    Due to advancements in medical technology and the shift towards chronic disease dominance, the dying process has progressively lengthened, and nuclear family structures along with increased women's labor participation have shifted primary end-of-life care for cancer patients from homes to hospitals. However, hospital-centric end-of-life care often fails to provide continuous and integrated care for terminal cancer patients experiencing multifaceted suffering, resulting in fragmented treatments as patients frequently transfer between hospitals. Such fragmented care and frequent transfers can significantly diminish the quality of life for patients and their caregivers. Existing research often provides fragmented analyses based on healthcare utilization during the final 1 to 6 months before death, failing to comprehensively describe actual healthcare trajectories. Longitudinal analyses reflecting changes in healthcare utilization goals post-terminal diagnosis are particularly lacking. Furthermore, in contexts such as South Korea, where inter-institutional coordination is limited and patient choice is emphasized, end of life care trajectories may significantly differ based on gender, age, region, and socioeconomic status. Consequently, empirical longitudinal studies are required to trace and analyze these trajectories and their determinants.

    This study longitudinally examines end of life care utilization trajectories and related determinants among cancer patients in their final year of life. The specific objectives include: firstly, identifying primary healthcare utilization trajectories using latent transition analysis; secondly, analyzing healthcare utilization changes and associated factors pre- and post-terminal diagnosis through panel logistic regression; and thirdly, employing interrupted time series analysis to explore healthcare utilization changes during the COVID-19 period. This approach aims to comprehensively understand cancer patients' healthcare utilization and derive policy implications for enhancing end-of-life care quality, especially during crises such as COVID-19.

    Data were sourced from National Health Insurance claims and cause-of-death records, covering 471,753 cancer-related deaths from 2016 to 2021. These data include sociodemographic details, diagnoses, medical expenses, healthcare utilization, and hospital characteristics, making them particularly suitable for this analysis.
    The research encompasses three phases: (1) trend analysis and latent transition analysis to map healthcare trajectories over the final year; (2) panel logistic regression to investigate changes in healthcare utilization and related determinants around the terminal diagnosis; and (3) interrupted time series analysis assessing healthcare utilization patterns during the COVID-19 pandemic.

    In phase (1), chemotherapy and nursing hospital utilization significantly increased across all periods within the final year (p<0.001). Hospice utilization also rose but was primarily concentrated within the final month of life. ICU admission rates showed no significant trend change. Latent transition analysis identified five primary latent healthcare utilization states: chemotherapy-focused, hospitalization-focused, nursing hospital-focused, hospice-focused, and minimal healthcare utilization. The predominant trajectory was transitioning from chemotherapy to acute hospital admission close to death, persisting despite increased hospice care. Trajectories varied considerably by gender, age, region, and insurance status.

    Phase (2) revealed significant changes post-terminal diagnosis, with about 50% of patients dying within 30 days, indicating very late-stage notification. Post-terminal diagnosis, chemotherapy, ICU admissions, vasopressor usage, and hospital transfer distances decreased. Admissions to tertiary and general hospitals declined, whereas nursing hospital admissions increased. However, younger patients frequently continued chemotherapy and traveled greater distances to tertiary hospitals post-diagnosis.

    Phase (3) demonstrated minimal changes in chemotherapy use during COVID-19 but significant increases in emergency department visits and vasopressor use, alongside reduced ICU admissions. General, nursing, and smaller hospital admissions decreased, shifting toward outpatient care, whereas tertiary hospital admissions remained stable. Hospice use dramatically declined immediately after the onset of COVID-19.
    The study also highlighted distinct variations in healthcare trajectories by gender, age, region, and socioeconomic status. Women utilized fewer intensive treatments but had higher hospice and nursing hospital utilization. After controlling for age, region, and socioeconomic status, women had higher tertiary hospital admission rates, suggesting that underlying economic conditions, caregiving availability, and health status disparities influenced hospital admissions.

