관계를 통한 정서조절인 상호조절은 타인과의 연결된 관계 맥락 속에서 정서를 탄력적으로 조절하는 과정이다. 이는 고전적인 상담이론에서부터 강조되어 온 내담자 변화의 핵심기제이다. 최근 상담학 연구에서도 정서조절을 관계적 수준에서 다루어야 한다는 인식이 확대되고 있다. 그럼에도 대부분의 연구는 내담자의 개인 내적 조절에 집중해 왔으며, 관계적 조절 측면은 간과됐다. 이에 본 연구는 상담사와의 상호작용 속에서 내담자가 경험하는 상호조절이 상담성과에 어떤 영향을 미치는지를 생리적 동기화를 통해 검토하고자 하였다.
생리적 동기화는 상호작용하는 두 사람의 생리적 반응이 유사한 패턴으로 동조되는 현상으로, 본 연구에서는 심박수 데이터를 활용하여 동기화의 강도와 패턴(동위상 vs. 역위상)을 산출하였다. 연구참여자는 정신역동이론을 기반으로 한 상담사 9명과, CESD-R에서 13점 이상의 우울 증상을 보인 내담자 30명으로 구성되었으며, 총 12회기의 대면 상담이 진행되었다. 모든 회기에서 상담사와 내담자의 심박수가 각각 실시간으로 측정하였고, 내담자는 각 회기 시작 전 상담 단기성과를 평가하는 ORS에 응답하였다. 장기성과는 상담 전후의 정서조절곤란(DERS), 전반적 기능수준(OQ-30), 우울 증상(CESD-R)을 통해 측정되었다. 총 355회기의 상담 데이터를 기반으로, 단기성과는 2수준 다층모형으로, 장기성과는 다중회귀분석을 통해 분석하였다.
연구 결과, 단기성과 측면에서 내담자의 초기 정서조절곤란 수준이 높을수록 특정 회기에서 상담사와의 생리적 동기화 강도가 다음 회기의 상담성과를 유의하게 예측하는 것으로 나타났다. 또한 역위상(anti-phase) 동기화 패턴은 동위상(in-phase) 동기화보다 다음 회기 성과에 긍정적인 영향을 미쳤다. 장기성과와 관련하여, 역위상 동기화 패턴은 정서조절곤란 변화량을 유의하게 예측하였다. 전반적 기능 수준(OQ-30)에서는 내담자의 초기 정서조절곤란 수준에 따라 동기화 강도가 OQ-30의 하위영역인 대인관계 영역 변화량에 미치는 영향이 달라, 정서조절곤란이 큰 내담자일수록 12회기 평균 동기화 강도가 높을 때 대인관계 영역에서의 상담 효과가 큰 것으로 나타났다. 아울러, 역위상 동기화 패턴은 동위상 패턴일 때보다 전반적 기능 수준의 총변화량뿐 아니라 하위영역인 주관적 불편감 변화량에도 유의미하게 더 큰 영향을 미쳤다. 한편, 동기화의 강도와 패턴 모두 우울 증상의 변화량에 유의한 영향을 미친다는 통계적 증거는 확인되지 않았다.
본 연구는 실제 상담 과정 전반에서 상담사–내담자 간 생리적 동기화가 내담자의 변화와 어떤 관련이 있는지를 체계적으로 분석하였다는 점에서 의의가 있다. 특히, 동기화의 강도뿐 아니라 패턴과 내담자의 초기 정서조절 특성을 함께 고려함으로써, 관계적 정서조절의 작동 메커니즘을 보다 구체적으로 설명할 수 있는 경험적 근거를 제공한다. 이는 상담 장면에서 상호조절을 촉진하기 위한 임상적 개입 가능성을 시사할 뿐만 아니라, 생리적 동기화라는 객관적 생물학적 지표를 활용하여 상담성과의 변화를 설명할 수 있는 경험적 근거를 제시하였다는 점에서 학술적 의의가 있다.

Interactive regulation is the resilient modulation of emotional states through interpersonal interactions within connected relational contexts. This process has long been emphasized as a core mechanism of client change in traditional counseling theories. Recently, there has been growing recognition in counseling research that emotion regulation should be addressed at the interpersonal level. However, most studies have continued to focus on clients’ intrapersonal regulation, overlooking the relational dimension. Therefore, the present study aimed to examine how interactive regulation, as experienced by clients through interactions with their therapists, influences counseling outcomes, using physiological synchrony as an indicator.
Physiological synchrony refers to the phenomenon in which two individuals' physiological responses become aligned in a similar pattern during interaction. In this study, heart rate data were used to compute both the magnitude and sign of synchrony (in-phase vs. anti-phase) between therapists and clients. Participants included 9 psychodynamic-oriented therapists and 30 clients who reported elevated depressive symptoms (CESD-R score ≥13). They engaged in 12 sessions of individual counseling. Heart rate was recorded for both therapists and clients during all sessions. Clients completed the ORS before each session to assess short-term outcomes. Long-term outcomes were assessed using the DERS, OQ-30, and CESD-R, administered before and after the counseling sessions. Data from a total of 355 counseling sessions were analyzed. A two-level hierarchical linear model was used to analyze short-term outcomes, and multiple regression analysis was applied for long-term outcomes.
The results indicated that, in terms of short-term outcomes, clients with higher initial emotion regulation difficulties showed stronger predictive associations between physiological synchrony with the therapist in a given session and outcome ratings in the following session. Notably, anti-phase synchrony had a more positive effect on post session outcomes than in-phase synchrony. Regarding long-term outcomes, anti-phase synchrony was a significant predictor of changes in emotion regulation difficulties. In the OQ-30, the impact of synchrony magnitude on interpersonal functioning was moderated by clients’ initial emotion regulation difficulties. Specifically, clients with higher baseline difficulties exhibited greater improvement in interpersonal functioning when average synchrony intensity across 12 sessions was high. Furthermore, anti-phase synchrony significantly predicted changes not only in overall OQ-30 but also in the subdomain of subjective distress. In contrast, neither the intensity nor the pattern of synchrony significantly influenced changes in depressive symptoms.
This study is significant in that it systematically analyzed the relationship between client change and therapist–client physiological synchrony across the full course of counseling, rather than focusing solely on early or isolated sessions. By considering not only the magnitude and sign of synchrony but also the client’s initial emotion regulation difficulties, the study provides empirical evidence that helps clarify the mechanisms of interpersonal emotion regulation. These findings suggest the potential for clinical interventions aimed at enhancing interpersonal emotion regulation in counseling and, furthermore, demonstrate the scientific utility of physiological synchrony as a biological marker for evaluating counseling outcomes. This highlights the association between interpersonal emotion regulation and counseling outcomes.
