본 연구는 1차 의료서비스를 중심으로 의료소비자의 불만처리 경험이 의료기관에 대한 신뢰와 관계몰입을 매개로 의료서비스 지속이용의도에 미치는 영향을 실증적으로 분석하였다. 의료서비스는 정보 비대칭성과 전문지식 의존도가 높은 대표적인 신뢰재로, 소비자가 불만을 경험했을 때 이를 어떻게 처리되는가가 향후 신뢰 형성과 장기적 관계 유지에 중요한 영향을 미친다. 본 연구는 국내 1차 의료기관 이용자 중 불만 경험과 불만처리 경험이 모두 있는 소비자를 대상으로 불만의 원인과 불만호소행동, 불만처리에 대한 기대와 인식 수준을 파악하고, 불만처리의 공정성 인식(분배·절차·상호작용)이 신뢰와 관계몰입을 통해 지속이용의도에 미치는 경로를 다중회귀분석과 경로분석으로 검증하였다. 분석 결과, 상호작용 공정성과 분배공정성 인식은 지속이용의도에 유의한 영향을 미쳤으며, 특히 상호작용 공정성은 신뢰와 관계몰입을 순차적으로 매개하여 장기적 서비스 이용을 촉진하는 것으로 나타났다. 이를 통해 의료서비스 불만처리가 단순한 문제해결을 넘어 소비자의 신뢰와 관계몰입을 강화하고, 궁극적으로 지속적인 서비스 이용과 건강관리로 이어질 수 있음을 확인하였다. 본 연구는 불만처리의 공정성 차원을 세분화하고 신뢰·관계몰입·지속이용의도와 연계하여 실증적으로 규명함으로써, 기존에 개별적으로 논의되던 소비자불만, 신뢰 및 관계몰입, 지속이용의도 간의 관계를 하나의 통합적 관점에서 설명했다는 점에서 학문적 의의가 있다. 또한, 실무적으로는 소비자 신뢰와 관계몰입을 강화하여 장기적인 의료서비스 이용과 지속적 건강관리를 지원할 수 있는 의료기관 불만처리 시스템의 개선 방향성을 제시했다는 점에서 의미가 있다.

This study empirically examines the impact of medical consumers’ complaint-handling experiences on their intention to continue using primary healthcare services, with a focus on the mediating roles of trust and relationship commitment. As healthcare services are characterized by high information asymmetry and a strong reliance on professional expertise, the way in which consumer complaints are addressed plays a critical role in building trust and maintaining long-term relationships. This research targeted users of primary medical institutions in Korea who had experienced both dissatisfaction and subsequent complaint handling. The study explored the causes of complaints, complaint behaviors, expectations for resolution, and perceived fairness in the complaint-handling process—specifically distributive, procedural, and interactional fairness. Using multiple regression and path analysis, the findings reveal that perceptions of interactional and distributive fairness significantly influence consumers’ intentions to continue using services, with interactional fairness demonstrating a pronounced indirect effect through sequential mediation by trust and relationship commitment. These results indicate that effective complaint handling can extend beyond immediate problem resolution to foster stronger trust and commitment, ultimately encouraging sustained service use and ongoing health management. The study contributes to the literature by integrating and empirically validating the relationships among consumer complaints, trust, relationship commitment, and continued use intention, domains that have often been examined separately. From a practical perspective, the findings offer insights for developing improved complaint-handling systems that strengthen consumer trust and relationship commitment, supporting long-term patient engagement in primary care.
