방법: 총 90개의 우치(법랑질) 시편을 준비하여 각각 홍차, 커피, 적포도주 용액에 총 7일간 침지하였다. 착색이 완료된 시편은 표준 슬러리(reference slurry), 연마제를 기반으로 한 상업용 치약, 그리고 과산화수소를 포함한 미백 치약의 세 가지 그룹으로 나누어 칫솔질 기기를 이용해 총 10,000회 자동 칫솔질하였다. 칫솔질 전후의 색상 변화는 분광광도계를 이용해 CIEDE2000(ΔE₀₀) 방식으로 측정하였고, 표면 거칠기(Ra)는 3D 표면 형상 측정 장비를 사용하여 정량화하였다. 착색의 침투 깊이는 시편을 단면 절단한 후, 미세분광광도계를 통해 법랑질과 상아질 층 각각에서 색상 차이를 측정하여 분석하였다. 통계 처리는 이원 분산분석(two-way ANOVA)과 Tukey 사후 검정을 사용하여 그룹 간 차이를 평가하였다.
결과: 음료 종류는 착색 제거 효과에 유의한 영향을 미쳤으며(p < 0.001), 음료와 치약 유형 간에는 강한 상호작용이 나타났다(p < 0.001). 연마제 기반 치약(표준 슬러리, 화이트 스파클)은 적포도주 착색 제거에 더 효과적이었으며, 과산화수소 기반 치약(뷰센28)은 차와 커피 착색 제거에 더 뛰어난 성능을 보였다. 칫솔질 후 법랑질 표면 거칠기는 증가하였고, 특히 적포도주로 착색된 시편에서 가장 큰 변화가 관찰되었다(p < 0.001). 단면 분석 결과 홍차에 의한 착색은 법랑질과 상아질 깊숙이 침투하였으며, 적포도주에 의한 착색은 주로 표면에 국한되고 불균일하게 분포하는 경향을 보였다. 미백 효과는 치약의 구성 성분과 착색 물질의 특성에 따라 달라진다. 적포도주와 같은 표면 착색은 연마제 기반 치약에 더 잘 반응하였으며, 차와 커피처럼 깊이 침투한 착색은 과산화수소 치약으로 더 효과적으로 제거되었다. 이러한 결과는 착색의 종류와 깊이에 따라 맞춤형 미백 접근이 필요함을 시사한다.

Purpose: Discoloration of teeth due to external factors—particularly from beverages like tea, coffee, and red wine—is a frequent aesthetic issue encountered in dentistry. The performance of whitening toothpastes in removing such stains may vary depending on both the chemical properties of the staining substances and the specific ingredients in the toothpaste. Furthermore, differences in how deeply the stains penetrate and adhere to the tooth surface can also influence the whitening effect. This study investigated the performance of various whitening toothpastes in removing stains caused by different beverages, with additional analysis of their impact on enamel surface roughness and the distribution of stains within tooth layers.
Methods: Ninety bovine enamel specimens were prepared and immersed in black tea, coffee, or red wine for 7 days to induce staining. Following immersion, the specimens were brushed with the following toothpastes: a reference slurry, an abrasive-based whitening toothpaste, and a hydrogen peroxide-based whitening toothpaste. A total of 10,000 brushing strokes were applied using a standardized brushing simulator. Color change (ΔE₀₀) was measured using a spectrophotometer, and surface roughness (Ra) was evaluated with a 3D surface profiler, both before and after brushing. Stain penetration depth was assessed through cross-sectional color measurements of enamel and dentin layers using a microspectrophotometer. Statistical analysis was performed using two-way ANOVA followed by Tukey’s post hoc test.
Results: The type of beverage significantly influenced stain removal efficacy (p < 0.001), with a strong interaction observed between beverage and toothpaste types (p < 0.001). Abrasive formulations (Reference slurry and White sparkle) were more successful in removing wine stains, while the hydrogen peroxide-based toothpaste (Vussen) proved more effective for tea and coffee discoloration. Surface roughness increased after brushing, with the greatest changes occurring in wine-stained samples (p < 0.001). Cross-sectional analysis revealed deeper penetration of tea stains into enamel and dentin, while wine stains tended to be more superficial and unevenly distributed.
The success of whitening treatments is influenced by both the composition of the toothpaste and the characteristics of the staining agent. Surface stains, such as those from wine, responded better to abrasive toothpastes, whereas deeper stains from tea and coffee were more effectively addressed by hydrogen peroxide. These results emphasize the need to customize whitening approaches based on the type and location of discoloration.
