본 연구는 서울시 노인의 영양취약성 현황을 파악하고, 영양취약계층 노인과 관련된 인구사회학적 특성 및 식생활 특성을 파악하는 것을 목적으로 하였다. 2024 서울시민 먹거리조사 원시자료 중 65세 이상 노인 720명의 자료를 분석하였다. 산출된 노인 영양지수에 따라 분석 대상자를 영양취약계층 노인(영양지수‘하’등급)과 비영양취약계층 노인(영양지수‘상’및‘중’등급)으로 분류하였다. 모든 분석은 가중치, 층화, 집락을 반영한 복합표본분석으로 수행하였다. 그 결과, 서울시 노인의 약 19%가 영양취약계층으로 추정되었다. 영양취약계층 노인은 아침식사 빈도 및 식생활역량, 식품디지털역량, 식품환경만족도가 비영양취약계층 노인에 비해 낮았다. 로지스틱 회귀분석 결과, 서울시 노인의 영양취약성과 관련된 특성은 성별, 학력, 식생활역량이었다. 남성 노인은 여성 노인에 비해, 학력이 중졸 이하인 노인은 고졸 이상인 노인에 비해, 영양취약계층일 가능성이 각각 6배 이상(OR=6.14), 4배 이상이었다(OR=4.11). 또한, 0점에서 100점 범위로 측정된 식생활역량 점수가 1점 증가할수록, 영양취약계층일 가능성이 약 10% 낮았다(OR=0.90). 남성 노인을 대상으로 추가 분석을 실시한 결과, 남성 노인의 영양취약계층 비율(26.3%)은 여성 노인(13.1%)의 약 2배였다. 남성 노인만을 대상으로 로지스틱 회귀분석을 실시한 결과, 남성 노인의 영양취약성과 관련된 특성은 학력, 가구형태, 식생활역량이었다. 학력이 중졸 이하인 노인은 고졸 이상인 노인에 비해, 1인 가구 노인은 다인 가구 노인에 비해, 영양취약계층일 가능성이 각각 약 6배 이상(OR=6.87), 약 3배 이상이었다(OR=3.05). 또한, 식생활역량이 1점 높을수록 영양취약계층일 가능성이 약 9% 낮았다(OR=0.91). 결론적으로, 서울시 노인 중 남성이거나 중졸 이하의 학력을 가진 경우, 식생활역량이 낮은 경우, 영양취약계층일 가능성이 높았다. 또한, 남성 노인 중에서는 1인 가구인 경우, 영양취약계층일 가능성이 높았다. 따라서 서울시 노인을 대상으로 한 식생활 정책 개발 시, 1인 가구 남성 노인을 정책 대상으로 우선 고려할 필요가 있다. 또한, 식생활역량이 낮은 노인이 건강한 식생활을 실천할 수 있도록 지원하는 정책이 필요하다. 이러한 정책으로는 노인의 식생활역량을 높이기 위한 식생활 교육 정책과 더불어 커뮤니티 급식 확대, 고령친화적인 가정간편식 보급 등과 같은 식생활 환경 정책이 병행되어야 한다.

This study aimed to assess the nutritional vulnerability status of the elderly in Seoul and to identify the sociodemographic and dietary characteristics associated with nutritional vulnerability. The present study analyzed data from 720 elderly individuals aged 65 years or older from the Seoul Food Survey 2024. Based on their NQ-E scores, respondents were classified into the nutritionally vulnerable group (low grade) and the non-vulnerable group (medium and high grades). All analyses were conducted using complex sample analysis, accounting for weighting, stratification, and clustering. The results showed that 19% of the elderly in Seoul were estimated to be nutritionally vulnerable. The nutritionally vulnerable elderly showed lower breakfast frequency, food literacy, food digital literacy, and food environment satisfaction than the non‑vulnerable elderly. Logistic regression analysis revealed that nutritional vulnerability was significantly associated with gender, educational level, and food literacy. The odds of being nutritionally vulnerable were over six times higher for elderly men than for elderly women (OR = 6.14), and over four times higher for those with a middle school education or less than for those with a high school education or higher (OR = 4.11). In addition, each one-point increase in food literacy (measured on a scale of 0 to 100) was associated with approximately a 10% decrease in the odds of being nutritionally vulnerable (OR = 0.90). As a result of additional analyses conducted for elderly men, the proportion of nutritionally vulnerable individuals was about twice as high among elderly men (26.3%) compared to elderly women (13.1%). Logistic regression analysis revealed that, among elderly men, nutritional vulnerability was significantly associated with educational level, household type, and food literacy. Specifically, the odds of being nutritionally vulnerable were over six times higher for those with a middle school education or less than for those with a high school education or higher (OR = 6.87), and over three times higher for those living alone than for those living with others (OR = 3.05). In addition, each one-point increase in food literacy was associated with approximately a 9% decrease in the odds of being nutritionally vulnerable (OR = 0.91). In conclusion, elderly individuals in Seoul who were male, had a middle school education or less, or had low food literacy were more likely to be nutritionally vulnerable. Among elderly men, those living alone showed a higher likelihood of nutritional vulnerability. Therefore, when developing dietary policies for the elderly in Seoul, priority should be given to elderly men living alone. In addition, policies are needed to support elderly individuals with low food literacy in practicing healthy dietary habits. Such policies should integrate educational initiatives aimed at enhancing food literacy with environmental interventions, such as expanding community meal services and promoting the distribution of senior-friendly home meal replacements.
