    This study aims to identify the general and clinical characteristics of pediatric and adolescent patients triaged as Level 1 according to the Korean Triage and Acuity Scale(KTAS), and to analyze the nursing domain and interventions that are provided to these patient episodes. The findings will provide foundational data for the pediatric emergency nursing education. A retrospective study was reviewed using the medical records of 207 patient episodes under 18 years old that were classified as KTAS Level 1 and received care at a pediatric emergency of a tertiary hospital between January 1, 2024 to December 31, 2024. The collected data were analyzed using SPSS WIN 29.0 program, with descriptive statistics and the Mann-Whitney U test. The study results are as follows 1) A total of 50.7% were female, and the median age was 60 months. Children aged 1 to 8 comprised the highest proportion at 54.6%, and 46.4% of patients arrived emergency by emergency medical services. The hospitalization rate including the ward and intensive care unit was 60.4%, and 57.0% of the study subject presented with altered consciousness upon arrival. 17.4% of study subject had visited the emergency department more than four times during the study period. 2) The most common chief complaints based on KTAS major symptom categories were neurological symptoms(56.5%) and respiratory symptoms(32.4%), accounting for 88.9% of all cases. The most common KTAS subcategory symptoms were seizures (106 episodes, 51.2%) among neurological symptom patients and respiratory distress (62 episodes, 30.0%) among respiratory symptom patients. 3) The most frequently performed nursing domain was ‘Assessment and Observation’ which accounted for 41.3%, followed by ‘Medication Administration’, ‘Counseling and Education’, and ‘Communication’. The frequency of ‘Assessment and Observation’ was performed most frequently to all patients, regardless of their level of consciousness or ages. The least frequently performed domains were ‘Nutritional Care’, ‘Wound Care’, and ‘Welfare and Comfort Care’. 4) The most commonly performed nursing interventions provided to the patients was ‘Caregiver explanation’(1,663 times, 9.1%), followed by ‘Pulse rate measurement’(1,368 times, 7.5%), Pulse oximetry measurement(1,332 times, 7.3%)’, and ‘Communication with medical staffs’(1,279 times, 7.0%). 5) When analyzing the nursing domains by level of consciousness, the overall distribution was similar between groups. ‘Specialized intensive care’(3.4%) and ‘Process and examination-related care’(6.1%) were more frequently performed in patients with altered consciousness than alert consciousness group. Meanwhile, the distribution of frequently performed nursing domains was generally consistent across all age groups. 6) Analysis of nursing intervention differences by KTAS major symptom category explained that patients with neurological symptoms more frequently received care in the ‘Temperature measurement’(Z=-2.943, p=.003), ‘Pulse oximetry measurement’(Z=-2.042, p=.041) and ‘Assessment of level of consciousness’(Z=-3.296, p=.001). Conversely, patients visited emergency with respiratory symptoms received significantly more interventions in ‘Communication with medical staffs’(Z=-3.322, p=.001) and ‘Aseptic technique’(Z=-7.826, p<.001). This study identified that the nursing activities provided to pediatric and adolescent patients classified as KTAS Level 1 are diverse and vary depending on the patient’s level of consciousness and KTAS major symptom categories. These findings can serve as a fundamental data for developing pediatric emergency nursing education programs in the future. In addition, further research is necessary to examine the characteristics of pediatric patients visiting to various emergency centers and comparative studies across different KTAS levels are recommended to improve the quality of pediatric emergency nursing care.

