    연구 배경: 호흡 주기 분석은 호흡기 질환 진단과 치료 효과 모니터링의 핵심 요소이다. 비정상적인 호흡 패턴은 천식, 만성 폐쇄성 폐질환, 폐렴 등 주요 호흡기 질환의 진단 및 중증도 평가에 활용되는 중요한 지표이다. 전통적인 청진은 의료진의 숙련도에 의존하여 진단 일관성 확보에 어려움이 있으며, 기존의 딥러닝 기반 폐 청진음 분석 연구들은 주파수 영역 변환 과정에서 원신호의 시간적 정보 손실과 하이퍼파라미터 의존성이라는 한계를 지닌다.
    연구 방법: 본 연구에서는 원시 폐 청진음 데이터를 기반으로 가변 길이의 호흡 주기를 예측하는 LUNAR (LUNg Auscultation Respiratory detector) 프레임워크를 제안하였다. 본 모델은 시간적 자기 유사성 매트릭스를 활용하여 호흡의 주기적 특성을 포착하는 RPAM (Respiratory Periodicity Awareness Module)을 도입하였으며, YOLO 기반의 객체 탐지 기법을 채택하였다. 성인 환자의 폐 청진음 데이터셋 HF_Lung_V1으로 모델을 학습하고, ICBHI 및 서울대학교어린이병원 소아 데이터셋으로 파인튜닝을 수행하였다.

    Background: The analysis of the respiratory cycle is pivotal in the diagnosis of respiratory diseases and the evaluation of treatment efficacy. Abnormal breathing patterns serve as a crucial indicator for diagnosing and assessing the severity of respiratory diseases, including asthma, chronic obstructive pulmonary disease, and pneumonia. Traditional auscultation methods rely on clinician expertise, which may result in inconsistent diagnoses. Existing deep learning-based lung sound analysis studies are limited by temporal information loss and hyperparameter dependency during the frequency domain transformation process from raw signals.
    Methods: This study proposes the LUNAR (LUNg Auscultation Respiratory detector) framework for predicting variable-length respiratory cycles based on raw lung sound data. The model incorporates a novel RPAM (Respiratory Periodicity Awareness Module) that captures the periodic characteristics of respiration using a temporal self-similarity matrix and adopts a YOLO-based object detection approach. The model was trained on the HF_Lung_V1 dataset comprising lung sounds from adult patients and fine-tuned on the ICBHI dataset and pediatric outpatient data from Seoul National University Children's Hospital.
    Results: The proposed framework demonstrated stable performance across diverse age groups and clinical environments. On the HF_Lung_V1 dataset, the proposed model achieved Average Precision (AP) of 0.880 and F1-score of 0.894. The RPAM module consistently contributed to performance improvements of 12.0% in AP and 6.3% in F1-score across various backbone networks. Direct processing of raw signals proved more effective than frequency domain transformation approaches.
    Conclusion: This study proposes an end-to-end framework that automatically detects variable-length respiratory cycles by effectively capturing the periodic characteristics of raw lung auscultation sounds. The framework demonstrates that stable prediction capability is possible by minimizing information loss through direct analysis of raw signals without frequency transformation and by explicitly modeling respiratory periodicity. The proposed model can accurately analyze respiratory cycles, complementing the limitations of traditional subjective auscultation methods. Additionally, it is expected to contribute to respiratory disease research through automated analysis of large-scale lung auscultation data, enabling investigation of correlations between breathing patterns and diseases, as well as treatment efficacy evaluation.
