연구 배경: 정신건강 문제의 심각성이 커지는 가운데, 녹지는 공중보건적 개입 수단으로 주목받고 있다. 그러나 녹지의 불균형한 공간적 분포는 건강 형평성의 지역 간 격차로 이어질 가능성이 있다. 2022년 서울시는 ‘초록길 프로젝트’를 통해 생활권 단위의 녹지 공급을 확대하겠다고 발표했으나, 실제로 생활권 녹지 수준이 향상되었는지, 소지역 단위의 녹지가 주민의 건강에 실질적으로 기여하는지에 대한 검토는 부족하다. 기존 연구들은 주로 구 단위에서 단순 면적 평균으로 녹지 수준을 측정하여 개인의 실제 생활권 내 녹지 노출을 부정확하게 반영할 우려가 있다.
연구 목적: 본 연구에서는 서울시의 행정동 수준에서 인구가중 평균 방법을 적용하여 생활권 내 주민이 실제로 체감하는 녹지 노출 수준을 보다 정밀하게 반영하고, 이를 기반으로 소지역(행정동) 단위에서 녹지 수준과 주요 정신건강 결과(우울, 스트레스, 걷기) 간에 어떠한 상관성을 보이는지를 분석한다.
연구 방법: 본 연구는 서울시 423개 행정동을 대상으로, 2021년부터 2023년까지의 지역사회건강조사 원시자료 기반으로 개인의 우울감 경험 여부, 우울증상 유병 여부, 주관적 스트레스 인지 여부, 걷기 실천 여부 변수와 위성데이터로 산출한 행정동 단위의 녹지 변수를 결합하여 건강과의 연관성을 분석하였다. 녹지 변수는 Sentinel-2 위성의 10m 해상도 EVI 데이터를 수집하여 100m 격자 단위 인구 자료와 결합해 인구가중 및 비가중 평균 녹지지수를 산출하였다. 지역 단위 녹지 변수와 개인 수준 건강 결과를 결합한 구조적 특성을 고려하여, 다수준 로지스틱 회귀분석을 수행함으로써 동일 행정동 내 응답자 간 상관구조를 통제하고 행정동 간 건강 결과의 구조적 차이를 추정하였다.
연구 결과: 행정동 수준에서 측정한 녹지 노출은 우울증상 유병 여부 및 주관적 스트레스 인지 여부와 음의 상관성을 보였고, 걷기 실천 여부와는 양의 상관성을 보였다. 구체적으로는 연도에 따른 고정효과를 보정하였을 때 행정동 단위에서 인구가중 평균(75m 반경) 방법으로 측정한 녹지 수준이 동일 연도 내에서 IQR만큼 높을수록, 우울증상 유병 오즈는 0.91배(95% CI: 0.833–0.994), 주관적 스트레스 인지 오즈는 0.963배(95% CI: 0.928-0.999), 걷기 실천 오즈는 1.062배(95% CI: 1.001-1.117)로 나타났다. 이러한 상관성은 녹지 산출 방법 및 개인의 인구사회학적 특성에 따라 다르게 나타났다.
결론: 본 연구는 서울시 행정동 수준에서 생활권 기반으로 측정한 녹지 노출이 정신건강과 보호적 상관성을 가질 수 있음을 실증적으로 제시하였다. 인구가중 방식의 소지역 단위 녹지 지표는 실제 거주 인구의 공간 분포를 반영함으로써 건강과의 관계를 보다 세밀하게 파악할 수 있는 분석 도구로 활용될 가능성을 보여주었다. 아울러, 본 연구는 생활권 내 녹지와 정신건강의 관계가 인구집단 특성에 따라 이질적으로 나타나고 행정동 간 건강 격차가 존재할 수 있음을 확인함으로써, 향후 보다 미시적 공간 단위에서의 건강 연구 및 형평성 평가의 필요성을 뒷받침하는 기초 자료로 활용될 수 있다.

Background: As mental health issues become increasingly serious, green space has garnered attention as a potential means of public health intervention. However, the uneven spatial distribution of green space may lead to regional disparities in health equity. In 2022, the Seoul Metropolitan Government announced the “Greenway Project” to expand neighborhood-level green space supply. Nevertheless, few studies have assessed whether neighborhood-level green space exposure has actually improved and whether such green space meaningfully contributes to residents’ health at small-area levels. Existing studies have mainly measured green space levels using simple area-averaged metrics at the district level, which may inaccurately reflect residents’ actual exposure to green space within their living environments.
Objective: This study aims to more precisely reflect perceived exposure to green space within residential living areas by applying a population-weighted averaging method at the administrative dong level in Seoul. Based on this measure, we analyze how green space levels at small-area units (administrative dongs) are correlated with key mental health outcomes, including depression, perceived stress, and walking behavior.
Methods: This study targeted 423 administrative dongs in Seoul and linked the 2021–2023 raw data from the Korea Community Health Survey with satellite-derived green space indicators at the dong level to examine associations with health outcomes. The green space variables were calculated using Enhanced Vegetation Index (EVI) data from the Sentinel-2 satellite (10m resolution), which were combined with 100m-grid population data to derive both population-weighted and unweighted average greenness indices. Given the hierarchical data structure linking area-level greenness with individual-level mental health outcomes, multilevel logistic regression models were employed to account for the intra-dong correlation among respondents and to estimate structural differences in health outcomes across dongs.
Results: Greenness exposure at the administrative dong level was negatively correlated with the prevalence of depressive symptoms and perceived stress, and positively correlated with walking practice. Specifically, when adjusting for fixed effects of survey year, an IQR increase in population-weighted green space (based on 75-meter buffers) within the same year corresponded to an odds ratio of 0.91 (95% CI: 0.833–0.994) for depressive symptoms, 0.963 (95% CI: 0.928–0.999) for perceived stress, and 1.062 (95% CI: 1.001–1.117) for walking practice. These patterns varied depending on the method of green space calculation and individual sociodemographic characteristics.
Conclusion: This study empirically demonstrates that neighborhood-level green space measured within daily living areas may have a protective correlation with mental health in Seoul. The population-weighted green space indicator, which reflects the actual spatial distribution of the residential population, may serve as a useful analytical tool for identifying more fine-grained relationships between green exposure and health. Furthermore, the study finds that the relationship between green space and mental health differs across population subgroups, and that health disparities may exist between neighborhoods. These findings may serve as foundational evidence supporting the need for future research and equity assessments at smaller spatial scales.