    본 연구의 목적은 소아전문응급의료센터에 내원한 소아청소년 중 한국형 응급환자 분류도구(Korean Triage and Acuity Scale, KTAS) Level 1로 분류된 소아청소년 환자 사례의 일반적 및 임상적 특성을 파악하고, 해당 사례를 대상으로 수행한 간호업무를 분석함으로써, 향후 소아 응급간호 교육의 기초자료를 마련하는 데 있다. 본 연구는 2024년 1월 1일부터 2024년 12월 31일까지 서울 소재 일개 소아전문응급의료센터에 내원하여 KTAS Level 1로 분류된 만 18세 미만 환자 207건의 의무기록을 분석한 후향적 조사연구이다. 수집 된 자료는 SPSS WIN 29.0 프로그램을 이용하여 기술통계 및 Mann-Whitney U test를 통해 분석하였다. 연구 결과는 다음과 같다. 1) 전체 207건 중 50.7%가 여아였고, 나이 중위수는 60개월이었다. 1세 이상 8세 미만 연령대가 54.6%로 가장 높은 비율을 차지하였으며, 전체 사례의 46.4%는 119구급차를 이용하여 내원하였다. 병동 및 중환자실의 입원율은 60.4%였고, 내원 당시 의식 변화가 있는 환자는 57.0%로 나타났다. 또한 연구 기간 중 응급의료기관에 4회 이상 내원한 다빈도 내원 환자의 비율은 17.4%로 확인되었다. 2) 주요 KTAS 대분류 증상은 신경계(56.5%)와 호흡기계(32.4%)로 전체 환자의 88.9%를 차지하였다. KTAS 세부 증상으로는 신경계 증상 환자 중 발작이 106건(51.2%)이었으며, 호흡기계 증상 환자 중 호흡곤란이 62건(30.0%)인 것으로 나타났다. 3) 가장 많이 수행된 간호영역은 ‘사정 및 관찰’(7,527회, 41.3%)이었고, 이어서 ‘투약간호’(2,347회, 12.9%), ‘상담 및 교육’(2,164회, 11.9%), ‘의사소통’(2,060회, 11.3%) 순으로 확인되었다. ‘사정 및 관찰’은 환자의 의식 수준이나 나이에 상관없이 빈번하게 수행되었다. 반면, 가장 적게 수행한 간호영역은 ‘영양간호’, ‘상처관리’, ‘안위간호’로 나타났다. 4) 가장 많이 수행된 간호행위 항목은 ‘보호자 설명’(1,663회, 9.1%)이었으며, ‘맥박 수 측정’(1,368회, 7.5%), ‘산소포화도 모니터링 연결 및 판독’(1,332회, 7.3%), ‘의료진과의 의사소통’(1,279회, 7.0%) 순서로 높은 빈도를 나타냈다. 5) 환자의 의식 수준에 따라 간호영역 수행 비율을 확인한 결과, 의식 변화가 있는 환자군에서 ‘특수간호’(3.4%)와 ‘시술 및 검사 관련 간호’(6.1%)의 비율이 의식 명료한 환자군보다 높은 비율을 나타냈다. 한편, 연령군별 자주 수행된 간호영역 빈도는 전반적으로 유사하였다. 6) KTAS 대분류 증상 중 대다수를 차지하는 신경계와 호흡기계 환자군 간 간호행위 수행 차이를 분석한 결과, 신경계 환자에게 ‘체온측정’(Z=-2.943, p=.003), ‘산소포화도 모니터링 연결 및 판독’(Z=-2.042, p=.041), ‘의식 상태 사정’(Z=-3.296, p=.001)이 유의하게 더 많이 수행되었다. 반면, 호흡기계 환자는 ‘의료진과의 의사소통’(Z=-3.322, p=.001), ‘무균술 적용’(Z=-7.826, p<.001)이 더 빈번하게 수행된 것으로 나타났다. 따라서 본 연구는 KTAS Level 1로 분류된 소아청소년 환자에 대해 제공하는 간호업무가 환자의 내원 당시 의식 수준, 연령 및 주요 증상 유형에 따라 수행 양상이 달라질 수 있음을 확인하였다. 이러한 결과는 향후 소아 응급간호 교육 프로그램 개발에 기초자료로 활용될 수 있으며, 다양한 응급의료기관의 사례를 추가로 확보하고, 다른 KTAS Level의 환자들과 비교·분석하여 소아 응급간호의 질 향상을 위한 후속 연구를 제언한다.