    Age was identified as a critical determinant: younger adults (20–64 years) exhibited higher rates of intensive treatments and tertiary hospital admissions than older adults (75+), a pattern persisting post-terminal diagnosis, reflecting stronger life-prolongation motivations driven by social and economic roles. Regionally, metropolitan residents had higher chemotherapy and tertiary hospital utilization rates, and residents from remote areas traveled greater distances, suggesting regional disparities in end-of-life care access. Socioeconomically, higher-income groups had greater utilization of tertiary hospitals and chemotherapy, while lower-income and medical aid recipients predominantly utilized nursing hospitals. Even post-terminal diagnosis, higher-income groups continued to preferentially seek care from distant tertiary hospitals, incurring additional caregiving and travel costs to access perceived superior care.
    This study underscores systemic issues in Korea's end-of-life care: (1) predominant reliance on acute hospital care until death, limiting timely hospice care; (2) excessively late terminal diagnosis notifications, hindering smooth care transitions; (3) socioeconomic factors prompting prolonged intensive treatments at distant hospitals; and (4) marked declines in hospice utilization during the COVID-19 pandemic, highlighting system vulnerability during crises.
    Recommendations include: implementing early palliative care interventions from cancer diagnosis onward; enhancing quality management in nursing hospital care; providing timely and clear prognostic information; developing tailored end-of-life care policies considering life-stage characteristics; restructuring healthcare delivery systems to mitigate regional and socioeconomic disparities; and establishing robust response strategies to ensure uninterrupted end-of-life care during future emergencies.

    의료기술이 발달하고 주된 사망 원인이 급성질환에서 만성질환으로 변화하면서 사망에 이르는 과정이 점차 연장되었다. 핵가족화와 여성의 사회 활동 참여 증가로 암 환자의 생애말기 돌봄의 장소가 가정에서 병원으로 이동하였다. 그러나 병원 중심의 생애말기 돌봄은 총체적 고통을 경험하는 말기 암 환자에게 연속적이고 통합적인 돌봄을 제공하지 못하고 있어, 암 환자는 상태가 악화됨에 따라 여러 병원을 전원하며 분절된 치료를 받고 있다. 이러한 분절된 치료와 잦은 전원은 환자와 보호자의 삶의 질을 악화시킬 수 있다. 그러나 기존 연구들은 암 환자의 사망 전 1~6개월의 평균 의료 이용량을 단편적으로 분석하여, 환자의 실제 의료 이용 경로를 파악하는 데 한계가 있다. 암 환자는 말기 고지를 받으면 치료 목적이 완치에서 완화로 전환되지만, 말기 고지 전후 의료 이용의 변화를 분석한 연구는 미흡한 실정이다. 더욱이 한국처럼 의료기관 간 연계가 미비하고 환자의 선택권이 강조되는 환경에서는 성별, 연령, 지역, 사회경제적 수준 등에 따라 의료 이용 경로가 상이하게 나타날 수 있다. 이에 따라, 암 환자의 사망 전 의료 이용 경로를 종단적으로 추적하고, 그 변화 양상과 결정 요인을 실증적으로 분석하는 연구가 필요하다.

    자료원은 국민건강보험 청구자료와 사망 원인 자료로, 2016년~2021년 암으로 사망한 471,753명을 대상으로 한다. 국민건강보험 청구자료에는 인구 사회학적 정보, 진단명, 의료비, 의료 이용 내역, 병원의 종별 특성 등이 포함되어 있어 암 환자의 사망 전 의료 이용 분석에 적합하다. 따라서 본 연구의 자료원으로 선정하였다.

    연구 ②에서는 말기 고지 전후의 의료 이용 변화와 병원 이동 거리 변화를 패널 로지스틱 회귀분석과 패널 선형 회귀분석으로 제시하였다. 말기 고지를 받은 환자들은 사망 전 항암치료, 중환자실 입원 및 승압제 이용이 감소하였으며, 병원 이동 거리가 감소하였다. 또한 상급종합병원 입원과 종합병원 입원은 감소하였으나 요양병원 입원은 증가하였다. 그러나 20~64세의 환자에서는 말기 고지 이후에도 항암치료를 지속하는 비율이 높았으며, 원거리의 상급종합병원을 이용하는 경향 또한 두드러지게 나타났다.

