본 연구는 노인요양시설에 입소한 노인의 돌봄 서비스 이용 경험에 관한 현상학적 연 구이다. 본 연구는 노인요양시설에 입소한 노인들의 관점에서 돌봄 서비스 이용 경험을 파악하였다. 이를 통해 좋은 돌봄은 무엇이고, 좋은 돌봄이 되기 위한 요건은 무엇인지 탐색하여 노인요양시설 입소 노인의 삶의 질 향상을 위한 방안을 제시하고자 하는 데 목적이 있다. 본 연구는 “노인요양시설 입소 노인이 경험한 돌봄 서비스 이용 경험은 어 떠한가?”라는 질문을 가지고 연구를 수행하였다. 본 연구에 참여한 참여자는 노인요양시 설 두 곳에 입소해 있는 노인 10명이다. 65세 이상의 의사소통과 인지 기능이 가능한 노인을 대상으로 의도적 표집 방법을 사용하였다. 참여자의 선정은 시설 기관장의 소개 로 선정하였다. 참여자에게 연구 목적과 연구 방법을 설명한 후 참여 의사를 확인한 후 연구 동의서를 받고 심층 면담을 진행하였다. 면접 횟수는 2회이고, 면접 시간은 1회 15분~1시간 정도 소요되었다. 면담 내용은 녹취하여 문서화하였다. 심층 면담 기간은 2023년 3월에서 2023년 4월 말까지이다. 추가 자료는 자료 분석 과정에서 전화, SNS 를 통해 이루어졌다. 자료 분석 방법은 지오르지의 현상학적 방법을 적용하여 수행하였 다. 연구의 분석 결과 노인요양시설 입소 노인의 돌봄 서비스 이용 경험에 대한 의미 단 위 88개, 주제 20개, 중심 의미 7개가 도출되었다. 7개의 중심 의미는 다음과 같다. 보 살핌 속에 머무는 평온한 일상, 나다움이 허용되는 생활, 홀로서기는 삶의 활력소, 따뜻 한 여운과 울림이 있는 관계, 소심하고 수동적인 시설 생활, 꽉 닫힌 벽같이 고립된 생 활, 서러운 시설 생활이다. 이런 경험들의 구체적인 분석 결과를 정리하여 제시하고자 한다. 참여자들은 노인요양시설에 가족들에 의하여 입소하였다. 참여자들은 입소 후 처음에 는 집과 가족, 친구 등 관계로부터 멀어져 당황하고 속상하였다. 노인요양시설이 낯설고 모든 것이 마음에 안 들어 답답하고 불안하였다. 반면에 시설이 깨끗하고 직원들이 잘 보살펴 주어 편하고 좋다고 느끼는 참여자들이 있다. 참여자들은 노인요양시설에서 음식을 제공해 주고, 신체활동을 도와주고, 간호・의료 처치를 해 주고, 감염 예방을 해주는 평온한 일상 속에 살고 있다. 직원들과의 따뜻한 여운과 울림이 있는 관계 속에서 친밀감을 형성하니 가족적인 분위기가 행복하였다. 참 여자들은 노래, 미술, 게임 등 프로그램에 참여하니 즐겁고 힐링이 되었다. 재활과 홀로 서기는 삶의 활력소가 되고, 직원과 운영자의 지극한 정성과 리더십은 생활 만족을 느 끼게 하였다. 참여자들은 직원, 동료 노인들을 의지하고 이해하며 적응해 갔다. 노인요 양시설 생활은 안락하고 안정감을 주었다. 반면에 참여자들은 개별성과 자율성이 미흡한 돌봄을 제공받고 있다. 직원과의 소통 과 공감은 별로 없고, 심신의 아픔으로 인한 고통, 타인과의 외롭고 괴로운 공존, 세심 한 배려 부족으로 불편하였다. 직원의 직무 유기로 인한 자존감 상실 등 서러운 시설 생활은 외롭고 힘들었다. 결론적으로 참여자들의 경험은 편하고 안락하면서 불편하고 고통스럽기도 하여 이중 감정이 오가는 빛과 그림자가 함께 공존하는 생활이라 할 수 있다. 이상과 같은 연구결과를 통해서 돌봄은 일상생활이 어려운 노인을 불편함 없이 편안 하게 보살피는 것이다. 좋은 돌봄은 심신이 취약한 노인일수록 따뜻한 말, 진정성, 전문 성을 갖추고, 돌봄의 강도를 높여 편하고 안전하게 서비스를 제공하는 것이다. 좋은 돌 봄은 노인이 소질을 발휘하도록 역할과 동기를 부여하여야 한다. 좋은 돌봄은 시설 환 경이 깨끗하고 안전하며, 개별성을 배려하고 인권을 보호하여 신체적, 정서적으로 건강 하고 평온한 삶을 유지하고 향상시키는 것이다. 그러기 위해서는 당사자, 운영자, 직원, 가족 등 사회 구성원들은 네트워크를 이루어 상생하며 함께 공존하여야 한다. 본 연구결과를 토대로 선행연구와 비교하여 논의한 후 제언을 제시하고자 한다. 실천 적 제언으로는 신체적・정서적 차원에서 첫째, 노인의 욕구를 고려한 개별적인 맞춤 서 비스 제공이 이루어져야 한다. 둘째, 선택권과 자기 결정권을 활용하도록 기회와 용기를 주어야 한다. 셋째, 개별적이고 다양한 노인 중심형 프로그램 개발과 공급이 이루어져야 한다. 넷째, 휴머니튜드에 기반한 의사소통 돌봄이 필요하다. 다섯째, 종사자의 전문성 강화와 점검이 이루어져야 한다. 여섯째, 다 직종 간 연계・협력 체계로 서비스 강화가 필요하다. 일곱째, 운영자와 종사자의 노인 인권 보호 교육 강화와 점검이 강화되어야 한다. 사회 환경적 차원에서 첫째, 운영자의 노인 중심적인 사고와 탁월한 운영능력 발 휘가 필요하다. 둘째, 안전 보장, 청결한 생활환경이 조성되어야 한다. 셋째, 사회관계망 과의 연계・교류가 활성화되어야 한다. 정책적 제언으로 첫째, 노인 돌봄 자립 원조 정책 이 강구되어야 한다. 둘째, 돌봄 종사자 증원 정책 및 제도 방안과 지원이 구축되어야 한다. 셋째, 개별 케어매니저 제도 도입이 필요하다. 넷째, 공공 노인요양시설이 확대되 어야 한다.

