past year, and within them, 125 nurses (71.6%) reported that caring for patients with mental illness was challenging. They reported difficulties in psychiatric nursing, particularly in communication with patients with mental illness and dealing with violence. 174 participants (95.6%) stated that psychiatric nursing education was necessary, yet the actual participation rate in psychiatric nursing-related education after university graduation was 12.6%. There was high demand for education on how to handle psychiatric emergencies and therapeutic communication.
The average attitude score to mental illness was 2.96 (±0.35), indicating a positive attitude. The average score for psychiatric nursing competency was 3.33 (±0.45), indicating moderate competence levels, with therapeutic communication skills showing the lowest competence level at 3.09 (±0.63).
Analysis of attitudes to mental illness based on participant characteristics revealed significant differences based on age (p=.011) and marital status (p=.01), with those in their 20s demonstrating more positive attitudes to mental illness than those in their 30s. Psychiatric nursing competency was significantly influenced by age (p<.001), education level (p=.048), general work experience (p=.032), experience interacting with mental health patients outside of clinical practice (p=.003), and willingness to work in a psychiatric ward (p<.001). Nurses in their 20s had higher psychiatric nursing competencies than other age groups. Attitudes toward patients with mental illness and psychiatric nursing competencies showed a significant positive correlation (r=.345, p<.001).
These findings suggest the need for nursing education that considers both attitude to mental illness improvement and psychiatric nursing competency enhancement. In particular, education content closely related to practical work, such as therapeutic communication and coping with psychiatric emergencies, should be provided as a priority, and effective educational methods, such as simulations using standardized patients and the use of virtual reality, should be introduced.
This study assessed attitudes to patients with mental illness and psychiatric nursing competencies among pediatric nurses without psychiatric training, analyzing the results while considering the characteristics of pediatric nursing. The results may serve as foundational data for establishing psychiatric nursing education and policies in response to the increasing trend of mental disorders among children and adolescents.