    연구 ③에서는 코로나 19 시기에 암 환자의 사망 전 의료 이용 변화를 단절적 시계열 분석을 통해 분석하였다. 코로나 19 시기 동안 항암치료 이용량은 큰 변화가 없었으나, 응급실 방문과 승압제 사용은 증가하고 중환자실 입원은 감소했다. 종합병원과 요양병원, 병원 입원이 감소하고 외래로 전환되는 경향이 보였으나 상급종합병원 입원은 유지되는 경향을 보였다. 호스피스 이용은 코로나 19 직후 가장 큰 폭으로 감소하였다.

    또한 본 연구에서는 성별, 연령, 지역, 경제적 수준에 따라 의료 이용 경로가 어떻게 상이한지를 제시하였다. 먼저, 암 환자의 사망 전 의료 이용 경로는 성별에 따라 뚜렷한 차이를 보였다. 여성은 남성보다 고강도 치료 비율이 낮았고, 호스피스 이용률과 요양병원 이용률이 높았다. 그러나, 연령, 지역, 경제적 수준 등을 통제한 후, 여성의 상급종합병원 입원율이 남성보다 높게 나타났다. 이는 성별에 내포된 경제적 수준, 돌봄 제공자 유무, 건강 상태의 차이가 병원 입원에 영향을 주는 것을 시사한다.
    연령은 의료 이용 경로에 가장 큰 영향을 미치는 요인으로 보인다. 20~64세의 청장년층은 고강도 치료 및 상급종합병원 입원 비율이 75세 이상 고령층보다 높았으며, 이러한 경로의 차이는 말기 고지 이후에도 뚜렷하게 유지되었다. 이는 청장년층이 사회적, 경제적 역할이 있기에 생명 연장에 대한 동기가 강력하며, 실제 의료 이용에 이러한 동기가 반영됨을 보여준다.
    지역별 분석의 경우 수도권에서 항암치료 이용과 상급종합병원 이용 비율이 높았다. 또한, 수도권에서 먼 지역일수록 평균 이동 거리가 증가하였다. 말기 고지 이후의 이동 거리에도 지역간 편차가 있는 것으로 보아 중소도시 거주자들은 자신이 살던 곳에서 사망하지 못하고 대도시 혹은 수도권의 병원에서 사망하는 것으로 보인다.
    건강보험 분위별 분석 결과, 상급종합병원 입원율과 항암치료 이용률은 경제적 수준이 높을수록 증가했고, 요양병원 입원율은 의료급여 수급자일수록 증가했다. 말기 고지 이후 경제적 수준에 따른 항암치료 이용률의 차이는 사라졌지만, 상급종합병원 입원 격차는 그대로 유지되었다. 또한, 병원 이동 거리에 있어서도 경제적 수준이 높을수록 더 먼 거리를 이동하는 경향이 관찰되었다. 이는, 경제적으로 여유가 있는 환자일수록 간병비, 숙박비, 이동 비용 등 많은 대가를 지불하고서라도 최선의 의료 서비스를 받을 수 있다고 생각되는 상급종합병원의 생애말기 돌봄을 선호하는 것을 의미한다.

    본 연구는 암 환자의 사망 전 의료 이용을 종단적으로 분석함으로써 국내 생애말기 돌봄 시스템의 문제점을 도출하였다. 첫째, 암 환자의 주된 사망까지의 경로는 사망 직전까지 고강도 치료를 받다가 급성기 병원에 입원하여 사망하는 경로이며, 이는 호스피스 완화의료가 제때 충분히 제공되지 못하고 있음을 시사한다(연구 ①). 둘째, 말기 고지가 이루어지면 항암치료 및 상급종합병원 입원이 감소하여 환자와 보호자가 합리적인 선택을 하는 것으로 보인다. 셋째, 소득 수준이 높고 연령이 낮을수록 말기 고지 이후에도 원거리의 상급종합병원에서 고강도 치료를 지속하는 경향이 있다(연구 ②). 마지막으로, 코로나 19시기 호스피스 이용은 급감하여 국가적 재난 상황에서 생애말기 돌봄 체계의 취약성을 드러내었다(연구 ③).