This study is a phenomenological study on the experience of using care services for the elderly in nursing homes. This study identified the experience of using care services from the perspective of the elderly residing in nursing homes. Through this, the purpose is to explore what good care is and what are the requirements for good care, and to suggest ways to improve the quality of life of the elderly in nursing homes. This study was conducted with the question, “What are the experiences of using care services experienced by the elderly in nursing homes?” The participants in this study were 10 elderly people in two nursing homes. The purposeful sampling method was used to target the elderly who were 65 years of age or older and capable of communication and cognitive functions. The participants were selected through an introduction by the director of the facility. After explaining the purpose and method of the study to the participants, their willingness to participate was confirmed, and a consent form was obtained and an in-depth interview was conducted. The number of interviews was 2, and each interview took about 15 minutes to 1 hour. The interview content was recorded and documented. The in-depth interview period is from March 2023 to the end of April 2023. Additional data was obtained through phone calls and SNS during the data analysis process. The data analysis method was conducted using Giorgi's phenomenological method. As a result of the analysis of the study, 88 meaning units, 20 themes, and 7 central meanings were derived regarding the experience of using care services for the elderly in nursing homes. The 7 central meanings are as follows: a peaceful daily life in care, a life where one's own identity is allowed, standing alone is a source of vitality in life, a relationship with warm aftertaste and resonance, a timid and passive facility life, an isolated life like a tightly closed wall, and a sad facility life. I would like to organize and present the specific analysis results of these experiences. The participants were admitted to the nursing home by their families. At first, the participants were confused and upset because they were separated from their homes, families, and friends. The nursing home was unfamiliar and everything was unpleasant, so they felt suffocated and anxious. On the other hand, there are participants who feel comfortable and good because the facility is clean and the staff takes good care of them. Participants live in a peaceful daily life where food is provided, physical activities are assisted, nursing and medical treatment are provided, and infection prevention is provided. The family-like atmosphere was happy because they formed intimacy in a warm and resonant relationship with the staff. The participants enjoyed and healed by participating in programs such as singing, art, and games. Rehabilitation and standing alone became a source of vitality in life, and the extreme sincerity and leadership of the staff and operators made them feel satisfied with their lives. The participants relied on and understood the staff and fellow elderly people and adapted. Life in the nursing facility was comfortable and stable. On the other hand, the participants were receiving care that lacked individuality and autonomy. There was little communication and empathy with the staff, and they were uncomfortable due to the pain caused by physical and mental pain, lonely and painful coexistence with others, and lack of meticulous consideration. The sad life in the facility was lonely and difficult, such as the loss of self-esteem due to the staff's dereliction of duty. In conclusion, the participants’ experiences can be described as a life in which light and shadow coexist-a blend of comfort and ease, yet also discomfort and pain, marked by ambivalent emotions. Through the research results above, care is to comfortably care for the elderly who have difficulty in daily life without inconvenience. Good care is to provide services comfortably and safely by providing warm words, sincerity, and professionalism to the elderly who are physically and mentally vulnerable, and by increasing the intensity of care. Good care should provide roles and motivation for the elderly to demonstrate their potential. Good care is to maintain and improve a physically and emotionally healthy and peaceful life by keeping the facility environment clean and safe, considering individuality and protecting human rights. To do so, the parties involved, operators, employees, families, and other members of society should form a network and coexist in a mutually beneficial manner. Based on the results of this study, I would like to present suggestions after comparing them with previous studies. As practical suggestions, first, individually tailored services should be provided that consider the needs of the elderly in terms of physical and emotional aspects. Second, opportunities and courage should be given to utilize the right to choose and self-determination. Third, individual and diverse elderly-centered programs should be developed and supplied. Fourth, communication care based on humanity is necessary. Fifth, the professionalism of workers should be strengthened and inspected. Sixth, it is necessary to strengthen services through a multi-professional linkage and cooperation system. Seventh, the protection of the human rights of the elderly by operators and workers should be strengthened and inspections should be strengthened. In terms of the social environment, first, the operator's elderly-centered thinking and excellent operational ability should be demonstrated. Second, safety guarantee and a clean living environment should be created. Third, connection and exchange with social networks should be activated. As a policy suggestion, first, a policy to support the independence of elderly care should be devised. Second, a policy and institutional plan and support for increasing the number of care workers should be established. Third, an individual care manager system should be introduced. Fourth, public elderly care facilities should be expanded.
