    배경: COVID-19 팬데믹은 급성기 감염 이후에도 신경학적 또는 혈전색전성 합병증을 유발할 수 있으며, 회복 이후에도 다양한 롱코비드 증상이 나타날 수 있다. 특히 기저질환을 가진 고위험군의 경우, 팬데믹을 거치며 경험한 건강 위험과 의료비용 변화에 대한 구조적이고 장기적인 영향 평가가 필요하다. 기존 연구들은 감염에 따른 단기적 건강 위험에 주로 초점을 맞추어 왔으나, SARS-CoV-2 감염, 백신 접종, 그리고 팬데믹이라는 보건의료적·사회적 맥락이 중장기적으로 건강에 미친 영향을 종합적으로 규명한 연구는 드물다. 이에 본 연구는 한국의 전국민 단위 자료를 활용하여 두 가지 주제를 중심으로 분석을 수행하였다: (1) 롱코비드 관점에서 SARS-CoV-2 감염 후 안면신경마비(Bell’s palsy, BP), 폐색전증(pulmonary embolism, PE), 심부정맥혈전증(deep vein thrombosis, DVT) 발생 위험 평가, (2) 주요 기저질환 보유자의 팬데믹 전후 초과사망 및 의료비용 변화 분석.
    방법: 국민건강보험공단과 질병관리청의 연계 자료(K-COV-N)를 기반으로 후향적 코호트 연구를 수행하였다. 제1장에서는 Fine and Gray의 경쟁위험모형과 Cox 비례위험모형을 활용하여 SARS-CoV-2 감염과 BP, PE, DVT 발생 간의 연관성을 분석하였다. 제2장에서는 10대 주요 기저질환(만성폐질환, 심장질환, 뇌졸중, 당뇨병, 고혈압, 만성간질환, 자가면역질환, 혈액투석이 필요한 만성신장질환, 치매·파킨슨병, 암)을 보유한 환자를 대상으로 팬데믹 이전(2017–2019)과 이후(2020–2022) 기간의 초과사망 및 의료비용 변화를 로지스틱 회귀모형과 음이항 회귀모형으로 비교 분석하였다. 분석 과정에서는 백신 접종력, 연령, 성별, 가구소득, 동반질환지수, 감염 시 중증도 등 주요 인구학적 및 임상적 요인을 고려하였다.
    결과: SARS-CoV-2 감염은 BP, PE, DVT의 발생 위험 증가와 유의한 연관이 있었으며, 특히 PE와 BP의 경우 백신을 완전 접종한 이후에도 일정 수준의 위험이 지속되어 롱코비드 질환으로서의 가능성을 뒷받침하였다. 팬데믹 전후 초과사망 및 의료비용 변화 분석에서는 기저질환군별로 상이한 양상이 확인되었다. A군(만성폐질환, 심장질환, 만성간질환, 자가면역질환)은 사망률과 의료비용이 모두 증가(또는 유지)하여 전반적으로 질병의 중증도가 높고 의료 자원 소모가 큰 임상적 특성을 보였다. B군(뇌졸중, 혈액투석이 필요한 만성신장질환, 치매·파킨슨병)은 사망률은 증가하였으나 의료비용은 오히려 감소하는 양상을 보여 의료 이용 접근성 저하 또는 돌봄 공백의 가능성을 시사하였다. C군(당뇨병, 고혈압, 암)은 사망률은 감소하고 의료비용은 증가(또는 유지)하였으며, 이는 원격진료, 의약품 배송 등 적응적 진료모형의 활용과 정책적 지원의 효과일 가능성을 보여준다.

    Introduction: The Coronavirus Disease 2019 (COVID-19) pandemic has led to long-lasting health consequences beyond the acute infection phase, including neurologic and thromboembolic complications. Additionally, high-risk populations such as individuals with underlying conditions may have experienced significant health threats and changes in healthcare utilization throughout the pandemic. While previous research has largely focused on short-term outcomes related to SARS-CoV-2 infection, comprehensive evaluations of the long-term health effects of the infection, vaccination, and the broader healthcare and social context of the pandemic remain limited. To address these gaps, this study analyzed nationwide data from South Korea to examine two key objectives: (1) to assess the risk of Bell’s palsy (BP), pulmonary embolism (PE), and deep vein thrombosis (DVT) following SARS-CoV-2 infection as potential manifestations of Long COVID; and (2) to compare changes in excess mortality and healthcare costs among individuals with major chronic conditions before and during the pandemic.
    Methods: We conducted a nationwide retrospective cohort study using linked data from the National Health Insurance Service and the Korea Disease Control and Prevention Agency (K-COV-N database). In Chapter 1, we analyzed the association between SARS-CoV-2 infection and the incidence of BP, PE, and DVT using Fine and Gray’s competing risk model and Cox proportional hazards regression. In Chapter 2, we investigated trends in excess mortality and healthcare costs from 2017 to 2022 among patients with ten major underlying conditions: chronic lung disease, heart disease, stroke, diabetes, hypertension, chronic liver disease, autoimmune disease, chronic kidney disease requiring dialysis, dementia/Parkinson’s disease, and cancer. Logistic regression and negative binomial regression were applied to compare pre-pandemic (2017–2019) and pandemic (2020–2022) periods. All analyses accounted for vaccination status, age, sex, household income, and severity of infection.
    Results: SARS-CoV-2 infection was significantly associated with increased risks of BP, PE, and DVT. Although complete vaccination attenuated the risks, particularly for PE and BP, a residual risk remained, underscoring the need for continuous monitoring and individualized follow-up. The analysis of excess mortality and healthcare costs revealed heterogeneous effects across disease groups. Group A (chronic lung disease, heart disease, chronic liver disease, autoimmune disease) experienced increases in both mortality and healthcare costs, suggesting high disease severity and resource intensity. Group B (stroke, chronic kidney disease requiring dialysis, dementia/Parkinson’s disease) showed increased mortality but decreased healthcare costs, indicating potential disruptions in access to care or care discontinuity. In contrast, Group C (diabetes, hypertension, cancer) showed decreased mortality alongside increased or sustained healthcare costs, possibly reflecting the adoption of adaptive care models such as telemedicine and home-based medication delivery, supported by healthcare policies.
    Conclusion: This study highlights that the impact of COVID-19 extends beyond acute infection, contributing to long-term neurologic and vascular complications and structural shifts in healthcare utilization, particularly among vulnerable populations. Persistent sequelae despite full vaccination emphasize the importance of targeted surveillance and sustained clinical follow-up. Moreover, the decoupling of mortality and healthcare costs observed in some disease groups suggests the need for policy interventions to address care discontinuity and unmet needs. By integrating analyses of Long COVID-related complications with disease-specific trends in mortality and healthcare costs, this study provides valuable evidence to inform future pandemic preparedness and the development of targeted health policies for high-risk populations.