    이에 본 연구는 암 환자의 생애말기 돌봄의 질 향상을 위해 다음과 같이 제도적 개선 사항을 제안하고자 한다. 먼저, 조기 완화의료 제도를 도입하여 암 진단 시기부터 완화의료 제도에 접근할 수 있도록 정책을 재설계해야 한다. 다음은, 요양병원에서의 생애말기 돌봄의 질적 관리를 강화해야 한다. 셋째, 환자와 보호자에게 시기적절하고 명확하게 예후에 대한 정보를 제공해야 한다. 넷째, 생애주기별 특성을 고려한 맞춤형 생애말기 돌봄 정책을 수립해야 한다. 다섯째, 지역 및 소득에 따른 의료 접근성 차이를 해소하기 위해 의료 전달체계를 개편해야 한다. 마지막으로, 미래의 재난 상황에서도 생애말기 돌봄이 중단되지 않도록 대응 전략을 수립해야 한다.

    연구는 크게 세 단계로 구성된다. ① 추세 분석과 잠재전이분석으로 암 환자의 사망 전 1년간의 의료 이용 경로 파악 ② 패널 로지스틱 회귀분석을 통해 말기 고지 전후 의료 이용 변화와 관련 요인 분석 ③ 단절적 시계열 분석으로 코로나 19 시기의 사망 전 1년 동안의 의료 이용 변화 분석

    연구 ①의 추세 분석 결과, 2016년부터 2021년까지 항암치료와 요양병원의 이용률은 사망 전 1년 동안 유의하게 증가하였다. 호스피스 이용률 또한 증가하였으나, 이용 시점은 주로 사망 전 1개월에 한정되었다. 반면, 중환자실 입원율은 유의한 증감이 없었다. 잠재전이분석의 결과, 주요 의료 이용은 ‘항암치료 중심군’, ‘입원 중심군’, ‘요양병원 중심군’, ‘호스피스 중심군’, ‘의료 이용 안함군’의 5개의 잠재 상태로 유형화되었다. 2016년 암 사망자의 주된 의료 이용 경로는 항암치료를 지속하다 사망에 가까워지면서 급성기 병원에 입원하여 사망하는 경로였다. 이 경로는 2016년~2021년 호스피스 이용률이 증가하였음에도 주요 의료 이용 경로로 유지되었다. 또한, 암 환자의 의료 이용 경로는 성별, 연령별, 지역별 및 건강보험 분위별로 상이하였다.

    연구 ②에서는 말기 고지 전후의 의료 이용 변화와 병원 이동 거리 변화를 패널 로지스틱 회귀분석과 패널 선형 회귀분석으로 제시하였다. 말기 고지를 받은 환자들은 사망 전 항암치료, 중환자실 입원 및 승압제 이용이 감소하였으며, 병원 이동 거리가 감소하였다. 또한 상급종합병원 입원과 종합병원 입원은 감소하였으나 요양병원 입원은 증가하였다. 그러나 20~64세의 환자에서는 말기 고지 이후에도 항암치료를 지속하는 비율이 높았으며, 원거리의 상급종합병원을 이용하는 경향 또한 두드러지게 나타났다.

    연구 ③에서는 코로나 19 시기에 암 환자의 사망 전 의료 이용 변화를 단절적 시계열 분석을 통해 분석하였다. 코로나 19 시기 동안 항암치료 이용량은 큰 변화가 없었으나, 응급실 방문과 승압제 사용은 증가하고 중환자실 입원은 감소했다. 종합병원과 요양병원, 병원 입원이 감소하고 외래로 전환되는 경향이 보였으나 상급종합병원 입원은 유지되는 경향을 보였다. 호스피스 이용은 코로나 19 직후 가장 큰 폭으로 감소하였다.

