    일시적 수용체 전위 (Transient receptor potential, 이하TRP) 채널은 다양한 감각 자극, 특히 통각 자극의 감지에 관여하는 핵심 전달자로 작용한다. 이러한 채널은 말초 감각 신경세포에서 높은 발현을 보이며, 외부의 환경적 신호를 감지하는 데 중요한 역할을 수행한다. 예를 들어, TRPV1과 TRPA1은 각각 유해한 열 자극 및 화학적 자극의 감지에 관여하며, TRPM8은 무해한 차가운 자극(냉각 자극)의 인식에 관여한다. 해당 TRP 채널의 활성화는 통각을 유도성 효과(TRPV1 및 TRPA1) 또는 진통 효과(TRPM8)를 나타낼 수 있다는 것이 잘 알려져 있다. 따라서 TRP 채널을 매개로 한 통각 및/또는 진통 메커니즘의 분자적 및 세포적 기전을 규명하는 것은 효과적인 통증 치료법 개발에 있어 필수적이다.

    제1장에서는 신규 항암제인 레이저티닙(lazertinib)에 의해 유도되는 이상감각의 말초 기전을 탐구하였다. 생체 내 칼슘 이미징을 통해, 레이저티닙이 소형 및 중형 크기의 마우스 감각 신경세포 중 일부 부분 집단에서 자발적인 칼슘 반응을 유도함을 확인하였다. 또한, 전기생리학적 기록을 통해, 레이저티닙은 TRPV1 계열 감각 신경세포의 자발적 발화 및 신경세포의 흥분성을 증가시켰으며, TRPA1 반응을 민감화시킴을 확인하였다. 레이저티닙에 의해 유도된 칼슘 반응은 TRPA1 길항제인 HC-030031 및 아데닐릴 고리화효소(adenylyl cyclase) 억제제인 MDL-12330A에 의해 약리학적으로 억제되었으며, 이는 레이저티닙이 아데닐릴 고리화효소 신호경로를 통한 간접적인 TRPA1 활성화를 통해 감각 신경세포를 활성화시킬 수 있음을 시사한다.

    제2장에서는 TRPM8 매개 진통 현상에 관여하는 중추신경계 회로를 분석하였다. 냉각 감각을 모방하는 TRPM8 작용제인 이실린(icilin)을 피부에 도포한 결과, 마우스 모델에서 유의한 진통 효과가 유도됨을 관찰하였다. 냉각 자극은 온도 감지 및 하행성 통증 억제에 관여하는 상위 뇌 구조들인 가측 부완핵 (lPBN), 복외측 중뇌수도관회색질 (vlPAG), 그리고 앞복외측연수 (RVM)—을 활성화시킴을 확인할 수 있었다. 유전학적 방법론을 사용, 냉각에 반응하는 lPBN 신경세포와 이들의 vlPAG로의 신경 투사체를 선택적으로 활성화한 결과, 엔케팔린 분비 및 vlPAG 내 국소 오피오이드 수용체 신호를 통해 진통 효과가 매개되었음을 확인하였다. 또한 냉각 반응성 lPBN 신경세포의 활성화는 유해 자극에 대한 척수 신경세포의 활성도를 유의하게 감소시켰으며, 이는 냉각 유도 진통 현상에 있어 하행성, 상위 조절 통증 억제 회로의 관여를 나타낸다.

    Transient receptor potential (TRP) channels serve as a key transducer molecule involved in the detection of diverse sensory stimuli, including nociceptive signals. These channels are abundantly expressed in peripheral sensory neurons, and play a crucial role in detecting environmental cues. For instance, TRPV1 and TRPA1 are involved in sensing of noxious heat and noxious chemicals, respectively, and TRPM8 is involved in detection of innocuous cold (cooling). Activation of TRP channels could be pronociceptive (e.g., TRPV1 and TRPA1) or antinociceptive (e.g., TRPM8). Therefore, elucidating the molecular and cellular mechanisms underlying TRP channel-mediated nociception and/or antinociception is essential for the development of effective pain therapies. Despite their well-recognized roles, the involvement of TRPA1 in chemotherapeutic agent-induced paresthesia remains poorly understood. Moreover, cold-induced analgesia, likely mediated by TRPM8, is suggested to involve central nervous system mechanisms through endogenous opioid signaling. However, direct evidence supporting the role of central opioidergic neural circuits underlying cold-induced analgesia remains limited. This study investigated the mechanism of TRP channels in chemotherapeutic agent-induced paresthesia and centrally mediated analgesia.

    In the first chapter, the peripheral mechanisms underlying paresthesia induced by the novel chemotherapeutic agent lazertinib was investigated. Through in vitro calcium imaging and electrophysiological recordings, I found that lazertinib induces spontaneous calcium responses in a subset of small-to-medium-diameter murine sensory neurons. Lazertinib also induced spontaneous firings and heightened excitability in a subset of TRPV1-lineage sensory neurons and sensitized transient receptor potential ankyrin 1 (TRPA1) response. Lazertinib-induced calcium responses were pharmacologically blocked with HC-030031 (TRPA1 antagonist) and MDL-12330A (adenylyl cyclase inhibitor), suggesting that lazertinib activates sensory neurons through indirect activation of TRPA1 via adenylyl cyclase signaling pathways.

    In the second chapter, the central neural circuitry involved in TRPM8-mediated analgesia was examined. I found that applying the cold-mimicking TRPM8 agonist icilin to the skin produced a significant antinociceptive effect in a mouse model. Cold stimuli activated multiple supraspinal regions involved in thermal perception and descending pain inhibition, such as the lateral parabrachial nucleus (lPBN), ventrolateral periaqueductal gray (vlPAG) and rostral ventrolateral medulla (RVM). Selective activation of cold-responsive lPBN neurons and their neuronal projection to the vlPAG elicited significant analgesic effects, which were mediated by enkephalin release and local opioid receptor signaling in the vlPAG. Activation of cold-responsive lPBN neurons significantly reduced spinal cord neuronal activity in response to noxious stimuli, indicating the involvement of top-down descending pain modulation in cold-induced analgesia.

    Collectively, these experimental findings provide a comprehensive understanding of the role of TRP channels involved in both nociceptive and antinociceptive processes, from molecular to circuit-levels, highlighting a potential target for novel pain treatment strategy.
