    Statin Use and the Risk of New-Onset Diabetes Mellitus
    - A Meta Analysis of Randomized Controlled Trials and Cohort Studies-


    Statins are widely used as primary and secondary prevention medications for cardiovascular disease. With the growing global use and expanding market for statin, clinical concern has been raised regarding an increased risk of diabetes. The U.S. Food and Drug issued a warning in 2012, stating that statin therapy may lead to elevated blood glucose levels and increased risk of developing type 2 diabetes, thus drawing attention to their potential metabolic side effects.
    This study aimed to comprehensively evaluated the association between statin use and (1) changes in glycemic metabolic markers (2) the risk of developing type 2 diabetes. A meta-analysis was conducted separately for randomized controlled trials (RCTs) and cohort studies reporting relevant outcomes, with the goal of integrating the findings.
    Following the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, a systematic and transparent literature selection process was implemented. A total 16 randomized controlled trials (RCTs) and 11 cohort studies published in the past two decades were included. For randomized controlled trials (RCTs), a meta-analysis was performed on outcomes in fasting plasma glucose, HbA1c, insulin sensitivity (QUICKI), and insulin resistance (HOMA-IR). For Cohort studies, adjusted hazard ratio (HRs) for new-onset diabetes were analyzed.
    In the Randomized Controlled Trials (RCTs) meta-analyses, most glycemic metabolic markers showed statistically insignificant change, and conflicting findings were observed between indicators of insulin resistance (HOMA-IR) and insulin sensitivity (QUICKI). HOMA-IR significantly decreased in relatively longer statin therapy in RCT studies. In contrast, the meta-analysis of cohort studies showed a statistically significant increase in the risk of new-onset type 2 diabetes associated with statin use (HR = 1.40, 95% CI: 1.24- 1.58). However, the included cohort studies employed highly heterogeneous adjustment methods and variable covariate structures, ranging from robust propensity score matching and time varying exposure models to minimal adjustment limited to demographics and comorbidities.
    Given the higher internal validity of randomized controlled trials, the present analysis suggests that evidence from Randomized Controlled Trials (RCTs) provides a more reliable basis for assessing the causal metabolic effects of statins. Therefore, statin therapy should not be withheld solely based on concerns about potential diabetogenic effects. Instead, clinical decisions should be grounded in a comprehensive risk-benefits assessment, with considering the well-established cardiovascular benefits of statins. In high-risk populations, concurrent implementation of lifestyle interventions such as diet and exercise remain essential. Regular monitoring of blood glucose and personalized treatment strategies are recommended when prescribing statins.

    스타틴(Statins)은 심혈관 질환 예방을 위해 사용되는 대표적인 약물이지만, 스타틴(Statins)복용이 당뇨병 발생 위험도를 증가한다는 연구는 계속해서 보고되어 왔다. 이에, 2012년 2월 28일, 미국식품의약국(U.S. Food and Drug Administration, FDA)은 스타틴(Statins) 계열의 콜레스테롤 저하제에 대한 혈당 수치 증가 및 제 2형 당뇨병 발병 위험 증가에 대한 안전성 정보 변경을 승인하였다. 이에, 본 연구는 스타틴(Statins) 복용이 혈당 대사 지표에 대한 변화 및 당뇨병 발생 위험 증가 위험도와의 연관성을 통합적으로 평가하고자 하였다. 본 연구에서는 무작위 대조시험 (Randomized Controlled Trial, RCT)와 코호트 연구를 선별하여, 보고된 결과 지표에 대하여 각각 메타분석을 수행하였으며, 이를 종합적으로 분석하는 것을 목적으로 하였다.