    또한 본 연구에서는 성별, 연령, 지역, 경제적 수준에 따라 의료 이용 경로가 어떻게 상이한지를 제시하였다. 먼저, 암 환자의 사망 전 의료 이용 경로는 성별에 따라 뚜렷한 차이를 보였다. 여성은 남성보다 고강도 치료 비율이 낮았고, 호스피스 이용률과 요양병원 이용률이 높았다. 그러나, 연령, 지역, 경제적 수준 등을 통제한 후, 여성의 상급종합병원 입원율이 남성보다 높게 나타났다. 이는 성별에 내포된 경제적 수준, 돌봄 제공자 유무, 건강 상태의 차이가 병원 입원에 영향을 주는 것을 시사한다.
    연령은 의료 이용 경로에 가장 큰 영향을 미치는 요인으로 보인다. 20~64세의 청장년층은 고강도 치료 및 상급종합병원 입원 비율이 75세 이상 고령층보다 높았으며, 이러한 경로의 차이는 말기 고지 이후에도 뚜렷하게 유지되었다. 이는 청장년층이 사회적, 경제적 역할이 있기에 생명 연장에 대한 동기가 강력하며, 실제 의료 이용에 이러한 동기가 반영됨을 보여준다.
    지역별 분석의 경우 수도권에서 항암치료 이용과 상급종합병원 이용 비율이 높았다. 또한, 수도권에서 먼 지역일수록 평균 이동 거리가 증가하였다. 말기 고지 이후의 이동 거리에도 지역간 편차가 있는 것으로 보아 중소도시 거주자들은 자신이 살던 곳에서 사망하지 못하고 대도시 혹은 수도권의 병원에서 사망하는 것으로 보인다.
    건강보험 분위별 분석 결과, 상급종합병원 입원율과 항암치료 이용률은 경제적 수준이 높을수록 증가했고, 요양병원 입원율은 의료급여 수급자일수록 증가했다. 말기 고지 이후 경제적 수준에 따른 항암치료 이용률의 차이는 사라졌지만, 상급종합병원 입원 격차는 그대로 유지되었다. 또한, 병원 이동 거리에 있어서도 경제적 수준이 높을수록 더 먼 거리를 이동하는 경향이 관찰되었다. 이는, 경제적으로 여유가 있는 환자일수록 간병비, 숙박비, 이동 비용 등 많은 대가를 지불하고서라도 최선의 의료 서비스를 받을 수 있다고 생각되는 상급종합병원의 생애말기 돌봄을 선호하는 것을 의미한다.

    본 연구는 암 환자의 사망 전 의료 이용을 종단적으로 분석함으로써 국내 생애말기 돌봄 시스템의 문제점을 도출하였다. 첫째, 암 환자의 주된 사망까지의 경로는 사망 직전까지 고강도 치료를 받다가 급성기 병원에 입원하여 사망하는 경로이며, 이는 호스피스 완화의료가 제때 충분히 제공되지 못하고 있음을 시사한다(연구 ①). 둘째, 말기 고지가 이루어지면 항암치료 및 상급종합병원 입원이 감소하여 환자와 보호자가 합리적인 선택을 하는 것으로 보인다. 셋째, 소득 수준이 높고 연령이 낮을수록 말기 고지 이후에도 원거리의 상급종합병원에서 고강도 치료를 지속하는 경향이 있다(연구 ②). 마지막으로, 코로나 19시기 호스피스 이용은 급감하여 국가적 재난 상황에서 생애말기 돌봄 체계의 취약성을 드러내었다(연구 ③).

    이에 본 연구는 암 환자의 생애말기 돌봄의 질 향상을 위해 다음과 같이 제도적 개선 사항을 제안하고자 한다. 먼저, 조기 완화의료 제도를 도입하여 암 진단 시기부터 완화의료 제도에 접근할 수 있도록 정책을 재설계해야 한다. 다음은, 요양병원에서의 생애말기 돌봄의 질적 관리를 강화해야 한다. 셋째, 환자와 보호자에게 시기적절하고 명확하게 예후에 대한 정보를 제공해야 한다. 넷째, 생애주기별 특성을 고려한 맞춤형 생애말기 돌봄 정책을 수립해야 한다. 다섯째, 지역 및 소득에 따른 의료 접근성 차이를 해소하기 위해 의료 전달체계를 개편해야 한다. 마지막으로, 미래의 재난 상황에서도 생애말기 돌봄이 중단되지 않도록 대응 전략을 수립해야 한다.
