1. 목적
현재 제안되어 있는 치과 방사선영상과 관련된 여러 지침 및 평가표 중 일부는 전문가 합의 연구자료가 확인되지 않거나 항목 간 일관성 부족, 표현의 모호성, 그리고 통일성 결여 등의 한계를 보이고 있다. 이에 이번 연구는 대한영상치의학회에서 제안한 가이드라인 및 관련 지침과 평가표 중 전문가 합의 연구자료가 확인되지 않은 항목을 대상으로 델파이 기법(Delphi method)을 적용하여 전문가 합의를 검증하고, 항목 간 표현의 명확성과 용어의 통일성을 도모하고자 하였다.
2. 연구 재료 및 방법
대한영상치의학회에서 제안한 가이드라인 및 관련 지침과 평가표 중 전문가 합의 연구자료가 확인되지 않은 다음 항목들을 대상으로 설문 문항을 구성하였다.

전문가 합의 도출을 위해 델파이 기법을 적용하였고, 영상치의학 분야의 임상 경험과 학술적 전문성을 기준으로 선정된 11명의 전문가 패널이 참여하였다. 설문 응답은 9점 Likert 척도를 사용하여 수집하였으며, 평균값(average, AVE), 변동계수(coefficient of variation, CV), 동의율(agreement rate, AR)을 이용한 정량 분석을 수행하였다. 합의 기준은 AVE ≥ 8.0, CV ≤ 0.2, AR ≥ 80%로 설정하였다. 전문가 패널의 정성적 피드백을 바탕으로 문항별 추가, 삭제, 수정 및 재동의 과정을 반복적으로 수행하였으며, 임상 적합성, 표현의 명확성, 용어의 통일성을 중심으로 정성 분석을 병행하였다.
3. 결과
델파이 조사는 제1차 라운드에서 81개 문항으로 시작하여 전문가 피드백에 따른 문항 추가•삭제•수정 과정을 거쳐 제2차에서 93개, 최종적으로 95개 문항으로 구성되었다. 라운드별 합의 달성률은 제1차 75.3%(61/81개), 제2차 83.9%(78/93개), 제3차 94.7%(90/95개)로 점진적으로 향상되었다. 각 라운드에서는 전문가의 정성적 피드백을 바탕으로 문항의 임상 적합성, 표현의 명확성, 용어 통일성을 반복적으로 검토하였으며, 그에 따라 문장 수정과 용어 조정이 이루어졌다.
4. 결론
델파이 기법을 활용하여 치과 방사선영상 품질관리와 방사선안전관리 항목의 구성 타당성과 표현 명확성에 대한 전문가 합의를 도출한 이번 연구는 치과 방사선영상의 품질관리 및 안전관리 분야에서 보다 효율적인 실무 지침으로 활용될 수 있을 것이다.

Among the existing guidelines and evaluation checklists for dental radiographic imaging, several lack documented expert consensus and exhibit inconsistencies in item structure, ambiguous wording, and terminological non-uniformity. This study applied the Delphi method to those items without verified consensus in the guidelines and related documents issued by the Korean Academy of Oral and Maxillofacial Radiology (KAOMFR) to confirm expert agreement and enhance clarity and consistency across items.
Materials and Methods
This study targeted items lacking documented expert consensus within various guidelines and evaluation checklists issued by the KAOMFR. Ten source documents were included:
(1) a simplified image-quality checklist for periapical radiographs;
(2) a simplified image-quality checklist for bitewing radiographs;
(3) a simplified image-quality checklist for cephalometric radiographs;
(4) a simplified image-quality checklist for panoramic radiographs;
(5) an expert-oriented panoramic image-quality checklist;
(6) a CBCT image-quality checklist;
(7) a guideline on the safe use of portable dental X-ray devices;
(8) a guideline for CBCT radiation safety and usage;
(9) a CBCT quality control guideline; and
(10) a self-assessment form for dental radiation safety management systems.
A three-round Delphi survey was conducted with 11 experts selected for their clinical and academic expertise in oral and maxillofacial radiology. Survey items were rated using a 9-point Likert scale. Quantitative consensus was evaluated using three criteria: average score (AVE ≥ 8.0), coefficient of variation (CV ≤ 0.2), and agreement rate (AR ≥ 80%). In each round, qualitative feedback guided the iterative addition, deletion, or revision of items, emphasizing clinical relevance, clarity of expression, and terminological consistency.
Results
Round 1 began with 81 items; expert feedback expanded and refined the list to 93 items in Round 2 and to 95 items in Round 3. Agreement rates improved steadily from 75.3 % (61/81) in Round 1 to 83.9 % (78/93) in Round 2 and 94.7 % (90/95) in Round 3. Throughout the process, expert commentary prompted repeated reviews of clinical suitability, phrasing precision, and uniform terminology, resulting in progressive sentence revisions and vocabulary adjustments.
Conclusions
By employing the Delphi technique, this study achieved expert consensus on the validity and clarity of items addressing dental radiographic image-quality and radiation-safety management. The finalized set of 95 items offers streamlined, practical guidance for implementing effective quality and safety protocols in dental radiography.
