아토피 피부염(Atopic Dermatitis, AD)은 만성 염증성 피부 질환으로, 성인 AD 유병률과 식생활 질을 종합적으로 분석한 국내외 연구는 아직 부족한 상황이다. 본 연구는 2013-2021년의 국민건강영양조사 자료를 활용하여 한국 성인 AD 유병률과 식생활 질 간의 연관성을 분석하기 위해 수행되었다.
본 연구는 국민건강영양조사 6-8기에 참여한 69,776명 중 만 19세 이하이거나 만 65세 이상인 자(n=29,038), 아토피 피부염 의사진단 여부를 모르거나 무응답으로 답변한 자(n=4,395), 24시간 식품섭취조사에 참여하지 않은 자(n=5,503), 1일 에너지 섭취량이 500kcal 미만이거나 5,000kcal 초과인 자(n=552), 식생활평가지수 값이 결측치인 자(n=41), 임신부이거나 수유부인 자(n=464)를 제외한 총 29,783명을 최종 분석 대상으로 선정했다. 그 중 '아토피 피부염이라고 의사에게 진단을 받았음'이라고 응답한 경우를 'AD군(AD진단군, n=1,000)', '아토피 피부염이라고 의사에게 진단을 받지 않았음'이라고 응답한 경우를 'non-AD군(AD미진단군, n=28,783)'으로 정의했다. 그 후 24시간 회상법을 통해 수집된 식사 자료로부터 식품 섭취량, 에너지 및 영양소 섭취량, 식생활평가지수 총점 및 세부 항목 점수를 산출하여 AD군과 non-AD군 간 차이를 확인했다. 식생활평가지수와 AD 유병률과의 연관성은 식생활평가지수 총점과 세부 항목 점수 분포에 따라 대상자를 인원수별로 3그룹(high, middle, low)으로 나누어 다중 로지스틱 회귀분석으로 확인했다. 모든 값은 후방제거법(backward elimination)에 따라 p-value가 0.05 미만인 변수들(연령, 성, 결혼 유무)로 보정했다.
연구 대상 중 AD군이 약 3.4%를 차지하였고, 특히 AD군 중 20대 비율이 57.5%로 가장 높았다. 성인 AD 유병률은 식생활평가지수 총점이 낮은 그룹일수록 high그룹에 비해 유의하게 높았다(high vs middle, OR=1.30; high vs low, OR=1.33, p for trend=0.007). 식생활평가지수 세부 항목별로는, 아침 식사를 주 0-2회 하는 그룹(high vs low, OR=1.26), 잡곡 섭취 항목 점수가 낮은 그룹(high vs low, OR=1.26), 총 채소 섭취 항목 점수가 낮은 그룹(high vs low, OR=1.26), 당(당류·음료류) 에너지섭취비율이 높은 그룹(high vs low, OR=1.25)에서 성인 AD 유병률이 high그룹보다 유의하게 높았다(p for trend<0.05 for all). 반면, 나트륨 섭취 항목 점수가 낮은 그룹은 high그룹에 비해 성인 AD 유병률이 낮았다(high vs low, OR=0.81, p for trend=0.021). AD군은 non-AD군보다 식물성 식품, 감자·전분류, 채소류를 적게 섭취하는 반면, 음료류는 많이 섭취하는 것으로 확인되었고(p<0.05 for all), 탄수화물 에너지섭취비율, 탄수화물, 나트륨, 비타민 B1 섭취량이 적었으며, 지방 에너지섭취비율, 지방 및 지방 관련 영양소(포화지방산, 다가불포화지방, N-3 지방산, N-6 지방산) 섭취량은 많았다(p<0.05 for all).
본 연구 결과는 한국 성인 AD 유병률과 식생활 질 간에 유의한 연관성이 있음을 보여준다. 향후 종단적 연구를 통해 이들 간의 인과관계가 명확히 규명된다면, 한국 성인 AD 환자를 위한 식생활 지침 마련에 필요한 과학적 근거를 제시할 수 있을 것이다.

Atopic dermatitis (AD) is a chronic inflammatory skin disease, and few studies have comprehensively examined the association between adult AD prevalence and overall diet quality. This study aimed to investigate the relationship between AD prevalence and diet quality among Korean adults using data from the Korea National Health and Nutrition Examination Survey (KNHANES) from 2013 to 2021.
Among 69,776 participants from KNHANES phases VI–VIII, a total of 29,783 adults aged 20–64 years were included after excluding those aged under 19 or over 65 years, those with missing or unknown physician-diagnosed AD status, those who did not complete the 24-hour dietary recall, individuals with extreme energy intake (<500 or >5,000 kcal/day), those with missing KHEI scores, and pregnant or lactating women. Participants who responded "yes" to a physician diagnosis of AD were classified as the AD group (n=1,000), while those who responded "no" were classified as the non-AD group (n=28,783). Dietary intake, energy and nutrient intake, and scores on the Korean Healthy Eating Index (KHEI) were calculated based on the 24-hour dietary recall data. Multivariable logistic regression was used to assess the association between AD prevalence and diet quality based on three groups (high, middle, low) of the KHEI total and component scores. All models were adjusted for variables with a p-value<0.05, identified through backward elimination, were included in the final adjusted models (age, sex, and marital status).
AD prevalence was 3.4% among the study population, with the highest proportion in participants in their 20s (57.5%). AD prevalence was significantly higher in groups with lower KHEI total scores than in the high-score group (high vs. middle, OR=1.30; high vs. low, OR=1.33, p for trend=0.007). Among KHEI components, AD prevalence was significantly higher in groups with infrequent breakfast consumption (0–2 times/week) (high vs low, OR=1.26), lower mixed grain intake (high vs low, OR=1.26), lower total vegetable intake (high vs low, OR=1.26), and higher percentage of energy from sugar(sugars and beverages) (high vs low, OR=1.25) than in the high-score group (p for trend<0.05 for all). Conversely, the group with low sodium intake scores had a lower prevalence of adult AD than in the high-score group (high vs. low, OR=0.81, p for trend=0.021). Compared to the non-AD group, the AD group consumed less plant-derived foods, potatoes and starches, and vegetables, while consuming more beverages (p<0.05 for all). In addition, the AD group showed a significantly lower percentage energy from carbohydrate and a higher percentage energy from fat (p<0.05 for all). They also had lower absolute intake of carbohydrates, sodium, and vitamin B1, but higher intake of total fat, including saturated fatty acids, polyunsaturated fatty acids, and both N-3 and N-6 fatty acids (p<0.05 for all).
These findings suggest a significant association between AD prevalence in Korean adults and diet quality. Further longitudinal studies are needed to establish causal relationships, which may provide scientific evidence for developing dietary guidelines tailored to Korean adults with AD.
